    Currently, there are no studies in the literature evaluating salivary fluoride retention after small amounts of fluoride gel are applied to children's teeth. Therefore, the objective of the present study was to compare salivary retention after gel application using a toothbrush or by traditional application with trays. In this crossover study, children with active caries (n = 10) were randomized into one of the following treatment groups: a) application of fluoride gel using a tray (control), or b) application of fluoride gel with a toothbrush (treatment). After a 7-day washout period, the treatments were inverted. Unstimulated saliva samples were collected at baseline and 0.5, 5, 15, 30, 60 and 120 minutes after acidulated phosphate fluoride (APF) gel application in order to analyze fluoride retention in saliva. The area under the curve (AUC) was also calculated. There were no differences in fluoride retention after application of small amounts of APF with a toothbrush compared to traditional gel application using trays at all time points studied, and no differences in AUC were observed (Student t-test, p > 0.05). These results suggest that application of fluoride gel in children using a toothbrush can be utilized as an option rather than traditional trays, since the same salivary retention of fluoride is obtained using a lower dose.

    Dental implants have increased the use of titanium and titanium alloys in prosthetic applications. Whitening toothpastes with peroxides are available for patients with high aesthetic requirements, but the effect of whitening toothpastes on titanium surfaces is not yet known, although titanium is prone to fluoride ion attack. Thus, the aim of the present study was to compare Ti-5Ta alloy to cp Ti after toothbrushing with whitening and conventional toothpastes. Ti-5Ta (%wt) alloy was melted in an arc melting furnace and compared with cp Ti. Disks and toothbrush heads were embedded in PVC rings to be mounted onto a toothbrushing test apparatus. A total of 260,000 cycles were carried out at 250 cycles/minute under a load of 5 N on samples immersed in toothpaste slurries. Surface roughness and Vickers microhardness were evaluated before and after toothbrushing. One sample of each material/toothpaste was analyzed by Scanning Electron Microscopy (SEM) and compared with a sample that had not been submitted to toothbrushing. Surface roughness increased significantly after toothbrushing, but no differences were noted after toothbrushing with different toothpastes. Toothbrushing did not significantly affect sample microhardness. The results suggest that toothpastes that contain and those that do not contain peroxides in their composition have different effects on cp Ti and Ti-5Ta surfaces. Although no significant difference was noted in the microhardness and roughness of the surfaces brushed with different toothpastes, both toothpastes increased roughness after toothbrushing.
  • Addition of silver nanoparticles reduces the wettability of methacrylate and silorane-based composites Original Articles

    Incorporation of silver nanoparticles into composite resins is recommended for their reported antibacterial properties, but this incorporation can affect the wettability of such materials. Therefore, this study evaluated the effect of nano-silver addition to silorane-based and methacrylate-based composites on their contact angle. Nano-silver particles were added to Z250 (methacrylate-based) and P90 (silorane-based) composites at 0.5% and 1% by weight. The control group had no additions. SEM-EDX analysis was performed to confirm the homogeneity of the nano-silver distribution. Seventy-two composite discs were prepared and standardized to the identical surface roughness values, and then distributed randomly into 6 groups containing 12 samples each (N = 12). Two random samples from each group were observed by atomic force microscopy. Distilled water contact angle measurements were performed for the wettability measurement. Two-way ANOVA, followed by the Tukey-HSD test, with a significance level of 5%, were used for data analysis. It was observed that wettability was significantly different between the composites (p = 0.0001), and that the addition of nano-silver caused a significant reduction in the contact angle (p = 0.0001). Wettability varied depending on the concentration of the nano silver (p = 0.008). Silorane-based composites have a higher contact angle than methacrylate-based composites. Within the limitations of this study, it can be concluded that the addition of 0.5% nano-silver particles to the composites caused a decrease in the contact angle of water.
  • Degradation of resin-dentin bonds of etch-and-rinse adhesive system to primary and permanent teeth Original Articles

    The aim of this in vitro study was to compare the degradation of resin-dentin bonds of an etch-and-rinse adhesive system to primary and permanent teeth. Flat superficial coronal dentin surfaces from 5 primary second molars and 5 permanent third molars were etched with phosphoric acid and bonded with an adhesive system (Adper Single Bond 2, 3M ESPE). Blocks of resin composite (Z250, 3M ESPE) were built up and the teeth sectioned to produce bonded sticks with a 0.8 mm² cross-sectional area. The sticks of each tooth were randomly divided and assigned to be subjected to microtensile testing immediately (24 h) or after aging by water storage (6 months). Data were analyzed by two-way repeated measures ANOVA and Tukey post hoc test (α = 0.05). Failure mode was evaluated using a stereomicroscope (400×). Microtensile values significantly decreased after the 6 months aging, independent of the dentin substrate. In 24 h, the values obtained to primary dentin were lower compared with permanent dentin. This difference was not maintained after aging. Adhesive/mixed failure was predominant in all experimental groups. In conclusion, degradation of resin-dentin bonds of the etch-and-rinse adhesive system occurred after 6 months of water storage; however, the reduction in bond strength values was higher for permanent teeth.
  • Color agreement between nanofluorapatite ceramic discs associated with try-in pastes and with resin cements Original Articles

