Contribuição ao estudo dos efeitos locais do Tiotepa no pós-operatório do pterígio^*

Evaluation of Thio-tepa's local effects after pterygium excision

RESUMO

Determinaram-se alterações dos epitélios corneano e conjuntival e do filme lacrimal em 40 pacientes com diagnóstico clínico de pterígio, distribuídos ao acaso em 2 grupos (grupo sem Tiotepa e grupo com Tiotepa), através de biomicroscopoa, estesiometria corneana, do teste de Schirmer I, do tempo de rompimento do filme lacrimal pré-corneano e do teste do rosa bengala.

No grupo sem Tiotepa, um olho foi submetido à excisão do pterígio e recebeu no pós-operatório colírio, com a associação de dexametasona 1 mg e cloranfenicol 5 mg, 1 gota 3 vezes o dia até o 25º, tendo o olho contralateral servido de controle. Variações significativas de estesiometria corneana e teste do rosa bengala ocorreram apenas no 15º dia pós-operatório e foram imputadas ao trauma cirúrgico.

No grupo com Tiotepa, um olho foi submetido à excisão do pterígio e recebeu pós-operatório colírio, com a associação de dexametasona 1 mg e cloranfenicol 5 mg, 1 gota 3 vezes ao dia até o 25º e colírio de Tiotepa 1:2000, 1 gota 5 vezes ao dia até o 40º, servindo o olho contralateral de controle. As variações de estesiometria corneana, teste de Schirmer I, tempo de rompimento do filme lacrimal pré-corneano e teste do rosa bengala mantiveram-se significativas durante os 40 dias de utilização do Tiotepa, sendo que, para a maioria dos indivíduos, esses parâmetros se normalizaram entre 1 e 3 semanas após a suspendo da droga. No olho contralateral, evidenciaram-se alterações significativas de estesiometria corneana e do tempo de rompimento do filme lacrimal pré-corneano, que sugerem efeitos de absorção sistêmica da droga.

A pesquisa demonstra que, embora sendo transitórias, as alterações de sensibilidade corneana, do filme lacrimal (volume e estabilidade) e da vitalidade dos epitélios corneano e conjuntival estão relacionados ao uso do Tiotepa.

SUMMARY

Alterations of the corneal and conjunctival epithelii and also of the tear film were studied in 40 patients with clinic diagnosis of pterygium who were divided at random in two groups (group without Thio-tepa and group with Thio-tepa), through the biomicroscopy, corneal sensitivity measure, Schirmer test I, break-up time of the precorneal tear film and rose bengal teste.

In the group without Thio-tepa an eye was submitted to the excision of the pterygium and got until the 25^th post-operative day one drop 3 times a day of collirium of dexamethasone 1 mg and choramphenicol 5 mg, being the contralateral eye the control one. Significativy variations of corneal sensitivity measure and of the eye rose bengal test occurred only on the 15^th post-operative day, and were considered to be caused by surgery trauma.

ln the group with Thio-tepa an eye was submitted to the excision of pterygium and got a collirium of dexamethasone 1 mg and chloramphenicol 5 mg, one drop 3 times day until the 25^th post-operative day and also 1 drop of Thio-tepa 1 : 2000,5 times a day until the 40^th post-operative day, being the contralateral eye the control one. The variations of the corneal sensitivity, Schirmer test I, break-up time of the precorneal tear film and rose bengal test were significative during the 40 days of use of Thio-tepa and, for the majority of the individuals, these parameters were normaliized between the 1^st and 3^rd weeks after the discontinuation of the drug. In the contralateral eye alterations of the corneal sensitivity and of break-up the precorneal tear film were significative suggesting effects of systemic absortion of the drug.

The research demonstrates that the alterations of the corneal sensitivity, though transitory, as well as the ones of the volume and stability of the tear film, besides the vitality of the corneal and conjunctival epithelium are related to the use of Thio-tepa.
