ABSTRACT: This study analyses the social representations of surgical patients, specially those representations which can cause feelings of fear. The theory of Social Representation was the theoretical basis used in this study, supporting the definition of the objectives and the concepts of fear and nursing pre-surgical procedures. The methodology adopted to analyze the qualitative data was the unstructured interview content analysis. Forty patients were interviewed in a large Brazilian hospital in Santa Maria -RS. Those patients were undergoing medium complexity surgeries and non-emergency pre-surgical procedures. Twenty-four of the patients did not use a sleep inductor for the surgery. The data was codified according to the record structure. This procedure enabled the classification of social representations of the surgical patients. Results showed that fear was the preponderant factor of the social representations, due to lack of information. Confidence in the system, the religious aspect, and the information provided to the patient were identified as the main reasons for the lack of fear. Based on the results of this study, some considerations were made regarding nursing presurgical procedures, in order to clarify patients' understanding of their situation, and minimize the feeling of fear.