    The aim of this study was to evaluate the in vitro color agreement between nanofluorapatite ceramic discs (e.max Ceram / Ivoclar Vivadent / A2) associated with try-in pastes and those bonded with resin cements (Vitique / DMG/ try-in shade A2½ and cement shade A2½, Variolink II / Ivoclar Vivadent / try-in shade A1 and cement shade A1, and Choice 2 / Bisco / try-in shade A2 and cement shade A2), and to evaluate the shade stability of the discs bonded with resin cements. The shades of composite resin discs (Lliss / FGM / A2) and nanofluorapatite ceramic discs with try-in pastes or cements were evaluated according to the Vita Classical shade guide by a digital spectrophotometer (Micro EspectroShade, MHT) immediately after placing the try-in pastes or resin cements between composite resin discs and ceramic discs. Other evaluations were performed at 2, 5, and 6 day intervals after cementation with the resin cements. All ceramic discs that received try-in pastes presented an A2 shade. There was no statistical difference in the shade of the ceramic specimens fixed with different cements at the different intervals, as evaluated by the Friedman test (p > 0.05). Two try-in pastes presented shade compatibility with those recommended by the manufacturers. There was no similarity of shades between the ceramic discs with try-in pastes and those with the respective resin cements. Shade stability was observed in ceramic discs with resin cements within the intervals evaluated.

    Volatile sulphur compounds (VSC) are the gases mainly responsible for halitosis (bad breath). The aim of this research was to evaluate the effects of medicinal plants on halitosis control. Two commonly used plants were tested: Curcuma zedoaria and Camellia sinensis (green tea). These plants were prepared as an aqueous solution and used as mouthwashes, compared with a standard mouthwash of 0.12% chlorhexidine gluconate and a placebo (water). The experiment was conducted with 30 volunteers from the School of Dentistry of São Jose dos Campos, Univ. Estadual Paulista - UNESP, SP, Brazil. Each volunteer tested the four mouthwashes. The Cysteine Challenge Method, modified for this study, was used for initial breath standardization. Four breath assessments were conducted after volunteers rinsed orally with acetylcysteine: one before the test mouthwash was used; the second, one minute after its use; a third 90 minutes later; and the last 180 minutes later. The results showed that chlorhexidine gluconate lowered VSC production immediately, and that this effect lasted up to 3 hours, while the tested plants had immediate inhibitory effects but no residual inhibitory effects on VSC. We concluded that Curcuma zedoaria and Camellia sinensis, prepared as infusions and used as mouthwashes, did not have a residual neutralizing effect on VSC.

    The Lucia jig is a technique that promotes neuromuscular reprogramming of the masticatory system and allows the stabilization of the mandible without the interference of dental contacts, maintaining the mandible position in harmonic condition with the musculature in normal subjects or in patients with temporomandibular dysfunction (TMD). This study aimed to electromyographically analyze the activity (RMS) of the masseter and temporal muscles in normal subjects (control group) during the use of an anterior programming device, the Lucia jig, in place for 0, 5, 10, 20 and 30 minutes to demonstrate its effect on the stomatognathic system. Forty-two healthy dentate individuals (aged 21 to 40 years) with normal occlusion and without parafunctional habits or temporomandibular dysfunction (RDC/TMD) were evaluated on the basis of the electromyographic activity of the masseter and temporal muscles before placement of a neuromuscular re-programming device, the Lucia jig, on the upper central incisors. There were no statistically significant differences (p < 0.05) in the electromyographic activity of the masticatory muscles in the different time periods. The Lucia jig changed the electromyographic activity by promoting a neuromuscular reprogramming. In most of the time periods, it decreased the activation of the masticatory muscles, showing that this device has wide applicability in dentistry. The use of a Lucia jig over 0, 5, 10, 15, 20 and 30 minutes did not promote any statistically significant increase in muscle activity despite differences in the data, thus showing that this intra-oral device can be used in dentistry.

