Estudo comparativo da eficácia clínica e microbiológica da associação de ciprofloxacina 0,3% e dexametasona 0,1%, versus gentamicina 0,3% e dexametasona 0,1%, no tratamento de conjuntivites bacterianas agudas ⁺

Evaluation of the clinical and microbiological efficacy of 0.3% ciprofloxacin and 0.1% dexamethasone drops versus 0.3% gentamicin and 0.1% dexamethasone drops in the treatment of acute bacterial conjunctivitis

RESUMO

Foi realizado um estudo clínico, duplo mascarado e randomizado, compreendendo 83 pacientes com diagnóstico de conjuntivite aguda de etiologia presumivelmente bacteriana, dos quais 53 apresentavam cultura positiva para bactérias.

Os resultados dos testes de sensibilidade in vitro não mostraram associação entre os antibióticos testados e resistência dos agentes etiológicos S. aureus, S. epidermidis e Streptococos sp. Considerando-se as cepas de S. aureus e S. epidermidis, não houve diferença entre o efeito de negativação das culturas de uma associação de drogas com a outra.

Os valores obtidos de eficácia microbiológica e clínica com o uso de ciprofloxacina-dexametasona foram respectivamente 84,6% e 92,3% contra 70,4% e 81,5% da combinação de gentamicina e dexametasona. Embora os resultados fossem favoráveis à associação de ciprofloxacina e dexametasona, essas diferenças não resultaram significativas.

Não ocorreram reações adversas graves com o uso de qualquer das formulações. Efeitos colaterais ocorreram em 7,1 % (3 de 42 pacientes) dos casos medicados com ciprofloxacina e dexametasona (erosão epitelial corneana, ceratite puntiforme epitelial e depósito de cristais nos cílios) e em 9,8% (4 de 41 pacientes) dos casos tratados com gentamicina e dexametasona (erosão epitelial corneana, ceratite puntiforme epitelial).

SUMMARY

A clinic, randomized and double masked study was conducted in 83 patients with presumed acute bacterial conjunctivitis (53 of them culture-proven cases of bacterial conjuntivitis).

Regarding the causative agents - S. aureus, S. epidermidis and Streptococos sp - the statistical analysis of the two antibiotics didn 't show significant difference concerning in vitro sensibility. Regarding the agents - S. aureus and S. epidermidis - there was no significant difference in bacterial erradication with the use of two combinations of drugs.

The comparison of the effect of the combinations of the drugs on microbiological and clinical efficacy demonstrates that there is no significant difference between them. The microbiological and clinical rate from the association of ciprofloxacin and dexamethasone was respectively 84.6% and 92.3% and from gentamicin and dexamethasone was respectively 70.4% and 81.5%.

No severe adverse reaction was detected - 7.1% (three cases) of the patients who received ciprofloxacindexamethasone had punctate epithelial erosions, puntacte epithelial keratitis and crystalline deposits on ciliae. 9.8% (four cases) of the patients who were given gentamycin-dexamethasone had punctate epithelial erosions or punctate epithelial keratitis.
