  • Auditory effects of recreational and occupational noise exposure among dental students: a cross-sectional study Original Articles

    RESUMEN Objetivo: determinar los efectos auditivos producto de la exposición a ruido recreacional y ocupacional en estudiantes de Odontología. Métodos: se reclutó a 42 estudiantes de odontología expuestos rutinariamente a ruido ocupacional, y a 72 estudiantes de otras carreras del área de la salud (grupo control). Se evaluó mediante audiometría, emisiones otoacústicas, junto con aplicar cuestionarios para determinar la exposición a ruido ocupacional y recreacional. A partir de los umbrales audiométricos e determinó la presencia de escotoma usando el criterio de Coles et al. Se analizaron las diferencias en prevalencia de escotomas entre los grupos mediante regresiones logísticas, ajustando por otras variables. Resultados: no existieron diferencias significativas en los umbrales audiométricos entre los grupos estudiados. El grupo control mostró mayor prevalencia de escotoma en 4 kHz al ajustar por sexo y edad, sin embargo, dejó de ser significativa al ajustar adicionalmente por exposición a ruido recreacional. Conclusiones: la exposición a ruido ocupacional no se asoció a alteraciones auditivas. Además, las diferencias en prevalencia de escotomas estarían relacionadas a exposición a ruido recreacional.

    ABSTRACT Purpose: to determine the auditory effects of noise exposure from recreational and occupational sources among dentistry students. Methods: forty-two dentistry students, routinely exposed to occupational noise, and 72 students from other health science schools were recruited (control group). Audiometric testing, otoacoustic emissions and questionnaires to assess recreational and occupational noise were applied to the sample. The presence of a notch was determined for each participant audiometry´s test based on the criteria proposed by Coles et al. Differences in notch prevalence were analyzed by applying univariate regression models as well as a multivariate model adjusted by covariates. Results: non-significant differences in auditory thresholds between groups were found. The controls exhibited a significantly higher prevalence of a notch at 4 kHz than the group exposed to noise. However, the differences were not statistically significant in the multivariate model adjusted by recreational noise exposure. Conclusions: the occupational noise exposure was not significantly associated to auditory system dysfunction. In addition, the differences in notch prevalence could be related to recreational noise exposure.

    RESUMEN Objetivo: describir el perfil de usuarios atendidos por fonoaudiólogos en una unidad de paciente crítico. Métodos: se realizó un estudio observacional descriptivo con temporalidad ex post facto. Se analizaron los resúmenes estadísticos mensuales de usuarios internados durante los meses de enero a diciembre de 2018, en la unidad de cuidados intensivos de un hospital público. Se describieron los datos a partir de análisis de frecuencia y medidas de tendencia central. La distribución de las variables se determinó mediante skewness-kurtosis test, considerando un nivel de significancia p<0,05. Resultados: 217 sujetos recibieron 868 atenciones fonoaudiológicas. Los hombres (57,26%) mayores de 65 años requirieron una intervención más frecuente. Los principales diagnósticos médicos de ingreso a la unidad correspondieron a patologías inespecíficas (57,14%), enfermedades respiratorias (15,21%) y cerebrovasculares (12,79%). Las funciones de fonoaudiología se relacionaron con la evaluación de la deglución (54,31%) y de la voz (32,4%). En cuanto a la intervención, el tratamiento de la disfagia (25,82%) y las funciones motoras orales (25,04%) dominaron la carga laboral. Las funciones asociadas con el lenguaje, el habla y la cognición fueron secundarias. Conclusión: el perfil del paciente crítico y las labores fonoaudiológicas en este ámbito representan un primer paso para caracterizar el rol profesional del fonoaudiólogo en el contexto de los equipos de medicina intensiva.

    ABSTRACT Objective: to describe the profile of patients treated by Speech-Language therapists in a Critical Patient Unit. Methods: an ex post facto, observational and descriptive study was carried out. Monthly statistical data of patients hospitalized in the period January-December 2018 were analyzed, in the Intensive Care Unit at a public hospital. Data were described from the analysis of frequency and measures of central tendency. The distribution of the variables was determined through the skewness-kurtosis test, considering a significance level of p<0.05. Results: 217 individuals got 868 speech-language therapy services. Men (57.26%), older than 65 years old, required a more frequent intervention. The main medical diagnosis of admission to the unit corresponded to non-specific pathologies (57.14%), respiratory disease (15.21%) and cerebrovascular disease (12.79%). The speech-language therapy functions were related to the evaluation of swallowing (54.31%) and voice (32.4%). In relation to the intervention, the treatment of dysphagia (25.82%) and oral motor functions (25.04%) was predominant in the duties. Functions associated with language, speech and cognition were secondary. Conclusion: the profile of the critical patient and the speech-language therapy work in this field represent a first step to characterize the role of the speech-language therapist in Intensive Medicine teams.
  • Escala de Funcionamento Auditivo no monitoramento do treinamento auditivo acusticamente controlado Artigos Originais