    This study evaluated the influence of fluoride-containing carbamide peroxide (CP) bleaching agents and adhesive systems on bonded enamel interfaces that are part of the dynamic pH cycling and thermal cycling models. The buccal surfaces of 60 bovine incisors were restored with a composite resin and bonded with three- and two-step, etch-and-rinse, fluoride-containing adhesives, Optibond FL (FL) and Optibond Solo Plus (SP), respectively. Restored teeth were subjected to thermal cycling to age the interface. Both SP and FL adhesive-restored teeth were bleached (n = 10) with 10% CP (CP) and 10% CP + fluoride (CPF) or were left unbleached (control). Bleaching was performed for 14 days simultaneously with pH cycling, which comprised of 14 h of remineralization, 2 h of demineralization and 8 h of bleaching. The control groups (FL and SP) were stored in remineralizing solution during their bleaching periods and were also subjected to carious lesion formation. Parallelepiped-shaped samples were obtained from the bonded interface for microtensile bond strength (∝TBS) testing. The enamel ∝TBS of the FL and SP groups (control, not bleached) were higher (p < 0.05) than those of the bleached interfaces (FL > FL + CPF = FL + CP and SP > SP + CPF = SP + CP). The groups subjected to treatment with the fluoride-containing bleaching agents exhibited similar ∝TBS compared to regular bleaching agents. Bleaching agents, regardless of whether they contained fluoride, decreased enamel bond strength.

    Oral leukoplakias (OL) are potentially malignant lesions that are typically white in color. Smoking is considered a risk factor for developing OL, and dysplastic lesions are more prone to malignant transformation. The aim of this study was to describe the clinical features observed in dysplastic and non-dysplastic OL in both smokers and non-smokers. A total of 315 cases of OL were retrieved and separated into either dysplastic or non-dysplastic lesions, and these cases were further categorized as originating in either smokers or non-smokers. Frequencies of the type of OL lesion, with respect to whether the patients smoked, were established. The results demonstrated that 131 cases of OL were dysplastic (74 smokers and 57 non-smokers), and 184 were non-dysplastic (96 smokers and 88 non-smokers). For OL cases in smokers for which information about alcohol consumption was also available (84 cases), the results revealed no significant difference in the amount of dysplastic and non-dysplastic lesions. Dysplastic lesions were more frequent in male smokers and in non-smoking females. The median age of smokers with cases of OL was significantly lower than in non-smokers; the lowest median ages were observed for female smokers with dysplastic OL. The most frequent anatomical sites of dysplastic lesions were the floor of the mouth in smokers and the tongue in non-smokers. Dysplastic lesions in smokers were significantly smaller than non-dysplastic lesions in non-smokers. Being a male smoker, being female, being younger, and having smaller lesions were associated with dysplastic features in OL. These clinical data may be important for predicting OL malignant transformation.

    The aim of this study was to evaluate the validity of the bone density value of potential implant sites in HU obtained by a specific cone-beam computed tomography (CBCT) device. In this study, the HU values obtained using a MSCT scanner were used as the gold standard. Twenty mandibles (40 potential implant sites) were scanned using an MSCT scanner (Somatom Sensation 40) and a CBCT scanner (i-CAT). The MSCT images were evaluated using the Syngo CT Workplace software and the CBCT images, using the XoranCat software. The images were evaluated twice by three oral radiologists, at 60 day intervals. The trabecular bone density of the same area was evaluated on both images. Intraclass coefficients (ICC) were calculated to examine the agreement between the examiners and between the two periods of evaluation. The bone density and area of the ROI were compared by the Student t test and Bland-Altman analysis. ICCs were excellent. The mean HU value obtained using CBCT (418.06) was higher than that obtained using MSCT (313.13), with a statistically significant difference (p < 0.0001). In addition, Bland-Altman analysis showed that the HU measures were not equivalent. In conclusion, the bone density in HU with CBCT images obtained using the device studied proved unreliable, since it was higher than that obtained using MSCT.

    Parents may feel guilty about their children's oral problems, which can affect their quality of life. The aim of this study was to assess the presence of parental guilt and its association with early childhood caries (ECC), traumatic dental injuries (TDI) and malocclusion (AMT) in preschool children. All 2 to 5 year-old children (N = 305), and their parents, seeking dental care at the University of São Paulo Dental School one-week Screening Programme, were asked to participate in the study, and 260 agreed. Children were examined by two calibrated dentists, and their parents answered a socioeconomic and ECOHIS questionnaire; the question on guilt was used as the dependent variable. Regression analyses examined the association between parental guilt and ECC, TDI, AMT and socioeconomic factors. A total of 35.8% of parents felt guilty. This was only associated with caries severity. No association was found between guilt and TDI, AMT or socioeconomic factors. ECC was present in 63.8% of the children; the mean (± sd) dmf-t score was 7.29 (± 2.78). Thus, the number of parents feeling guilty increases with the increase of their children's dental caries severity. Parental guilt is related to caries but is not associated with TDI or AMT.

