Trabeculectomia: Retalho conjuntival base-limbo versus base-fórnice

Trabeculectomy: Limbal-based vs fornix-based conjunctival flap

RESUMO

São analisados, retrospectivamente, 18 olhos de 13 pacientes submetidos à trabeculectomia com retalho conjuntival de base-fórnice (grupo A) e 18 olhos de 15 pacientes com base-limbo (grupo B). O período médio de avaliação foi de 8,8 meses (3-19 meses) e 37,1 meses (3-76 meses), respectivamente. Não houve nenhuma diferença significativa entre a eficácia e a morbidade das duas técnicas cirúrgicas. No grupo A, 95% dos olhos estavam controlados (72% sem medicação), tendo sido a redução média da PIO de 23,97 mmHg (43,82%). No grupo B, 90% dos olhos estavam controlados (67% sem medicação), tendo sido a redução média da PIO de 19,19 mmHg (37,26%). Não ocorreu encistamento da bolha filtrante no grupo A. Por isto e pelas vantagens técnicas na variante com retalho conjuntival de base-fórnice, esta continua sendo a preferida dos autores, que a recomendam.

SUMMARY

Trabeculectomy with fornix-based conjunctial flap was performed in 18 eyes of 13 patients (group A) and with Iimbal-based flap in 18 eyes of 15 patients (group B). In a retrospective study, patients were followed up for an average of 8.8 months and 37.1 months. respectively. No significant difference between the two surgical techniques was noted concerning efficacy and morbidity. Group A, 95% of the eyes were controlled (72% without medication), with an average 23.97 mmHg decrease (43.82%) of ocular pressure. Group B, 90% of the eyes were controlled (67% without medication) with an average 19.19 mmHg decrease (37.26%) of ocular pressure. Cystic bleb was not observed in group A. Because of the practical advantages and the positive surgical results, the authors favour and recommend the fornix-based method.
