Manifestações oculares da síndrome EEC: revisão da literatura e relato de caso

Ocular manifestations of the EEC syndrome: case report and review of the literature

RESUMO

A “Síndrome EEC” (ectrodactilia, displasia ectodérmica e fenda labial e/ou palatina) é um raro distúrbio de herança autossômica dominante, expressividade variável e baixa penetrância. Além das malformações dos pés e das mãos, dos lábios e/ou pálato, a síndrome manifesta-se com as características típicas da displasia ectodérmica. As manifestações oculares incluem fotofobia, blefarite crônica, ceratoconjuntivite seca, vascularização corneana e alterações de vias lacrimais. Apresentamos um caso de ectrodactilia e displasia ectodérmica com importante comprometimento ocular ocasionando baixa acuidade visual. A ausência de fenda labial ou palatina é explicada pela variabilidade de expressão da síndrome. O pronto reconhecimento da “Síndrome EEC” e o adequado acompanhamento oftalmológico são indispensáveis, uma vez que a deteriorização progressiva da visão é, na maioria das vezes, o aspecto mais incapacitante da síndrome.

SUMMARY

The “EEC Syndrome” (ectrodactyly, ectodermal dysplasia and cleft lip/palate) is a rare disorder with autosomal dominant inheritance and incomplete penetrance. Ocular findings include photophobia, chronic blepharitis, keratoconjunctivitis sicca, superficial corneal vascularization and defects of lacrimal duct system. We are presenting a case report of ectrodactyly and ectodermal dysplasia with important ocular involvement causing decreased visual acuity. Clefting of the lip and palate were absent due, it is postulated, to the variable expressivity of EEC Syndrome. Early and continued ophthalmologic evaluation and management are imperative, since decreased visual acuity can be the major debilitating problem in this disorder.