    The aim of this study was to analyze the influence of socio-behavioral variables on the prevalence and severity of dental caries in 4- to 6-year-old children. A cross-sectional study was performed on a sample of 1993 children enrolled in 58 public preschools from Araçatuba City, São Paulo State, Brazil, during 2010. The exams were made using the decayed, missing, and filled teeth index (World Health Organization methodology) and detection criteria for non-cavitated lesions. A tested, self-administered questionnaire was sent to parents to obtain information about their socio-behavioral characteristics. Standardization was performed to verify concordance among examiners (kappa = 0.84). The prevalence of cavitated caries lesions was 41.2% (821), and the prevalence of both, cavitated and non-cavitated caries lesions, was 43.9% (875). The means ± standard deviations of the decayed, missing, and filled teeth index for children aged 4, 5, and 6 years were 1.18 ± 2.45, 1.65 ± 2.67, and 1.73 ± 2.77, respectively. Caries were significantly more prevalent in children from families with low incomes and low educational levels. The presence of dental caries was associated with access to dental services (p < 0.05). The associations between both, cavitated and non-cavitated dental caries lesions, and the frequency of oral hygiene were statistically significant. The prevalence of dental caries in preschoolers was strongly associated with factors related to the children's parents. Therefore, information about parents' socio-economic status, behaviors, and attitudes in relation to oral health should be considered when planning prevention and educational programs for the oral health of preschool children.
  • Correlation analysis of alveolar bone loss in buccal/palatal and proximal surfaces in rats Original Articles

    The aim was to correlate alveolar bone loss in the buccal/palatal and the mesial/distal surfaces of upper molars in rats. Thirty-three, 60-day-old, male Wistar rats were divided in two groups, one treated with alcohol and the other not treated with alcohol. All rats received silk ligatures on the right upper second molars for 4 weeks. The rats were then euthanized and their maxillae were split and defleshed with sodium hypochlorite (9%). The cemento-enamel junction (CEJ) was stained with 1% methylene blue and the alveolar bone loss in the buccal/palatal surfaces was measured linearly in 5 points on standardized digital photographs. Measurement of the proximal sites was performed by sectioning the hemimaxillae, restaining the CEJ and measuring the alveolar bone loss linearly in 3 points. A calibrated and blinded examiner performed all the measurements. Intraclass Correlation Coefficient revealed values of 0.96 and 0.89 for buccal/lingual and proximal surfaces, respectively. The Pearson Correlation Coefficient (r) between measurements in buccal/palatal and proximal surfaces was 0.35 and 0.05 for the group treated with alcohol, with and without ligatures, respectively. The best correlations between buccal/palatal and proximal surfaces were observed in animals not treated with alcohol, in sites both with and without ligatures (r = 0.59 and 0.65, respectively). A positive correlation was found between alveolar bone loss in buccal/palatal and proximal surfaces. The correlation is stronger in animals that were not treated with alcohol, in sites without ligatures. Areas with and without ligature-induced periodontal destruction allow detection of alveolar bone loss in buccal/palatal and proximal surfaces.

    The aim of this study was to analyze the color alterations performed by the CIE L*a*b* system in the digital imaging of shade guide tabs, which were obtained photographically according to the automatic and manual modes. This study also sought to examine the observers' agreement in quantifying the coordinates. Four Vita Lumin Vaccum shade guide tabs were used: A3.5, B1, B3 and C4. An EOS Canon digital camera was used to record the digital images of the shade tabs, and the images were processed using Adobe Photoshop software. A total of 80 observations (five replicates of each shade according to two observers in two modes, specifically, automatic and manual) were obtained, leading to color values of L*, a* and b*. The color difference (ΔE) between the modes was calculated and classified as either clinically acceptable or unacceptable. The results indicated that there was agreement between the two observers in obtaining the L*, a* and b* values related to all guides. However, the B1, B3, and C4 shade tabs had ΔE values classified as clinically acceptable (ΔE = 0.44, ΔE = 2.04 and ΔE = 2.69, respectively). The A3.5 shade tab had a ΔE value classified as clinically unacceptable (ΔE = 4.17), as it presented higher values for luminosity in the automatic mode (L* = 54.0) than in the manual mode (L* = 50.6). It was concluded that the B1, B3 and C4 shade tabs can be used at any of the modes in digital camera (manual or automatic), which was a different finding from that observed for the A3.5 shade tab.
