Acute bilateral corneal calcification after alkali burn

RESUMO

Relatamos um caso de calcificação corneana bilateral desenvolvida 4 dias após uma queimadura por álcali. Na ocasião da calcificação o paciente apresentava estroma corneano exposto, com processo inflamatório em atividade e vinha fazendo uso freqüente de lágrimas artificiais com preservativos. Seus níveis de cálcio e fósforo séricos eram normais. Acreditamos que a queimadura alcalina e conseqüente elevação do pH tenha favorecido o rápido depósito de cálcio no estroma corneano deste paciente. Resultados favoráveis foram obtidos através de uma ceratectomia superficial associada a um transplante de conjuntiva e limbo em ambos os olhos, em um primeiro tempo, e posterior transplante de córnea no olho direito. Estudos histopatológicos do material corneano excisado revelaram depósito de cálcio extracelular.

SUMMARY

This case report describes the development of an acute bilateral corneal calcification 4 days after an alkali burn. At the time of the corneal calcium deposition, the patient had an acute inflammatory process of the exposed corneal stroma, and was in frequent use of artificial tears with preservatives. Serum calcium, phosphate and carbon dioxide levels were all within normal limits. We believe that the local increase in pH, secondary to the alkali burn, favored the rapid corneal calcification. Successful results were achieved with superficial keratectomy and conjunctival transplantation in both eyes and posterior corneal graft in the right eye. Histologic studies of the superficial cornea disclosed extracellular calcium deposit.