    RESUMO Objetivo: identificar a percepção subjetiva por meio da Escala de Funcionamento Auditivo após um programa de treinamento auditivo acusticamente controlado. Métodos: participaram deste estudo 23 indivíduos de ambos os sexos, na faixa etária de 6 a 15 anos. Critérios de inclusão: apresentar limiares auditivos menores do que 20 dBNA entre 250 e 8000 Hz, timpanometria com curvas tipo A e diagnóstico de transtorno de processamento auditivo central confirmado por testes comportamentais com indicação de treinamento auditivo acusticamente controlado. O treinamento auditivo acusticamente controlado foi realizado em dez sessões com duração de uma hora cada. Foram utilizados testes auditivos apresentados em condições de escuta progressivamente adversas. Na primeira e na última sessão do treinamento auditivo acusticamente controlado os indivíduos foram submetidos à aplicação da Escala de Funcionamento Auditivo - SAB. Foram aplicados os testes estatísticos apropriados, considerando-se valor de p menor que 0,05. Resultados: as questões Q1, Q2, Q4, Q7, Q8 e Q11 demonstraram resultados estatisticamente significantes, evidenciando melhora dos comportamentos após o treinamento auditivo acusticamente controlado. Já as questões Q3 e Q10 apresentaram tendência á significância e a pontuação total no momento pós aumentou em relação ao momento pré, demonstrando diferença estatisticamente significante representando uma redução nas queixas. Conclusão: a percepção subjetiva do indivíduo e de seus familiares foi passível de ser identificada pela aplicação da Escala de Funcionamento Auditivo no pré e pós Treinamento Auditivo Acusticamente Contolado, revelando melhora dos comportamentos auditivos e de atenção.

    ABSTRACT Purpose: to identify the Scale of Auditory Behaviors perception after an acoustically controlled auditory training program. Methods: 23 individuals of both genders, aged 6 to 15 years, participated in this study. Inclusion criteria: to present auditory thresholds lower than 20 dBHL between 250 and 8000Hz, type A tympanometry and diagnosis of central auditory processing disorder confirmed by behavioral tests with indication of acoustically controlled auditory training which was performed in ten one-hour sessions. Hearing tests in progressively adverse listening conditions were used. In the first and last session of the acoustically controlled auditory training program, the individuals were submitted to the Scale of Auditory Behaviors. The appropriate statistical tests were applied considering a p-value less than 0.05. Results: questions Q1, Q2, Q4, Q7, Q8 and Q11 showed statistically significant results and improvement in behaviors after the acoustically controlled auditory training. On the other hand, questions Q3 and Q10 showed a tendency towards significance and the total score in the post time period increased in relation to the pre-moment, pointing to a statistically significant difference that represents complaint reductions. Conclusion: the subjective perception of the individuals and their family members could be identified by the application of SAB in the pre and post auditory training, revealing an improvement in auditory and attention behaviors.
  • Instrumento de Rastreio para a Gagueira do Desenvolvimento: elaboração e validação de conteúdo Artigos Originais

    RESUMO Objetivo: elaborar um instrumento de rastreio com fins de identificação do risco para gagueira do desenvolvimento em crianças pré-escolares. Métodos: os procedimentos foram divididos em duas etapas: A primeira etapa - teórica, consistiu na revisão da literatura e a construção dos itens que compõe o instrumento; A segunda etapa - validação de conteúdo, consistiu da análise do instrumento por um comitê de juízes composto por 10 fonoaudiólogas atuantes na área da Fluência. Para análise dos dados foi calculado o índice de validade de conteúdo por item (IVC-I) e pelo total de respostas (IVC-T), bem como realizada uma análise da confiabilidade dos itens por meio do alfa de Cronbach. Resultados: o Instrumento de Rastreio para a Gagueira do Desenvolvimento (IRGD) foi composto por 24 itens distribuídos em quatro categorias-chaves. Os índices IVC-I e IVC-T evidenciaram alta concordância entre os juízes. Os coeficientes do alfa de Cronbach indicaram alta consistência interna entre as respostas dos juízes em 19 dos 24 itens. A análise qualitativa destacou a necessidade de novos ajustes. Todas as análises possibilitaram a construção da segunda versão do instrumento. Conclusão: o instrumento proposto mostrou evidências de validade baseada no conteúdo que permitiram, até o momento, ajustá-lo em relação ao seu construto. Sugere-se a continuidade do processo de validação por meio do emprego de novas medidas de acurácia.

