Ceratite ulcerativa periférica após cirurgia de catarata em artrite reumatóide

RESUMO

Ceratite ulcerativa periférica e esclerite necrosante estão descritas na literatura após cirurgias e traumas oculares. Relata-se o caso de uma paciente com diagnóstico de Artrite Reumatóide que desenvolveu afinamento corneano periférico progressivo após cirurgia de catarata. A conduta incluiu exérese da conjuntiva adjacente à área de afinamento, cola sobre a área afinada, uso de inibidores da colagenase, adaptação de lente de contato terapêutica e imunossupressão sistêmica. A presença de afinamento corneano periférico pode ser uma manifestação primária ou tardia de Artrite Reumatóide, doença vascular sistêmico autoimune potencialmente letal, exigindo uma abordagem criteriosa desses pacientes.

SUMMARY

Peripheral ulcerative keratitis and necrotizing scleritis are reported after ocular trauma or surgery. We report a case of a Rheumatoid Arthritis patient that presented with a progressive marginal furrow after extracapsular cataract extraction. The treatment included conjuntival resection and organic glue, colagenase inhibitors, bandage contact tens and systemic immunosupression. Peripheral ulcerative keratitis can be the initial or a late manifestation of Rheumatoid Arthritis, a vascular systemic disease with a death potential that require an adequade approach of these patients.