    ABSTRACT Purpose: to develop a screening instrument to identify the risk of developmental stuttering in preschoolers. Methods: the procedures were divided into two stages: The first one (theoretical), consisted of the literature review and the construction of the items that make up the instrument; the second stage (content validation), consisted of the analysis of the instrument by a judging committee formed by 10 speech-language-hearing therapists who work in the field of fluency. The data analysis was based on the calculation of the item content validity index (I-CVI) and the total number of answers (T-CVI). Also, an item reliability analysis was conducted with Cronbach’s alpha. Results: the Developmental Stuttering Screening Instrument (DSSI) encompassed 24 items distributed into four key categories. The I-CVI and T-CVI revealed a high agreement between the judges. The Cronbach’s alpha coefficients indicated a high internal consistency between the judges’ answers in 19 of the 24 items. The qualitative analysis pointed to the need for new adjustments. All the analyses contributed to the construction of the second version of the instrument. Conclusion: the proposed instrument showed content-based validity evidence that made it possible, up to the present moment, to adjust it to its construct. It is suggested that the validation process continue, employing new accuracy measures.
  • Associação entre o perfil auditivo e o diagnóstico fonoaudiológico em crianças e adolescentes Artigos Originais

    RESUMO Objetivo: verificar a associação entre o resultado da avaliação auditiva e o diagnóstico fonoaudiológico de crianças e adolescentes atendidos em um ambulatório universitário. Métodos: trata-se de estudo observacional transversal, baseado em análise dos prontuários de crianças e adolescentes atendidos em um ambulatório de avaliação fonoaudiológica entre 2010 e 2014. Foram coletados dados referentes às características sociodemográficas, diagnóstico fonoaudiológico, resultados do exame auditivo e da Avaliação Simplificada do Processamento Auditivo. Foi realizada análise descritiva e de associação, utilizando o teste Qui-quadrado ou Exato de Fisher e considerando nível de significância estatística de 5%. Resultados: a amostra foi composta por 122 participantes, a maioria do sexo masculino (67,2%), com média de idade de 8,78 anos. Foi verificada associação estatisticamente significante entre o resultado da audiometria e a hipótese diagnóstica de alteração da linguagem escrita (p = 0,011); entre os resultados dos testes de Memória Sequencial Não Verbal e de localização sonora com as hipóteses diagnósticas de alteração dos aspectos cognitivos da linguagem (p = 0,019 e p = 0,033, respectivamente) e da fala (p = 0,003 e p = 0,020, respectivamente) e entre o resultado do teste de Memória Sequencial Verbal com a hipótese diagnóstica de alteração de fala (p = 0,005). Conclusão: diante das associações encontradas, propõe-se que crianças e adolescentes com alterações na fala realizem a Avaliação Simplificada do Processamento Auditivo com a finalidade de verificar possíveis aspectos alterados, favorecendo intervenções terapêuticas direcionadas.

    ABSTRACT Purpose: to verify the association between the auditory assessment result and the speech-language-hearing diagnosis in children and adolescents. Methods: an observational, cross-sectional study based on the analysis of medical records of children and adolescents that received care at a speech-language-hearing assessment outpatient center between 2010 and 2014. Data on sociodemographic characteristics, speech-language-hearing diagnosis, auditory examination results, and Auditory Processing Simplified Assessment results were collected. Descriptive and association analyses were conducted with either the chi-squared or Fisher’s exact test, considering the 5% statistical significance level. Results: the sample comprised 122 participants, most of them males (67.2%), mean age 8.78 years. A statistically significant association was verified between the audiometry result and the diagnostic hypothesis of change in written language (p = 0.011); between the results of both the sequential memory test for nonverbal sounds and sound localization and the diagnostic hypotheses of change in the cognitive aspects of language (p = 0.019 and p = 0.033, respectively) and of speech (p = 0.003 and p = 0.020, respectively); and between the result of the sequential memory test for verbal sounds and the diagnostic hypothesis of change in speech (p = 0.005). Conclusion: given the associations found, it is proposed that children and adolescents with changes in speech undergo the Auditory Processing Simplified Assessment to verify the possibility of changed aspects, favoring directed therapeutic interventions.
  • Alterações miofuncionais orofaciais e atividade eletromiográfica do músculo orbicular da boca em trombonistas Artigos Originais

    RESUMO Objetivos: analisar se existem alterações miofuncionais orofaciais clínicas e autorreferidas em trombonistas, assim como comparar a atividade elétrica do músculo orbicular da boca antes e após o uso do trombone. Métodos: estudo transversal e observacional, cuja amostra foi composta por 20 universitários trombonistas. Foi utilizado um questionário que investiga algumas particularidades sobre o instrumentista, da Avaliação Miofuncional Orofacial Com Escores Ampliados - AMIOFE-A e análise eletromiográfica do músculo orbicular da boca antes e após execução de uma tarefa com o trombone. Foram calculadas: média, desvio padrão, frequência e porcentagem e utilizou-se o teste t-Student pareado na comparação das médias dos registros eletromiográficos pré e pós-execução do trombone, significância=5%. Resultados: setenta por centro dos trombonistas relataram algum sintoma de alteração miofuncional orofacial, sendo compensações musculares, tensão e dor. Na avaliação miofuncional encontrou-se alteração leve no volume de bochechas (90%) e comissuras labiais com assimetria (60%). Houve redução significante da atividade elétrica do músculo orbicular inferior após a execução de tarefa com trombone (p=0,04), enquanto a do superior permaneceu inalterada. Conclusão: trombonistas apresentam alterações oromiofuncionais clínicas e autorreferidas e diminuição da atividade elétrica do músculo orbicular inferior da boca após uso do trombone.

    ABSTRACT Objective: to analyze whether trombonists present orofacial myofunctional changes and compare the electrical activity of the orbicularis oris muscle before and after playing their instrument. Methods: an observational, descriptive, and cross-sectional study. The sample consisted of 20 university trombonists. Data collection involved three steps: application of a questionnaire investigating some peculiarities of trombonists (Orofacial Myofunctional Evaluation with Expanded Scores - OMES-E), and electromyographic analysis of the orbicularis oris muscle before and after performing a piece with the trombone. The mean, standard deviation, frequency, and percentage were calculated, and the paired Student’s t test was used to compare the means of the electromyographic records before and after playing the trombone, at a 5% significance. Results: seventy per cent of trombonists reported some symptom of orofacial myofunctional alteration, namely, muscle compensations, tension, and pain. The myofunctional evaluation showed a mild change in cheek volume (90%) and labial commissure asymmetry (60%). The electrical activity of the lower orbicularis oris muscle decreased significantly after the trombone practice (p = 0.04), while that of the superior orbicularis oris muscle remained unchanged. Conclusion:trombonists show clinical and self-reported oromyofunctional changes as well as decreased electrical activity of the lower orbicularis oris muscle, after playing the trombone.

    RESUMO Objetivo: investigar o conhecimento de professores sobre a Fonoaudiologia Educacional e sobre a importância das habilidades auditivas e linguísticas para aprendizagem, comparando as respostas dos docentes entre escola pública e privada. Métodos: estudo exploratório de abordagem mista, quantitativa e qualitativa, realizado com 25 professores que lecionam na Educação Infantil e Ensino Fundamental I. A coleta de dados foi realizada mediante um questionário composto por 19 perguntas fechadas, que exploraram a relação entre linguagem, audição e aprendizagem e 01 pergunta aberta, que abordou o conhecimento sobre a Fonoaudiologia Educacional. Os dados quantitativos foram analisados por meio de análises estatísticas descritivas e inferenciais e os qualitativos por meio de análise de conteúdo na modalidade temática. Resultados: os professores reconhecem, de modo geral, a importância da audição e linguagem para a aprendizagem, embora não se sintam preparados para lidar com isso na escola e não possuam conhecimentos mais específicos sobre o processamento auditivo. Os resultados obtidos na pergunta aberta também revelaram um conhecimento limitado sobre a Fonoaudiologia Educacional e a principal fonte de conhecimento foi a experiência prévia de alguns professores com trabalho do fonoaudiólogo inserido no contexto educacional. Não foram observadas diferenças importantes entre as repostas dos docentes de escolas pública e privada. Conclusão: os conhecimentos dos professores acerca da Fonoaudiologia Educacional e da importância da audição e linguagem para a aprendizagem ainda são limitados, sendo necessários maiores investimentos em formações nesta área.

    ABSTRACT Purpose: to investigate the teachers’ knowledge on educational speech-language-hearing pathology and the importance of auditory and linguistic skills to learning, comparing the answers of public-school teachers with those of private ones. Methods: an exploratory study with a mixed quantitative and qualitative approach, conducted with 25 teachers working in kindergarten and elementary school. Data were collected with a questionnaire comprising 19 closed-ended questions (which dealt with the relationship between language, hearing, and learning) and one open-ended question (which approached their knowledge on educational speech-language-hearing pathology. The quantitative data were analyzed with descriptive and inferential statistical analyses, and the qualitative ones, with theme modality content analysis. Results: in general, the teachers recognize the importance of hearing and language to learning, although they do not feel prepared to deal with it at school, neither do they have more specific knowledge of auditory processing. The results obtained in the open-ended question revealed an also limited knowledge on educational speech-language-hearing pathology. In this sense, their greatest source of information was some teachers’ previous experience with the work of a speech-language-hearing pathologist within the educational setting. No important differences were observed between the answers given by public school teachers and those by private ones. Conclusion: the teachers’ knowledge on educational speech-language-hearing pathology and the importance of hearing and language to learning is still limited, which requires more investment in the training in this field.
  • Formação do fonoaudiólogo para atuação educacional: o que referem os estudantes de Fonoaudiologia Artigos Originais

    RESUMO Objetivo: discutir a formação do fonoaudiólogo para atuação educacional, a partir do que referem os estudantes de Fonoaudiologia. Métodos: participaram da pesquisa 29 discentes do 7º período do curso de Fonoaudiologia de duas Instituições de Ensino Superior. Utilizou-se um questionário elaborado especificamente para o estudo composto por questões abertas e fechadas a respeito do perfil dos estudantes e de sua formação para atuação em contexto educacional. Além disso, foram coletadas informações das ementas das disciplinas relacionadas a Fonoaudiologia Educacional nessas duas instituições. Os dados foram analisados de forma quanti-qualitativa. Resultados: os resultados indicaram que a soma das cargas horárias das disciplinas voltadas à educação pode ser considerada baixa e que as ementas de tais disciplinas não contemplam pontos importantes para a formação do fonoaudiólogo com vistas a atuação educacional. Além disto, os participantes referiram que a maioria das atividades práticas acontecem na educação infantil ou no ensino fundamental, com objetivos relacionados a uma visão médico-centrada. Os estudantes apontaram, ainda, a necessidade de melhorias na formação do fonoaudiólogo para atuação educacional. Conclusão: os estudantes referiram que sua formação necessita de ampliação de carga horária, conteúdos e vivências que permitam sua inserção em contexto educacional de forma consciente, crítica e responsável.

    ABSTRACT Purpose: to discuss the speech-language-hearing pathologist’s training to work in education, based on the report from speech-language-hearing students. Methods: a total of 29 students in the seventh semester of the Speech-Language-Hearing program at two higher education institutions participated in this study. The questionnaire used was developed specifically for this study. It comprised open- and closed-ended questions on the profile of the students and their training to work in educational settings. Data were also collected on the syllabi of the courses related to Educational Speech-Language-Hearing Pathology at both institutions. The data were quantitatively and qualitatively analyzed. Results: the results indicated that the summed course load of the education-related courses is low, and their syllabi do not cover important aspects for the speech-language-hearing training aimed at the work in education. Also, the students reported that most of the practical activities took place in kindergarten or elementary school, with objectives related to a medical-centered view. The students also pointed out the need for improvements in the speech-language-hearing training to work in education. Conclusion: the students reported that their training should involve a broader course load, as well as contents and experiences to help them to consciously, critically, and responsibly join the educational setting.

    RESUMO Objetivo: avaliar a implantação do programa de conservação auditiva em duas empresas de grande porte. Métodos: trata-se de uma pesquisa avaliativa de análise de implantação do tipo 1b. O estudo foi conduzido em três etapas, sendo: determinação do grau de implantação do Programa de Conservação Auditiva das empresas avaliadas; análise dos contextos políticos e estruturais das empresas avaliadas; e, por fim, uma análise da influência dos contextos políticos e estruturais no grau de implantação nas empresas avaliadas. Foram realizadas as seguintes análises para coleta dos dados: entrevistas, análise documental e observacional. Resultados: o Programa de Conservação Auditiva na 'empresa A' foi classificado como ‘implantado’ (91,1%) e na 'empresa B' como ‘parcialmente implantado’ (62,7%). O contexto de implantação da 'empresa A' se apresentou favorável à implantação do programa. Já na 'empresa B' o contexto foi considerado desfavorável, tendo categorias não favoráveis. Foi possível relacionar, também, que as características do contexto de implantação influenciaram positiva ou negativamente na implantação do programa. Conclusão: houve resultados satisfatórios quanto à implantação do Programa de Conservação Auditiva em ambas as empresas avaliadas.

    ABSTRACT Purpose: to assess the implementation of hearing conservation programs in two large companies. Methods: an evaluative research that analyzed a type 1b implementation. The study was conducted in three stages, namely: establishing the company’s degree of implementation, analyzing the companies’ policies and structures, and lastly, analyzing the influence of the companies’ policies and structures on their degree of implementation. The following analyses were conducted to collect data: interviews, document analysis, and observation. Results: the hearing conservation program in Company A was classified as implemented (91.1%), while in Company B, it was partially implemented (62.7%). The context in Company A was favorable to implementing the program, whereas in Company B, the context was considered unfavorable, due to disadvantageous categories. Also, the characteristics of the implementation context influenced the placement of the program either positively or negatively. Conclusion: there were satisfactory results regarding the implementation of the hearing conservation program in both companies assessed.
  • Associação entre falha nas emissões otoacústicas e indicador de risco para a perda auditiva Artigos Originais

    RESUMO Objetivo: descrever os resultados de um programa de saúde auditiva neonatal e verificar se há associação entre presença de indicador de risco para perda auditiva e falhas nos testes e no diagnóstico. Métodos: trata-se do estudo de uma coorte, observacional e retrospectiva, com dados secundários contidos em planilha eletrônica sobre condições sociodemográficas da família, história clínica e resultados dos exames de 7.800 participantes que realizaram a Triagem Auditiva, de 2010 a 2016. Na descrição da primeira e segunda etapa, utilizou-se frequência absoluta e valores percentuais. Na associação entre indicador de risco com falhas nas EOA, utilizaram-se Odds Ratio, Intervalo de Confiança e nível de significância 0,5%. Resultados: dos 12 lactentes com perda auditiva, oito apresentaram como indicador de risco, a medicação ototóxica e permanência em UTI, sendo que a chance de falhar nas EOA ocorreu com onze indicadores. A chance do diagnóstico de perda auditiva foi 13 vezes maior quando há indicador de risco, 18 vezes maior quando usou medicação ototóxica e 16,62 vezes maior com permanência em UTI. Conclusão: os resultados mostram que conhecer os indicadores levará à reflexão de ações a serem adotadas pela equipe responsável.

    ABSTRACT Purpose: to describe the results of a neonatal hearing health program and verify whether there is an association between the presence of risk indicators for hearing loss and failure in the tests and diagnosis. Methods: a one-cohort, observational, retrospective study with secondary data contained in a spreadsheet concerning the family’s sociodemographic condition, clinical history, and examination results of 7,800 participants who were submitted to hearing screening between 2010 and 2016. Absolute frequency and percentages were used in the description of the first and second stages. In the association between risk indicators and failures in the otoacoustic emissions, the odds ratio, confidence interval, and significance level at 0.5% were used. Results: the risk indicators in 8 out of the 12 infants presented with hearing loss were ototoxic medication and intensive care unit (ICU) stay, whereas the likelihood of failure in the otoacoustic emissions occurred along with 11 indicators. The likelihood of a diagnosis of hearing loss was 13 times greater when there was a risk indicator, 18 times greater when an ototoxic medication had been used, and 16.62 times greater when they stayed in ICUs. Conclusion: the results show that knowing the indicators leads to considering the actions the team in charge should take.
  • Esclerose lateral amiotrófica, disartria e alterações de linguagem: tipo de pesquisa e abordagens em diferentes áreas - revisão integrativa da literatura Artigos De Revisão

    RESUMO Objetivo: identificar o conhecimento produzido em pesquisas nacionais e internacionais das alterações de fala e de linguagem na Esclerose Lateral Amiotrófica, quanto tipo de pesquisa e abordagens em diferentes áreas. Métodos: revisão integrativa realizada em bases de dados, utilizando-se os descritores Amyotrophic Lateral Sclerosis, Dysarthria, Language Disorders, Speech Production Measurement e Speech Disorders. Os critérios de inclusão abrangeram artigos que abordassem alterações motoras de fala e de linguagem de 2013 a 2018, excluindo-se duplicações, categorizando-se os válidos para análise. Resultados: selecionou-se 83 artigos, após triagem dos títulos e resumos. Foi encontrada grande produção científica de diferentes países e áreas, principalmente Fonoaudiologia e Neurologia, na maioria pesquisas clínicas (65,06%), com foco principal em alterações motoras da fala (42,16%), alterações motoras de fala e linguagem, cognição e comportamento (27,71%) e alterações de linguagem (12,06%). Conclusão: as pesquisas encontradas foram, na maioria, clínicas e com objetivo determinar diagnóstico das alterações em diferentes áreas do conhecimento. No que se refere à comunicação, foram encontrados poucos estudos no Brasil e os internacionais voltavam-se à alta tecnologia. Os resultados confirmaram o caráter heterogêneo doença, que evidencia, além de comprometimento motor da fala, prejuízo cognitivo, comportamental e de linguagem.

    ABSTRACT Purpose: to identify the knowledge produced in national and international researches on speech and language disorders in Amyotrophic Lateral Sclerosis, regarding the type of research and approach in different areas. Methods: an integrative review performed on databases, using the following descriptors: Amyotrophic Lateral Sclerosis, Dysarthria, Language Disorders, Speech Production Measurement and Speech Disorders. The inclusion criteria covered articles that addressed motor speech and language disorders from 2013 to 2018, excluding duplications, and categorizing valid articles for analysis. Results: 83 articles were selected, after screening the titles and abstracts. A large scientific production from different countries and areas, mainly Speech Therapy and Neurology, was found. Most of them was clinical research (65.06%), with a main focus on speech motor disorders (42.16%), speech and language motor disorders, cognition and behavior (27.71%), and language disorders (12.06%). Conclusion: researches found were mostly clinical and aimed at determining the diagnosis of disorders in different areas of knowledge. With regard to communication, few studies have been found in Brazil, and international studies addressed high technology. The results confirmed the heterogeneous nature of the disease, which shows, in addition to motor impairment of speech, cognitive, behavioral and language impairments.
  • Efeitos terapêuticos da fotobiomodulação na clínica fonoaudiológica: uma revisão integrativa da literatura Artigos De Revisão

    RESUMO Objetivo: realizar revisão de literatura sobre efeitos terapêuticos da fotobiomodulação aplicáveis à Fonoaudiologia. Métodos: foram utilizadas as bases de dados Biblioteca Cochrane, Biblioteca Virtual de Saúde, Medical Literature Analysis and Retrieval Sistem online (Medline) via PuBmed e Web of Science/ISI. Foram selecionados artigos originais e com resumos disponíveis que avaliaram o efeito terapêutico da fotobiomodulação em situações relacionadas à prática fonoaudiológica. Resultados: a amostra consistiu em 23 artigos, a maioria indexada na PubMed. As áreas da Saúde com maior número de publicações foram Fisioterapia e Medicina. O tamanho das amostras variou de 1 a 99 indivíduos, faixas etárias entre 15 e 77 anos e o comprimento de onda mais aplicado foi o infravermelho. A maioria das pesquisas evidenciou resultados positivos da aplicação da fotobiomodulação, embora, em poucas publicações tenham sido avaliados os efeitos dessa modalidade de tratamento associados a exercícios de reabilitação. Conclusão: a fotobiomodulação traz benefícios em diferentes distúrbios tratados por fonoaudiólogos, no entanto, há grande diversidade metodológica e ausência de protocolos específicos da dosimetria ideal para cada distúrbio.

    ABSTRACT Purpose: to conduct a literature review on the therapeutic effects of photobiomodulation applicable to speech-language-hearing therapy. Methods: the databases searched were the Cochrane Library, Virtual Health Library, Medical Literature Analysis and Retrieval System Online (MEDLINE) via PubMed, and Web of Science/ISI.The selected articles were original ones whose abstracts were available and that evaluated the therapeutic effect of photobiomodulation in situations related to speech-language-hearing practice. Results: the sample comprised 23 articles, most of them being indexed in PubMed. The fields of health with the largest number of publications were Physical Therapy and Medicine. The sample size ranged from1 to 99 people, aged 15 to 77 years, and the most applied wavelength was the infrared one. Most of the studies had positive photobiomodulation application results - although in a few publications the effects of this treatment modality were assessed in a combination with rehabilitation exercises. Conclusion: photobiomodulation benefits different disorders treated by speech-language-hearing therapists, however, there is a broad methodological diversity, lacking specific protocols for the ideal dosimetry for each disorder.
  • Implicações audiológicas da COVID-19: revisão integrativa da literatura Artigos De Revisão

    RESUMO Objetivo: realizar um levantamento da literatura nacional e internacional sobre os impactos da infecção por coronavírus no sistema auditivo. Métodos: revisão integrativa com pesquisa nas bases de dados Bireme, PubMed, Scopus e Web of Sciences. Critérios de inclusão: artigos em português e em inglês que tinham como tema a infecção por coronavírus e seus efeitos no sistema auditivo. Critérios de exclusão: informações de livros e/ou capítulos, cartas ao editor, artigos de revisões e de relatos de experiência. Para a estratégia de busca, utilizou-se a combinação dos descritores em português e em inglês, respectivamente: “Infecções por coronavírus”, “Audição”, “Perda auditiva”, “Coronavirus infections”, “Hearing”, “Hearing Loss”. Resultados: dos 43 artigos encontrados, dois abordaram o tema proposto. O primeiro estudo avaliou 20 pacientes que testaram positivo para COVID-19, porém assintomáticos, esses realizaram audiometria tonal liminar e emissões otoacústicas. Observou-se como resultado um aumento significativo dos limiares auditivos nas altas frequências e menor amplitude de resposta no exame de emissões otoacústicas evocadas por estímulo transiente, quando comparados ao grupo controle. O segundo estudo relatou o caso de uma paciente de 35 anos de idade com COVID-19 assintomática, que apresentou queixa de otalgia e zumbido após a contaminação. A audiometria tonal liminar e timpanometria indicaram perda auditiva do tipo condutiva de grau leve unilateral à direita com curva timpanométrica do tipo B deste lado. Conclusão: os estudos incluídos mostraram diferentes repercussões da COVID-19 na audição, com possíveis acometimentos nas estruturas sensoriais e mecânicas do sistema auditivo. O conhecimento sobre a COVID-19 é limitado e mais estudos primários sobre seu real impacto no sistema auditivo são necessários.

    ABSTRACT Purpose: to survey the national and international literature on the impacts of the coronavirus infection on the auditory system. Methods: an integrative review with search in the BIREME, PubMed, Scopus, and Web of Sciences databases. Inclusion criteria: articles in Portuguese and English whose subject was the coronavirus infection and its effects on the auditory system. Exclusion criteria: information from books and/or chapters, letters to editors, review articles, experience reports. The search strategy was based on the following combined descriptors, respectively in Portuguese and English: “Infecções por coronavírus”, “Audição”, “Perda auditiva”, “Coronavirus infections”, “Hearing”, “Hearing Loss”. Results: out of 43 articles found, two approached the issue. The first study assessed 20 patients that tested positive for COVID-19, though asymptomatic, who underwent pure-tone threshold audiometry and otoacoustic emissions. A significant increase in the auditory thresholds at high frequencies and a smaller response amplitude in the transient evoked otoacoustic emissions of those who tested positive for COVID-19 were observed when compared to that of controls. The second study reported the case of an asymptomatic 35-year-old COVID-19 female patient, who complained of otalgia and tinnitus, after being contaminated. The pure-tone threshold audiometry and tympanometry indicated mild unilateral (right ear) conductive hearing loss, with a type B tympanometric curve on that side. Conclusion: the studies included in this review showed different consequences of COVID-19 on hearing, with possible impairments on the sensory and mechanical structures of the auditory system. The knowledge of COVID-19 is limited, and further studies on its real impact on the auditory system are necessary.
  • Instrumentos de avaliação fonoaudiológica da paralisia facial periférica: revisão integrativa de literatura Artigos De Revisão

    RESUMO Objetivo: descrever e analisar os instrumentos clínicos de avaliação de paralisia facial periférica por meio da revisão integrativa de literatura. Métodos: foram seguidos os preceitos deste tipo de revisão: questão norteadora, identificação, seleção de estudos e análise crítica. As bases utilizadas foram: “Biblioteca Virtual em Saúde (BVS)”, “Scientific Electronic Library (SCIELO)”, “Google Acadêmico” e “Pubmed”, na busca de artigos publicados em periódicos nacionais, no período entre janeiro de 2008 e julho de 2018, disponíveis na íntegra, sendo utilizados os termos: “Fonoaudiologia”, “Avaliação” e “Paralisia Facial” e na língua inglesa: “Speech, Language and Hearing Sciences”, “Evaluation” e “Facial Paralysis. Os dados obtidos foram organizados por autor, título, objetivo, instrumentos utilizados, descrição dos instrumentos e procedimentos de aplicação utilizados nos artigos. Resultados: de um total de 992 artigos encontrados, apenas 18 se encaixaram nos critérios de inclusão da pesquisa. Na maioria dos artigos encontrados, havia apenas citações ou considerações sobre instrumentos de avaliação, sem a plena descrição e procedimentos para aplicação. Conclusão: o estudo permitiu identificar publicações que indicam uso de instrumento de avaliação clínica e fonoaudiológica. No entanto, seria necessário detalhamento quanto a apresentação dos procedimentos de avaliação, visando cooperar na elaboração e no refinamento de metodologias e técnicas fonoaudiológicas.

    ABSTRACT Purpose: to describe and analyze the clinical instruments that assess peripheral facial palsy through an integrative literature review. Methods: the precepts for this type of review were followed: research question, identification, selection of studies, and critical analysis. The Virtual Health Library (VHL), Scientific Electronic Library Online (SciELO), Google Scholar, and PubMed databases were accessed to search for fully available articles published in national journals between January 2008 and July 2018. The terms used in the search were “Speech, Language and Hearing Sciences”, “Evaluation”, and “Facial Paralysis”, in both English and Portuguese. The data obtained were organized per author, title, objective, instruments used, description of the instruments, and application procedures used in the articles. Results: out of the total 992 articles found, only 18 met the inclusion criteria of the research. In most of them, there was only the citation of the assessment instruments or considerations about them, without fully describing the application procedures. Conclusion: the study identified publications that indicate the use of clinical and speech-language-hearing assessment instruments. However, further detailing is necessary regarding the assessment procedures to help develop and refine the speech-language-hearing methodologies and techniques.
  • Dialogic Reading for Comprehension: effects on children's story retelling - a case report Case Reports

    ABSTRACT The present study investigated if Dialogic Reading for Comprehension can help children improve their retelling skills. Earlier studies suggest that Dialogic Reading promotes the inclusion of psychological story elements, but has little effect on the organization of plot sequences. Dialogic Reading for Comprehension is a development of Dialogic Reading aimed at comprehension. It uses scaffolding based on (a) structure of story events and (b), the meaning of story events (narrative functions). A single subject design was used across participants for two of the children and an adapted alternating treatment design for the other two, with follow-up. Four children participated in four conditions: baseline, Dialogic Reading for Comprehension with scaffolding about narrative functions, Dialogic Reading for Comprehension with scaffolding about narrative events, and Dialogic Reading for Comprehension, a combination of both. At the baseline, the children were not including the minimum elements to create a coherent storyline and this skill was established with the intervention. Plot sequence and organization improved the most in the combined condition. Narrative enrichment was not affected. Thus, it is discussed that Dialogic Reading for Comprehension can help children organize plot sequence and coherence. The superiority of the combined condition may reflect an underlying interdependence between the psychological and action-related components of narratives.
