Abstract: Introduction: We sought to evaluate if internal medicine outpatients are candidates to COMPASS trial strategy (low dose rivaroxaban twice a day plus daily acetylsalicylic acid), to detail the inclusion and exclusion criteria of COMPASS trial presented by internal medicine patients and to evaluate the frequency of patients already under the strategy. Methods: Observational, retrospective, and transversal study in an internal medicine department of a tertiary university hospital. During a one-month period, we consulted all electronic medical records of internal medicine appointments to assess which patients presented inclusion and exclusion criteria of the COMPASS trial. Results: Of 228 patients, 40 (17.5%) met inclusion criteria for COMPASS trial. Twelve (30.0%) had coronary artery disease, 21 (52.5%) had peripheral artery disease, and 6 (1.5%) had both. One patient was already on the COMPASS trial strategy. Of those who met inclusion criteria, 70.0% (n = 28) presented at least one exclusion criteria. The most frequent was the use of oral anticoagulation (mostly due to atrial fibrillation), followed by the users of non-acetylsalicylic acid antiplatelet therapy. Conclusion: The number of internal medicine patients who are eligible to benefit from the COMPASS strategy is relevant. Internal medicine physicians must be aware of inclusion and exclusion criteria of this new prevention strategy to promptly apply it in clinical practice.

Resumo Introdução: O nosso objetivo foi avaliar se os doentes seguidos em consulta de Medicina Interna são elegíveis para a estratégia do estudo COMPASS (dose baixa de rivaroxabano bidiária e ácido acetilsalicílico diário), detalhar os seus critérios de inclusão e de exclusão e definir a frequência de doentes que já se encontravam sob esta estratégia terapêutica. Métodos: Estudo observacional, retrospetivo e transversal realizado num departamento de medicina interna de um hospital universitário e terciário. Durante um período de um mês, consultámos todos os registos médicos eletrónicos das consultas de Medicina Interna para avaliar quais os doentes que apresentavam critérios de inclusão e exclusão do estudo COMPASS. Resultados: Dos 228 doentes, 40 (17,5%) preenchiam os critérios de inclusão do estudo COMPASS. Doze (30,0%) tinham doença arterial coronária, 21 (52,5%) tinham doença arterial periférica e 6 (1,5%) tinham ambas. Um doente já se encontrava sob a estratégia. Dos que preenchiam os critérios de inclusão, 70,0% (n = 28) apresentavam, pelo menos, um critério de exclusão. O mais frequente era a anticoagulação oral (principalmente para o tratamento da fibrilhação atrial), seguido pela terapêutica antiplaquetária sem ácido acetilsalicílico. Conclusão: O número de doentes seguido em consulta de Medicina Interna elegíveis para beneficiar da estraté-gia do estudo COMPASS é relevante. Os internistas devem estar despertos para os critérios de inclusão e exclusão desta nova estratégia de prevenção para aplicá-la prontamente na prática clínica.

ABSTRACT: The present study evaluated the effect of increasing levels of wet brewery residue (WBR) as a source of roughage on nutrient intake, performance and ingestive behaviour in lambs finished in confinement. Thirty-two uncastrated male lambs were used, weaned at 50 days of age. The treatments comprised increasing levels of WBR as roughage in the diet: 31, 44, 57 and 70%, on a dry-matter. The concentrate consisted of disintegrated corn, soybean meal, calcitic limestone and mineral salt. There was a quadratic effect (P ≤ 0.05) from the increasing levels of WBR on the intake of DM, OM, CP and TCH. The intake of EE, NDF and ADF increased linearly (P ≤ 0.01), while TDN intake showed a linear reduction (P < .0001) for increases in the level of WBR. There was a quadratic effect (P ≤ 0.010) on the average daily weight gain (ADG) from the addition of WBR to the diet. The time spent on feeding and rumination (P ≤ 0.05), expressed in minutes, showed a linear increase. Feeding rate for DM showed decreasing linear behaviour (P ≤ 0.006), while NDF rumination rate showed increasing linear behaviour (P ≤ 0.013) due to the increased levels of WBR in the diet. There was a linear increase in the time spent on each feeding activity (P ≤ 0.007) with the increase in WBR in the diet. Maximum weight gain is achieved when 46.9% WBR is used as a source of roughage in the diet, and this level can be recommended for terminating lambs in confinement.

RESUMO: O objetivo do presente estudo foi avaliar o efeito de níveis crescentes de resíduo úmido de cervejaria (RUC) como fonte de volume sobre o consumo de nutrientes, desempenho e comportamento ingestivo de cordeiros terminados em confinamento. Foram utilizados 32 cordeiros machos não castrados, desmamados aos 50 dias de idade. Os tratamentos consistiram em níveis crescentes de RUC como volumoso na dieta: 31, 44, 57 e 70%, na matéria seca. O concentrado consistia em milho desintegrado, farelo de soja, calcário calcítico e sal mineral. Houve efeito quadrático (P ≤ 0,05) dos níveis crescentes de RUC sobre o consumo de MS, MO, PB e CHT. O consumo de EE, FDN e FDA aumentou linearmente (P < .0001), enquanto o consumo de NDT apresentou redução linear (P < .0001) para aumentos no nível de WBR. Houve um efeito quadrático (P ≤ 0,010) no ganho de peso médio diário (GMD) da adição de RUC à dieta. O tempo gasto com alimentação e ruminação (P ≤ 0,05) e para mastigação total (P ≤ 0,003), expresso em minutos, apresentou aumento linear. A taxa de alimentação para MS apresentou comportamento linear decrescente (P ≤ 0,006), enquanto a taxa de ruminação da FDN apresentou comportamento linear crescente (P ≤ 0,013) devido ao aumento dos níveis de RUC na dieta. Houve aumento linear no tempo gasto em cada atividade alimentar (P ≤ 0,007) com o aumento da RUC na dieta. O ganho de peso máximo é alcançado quando 46,9% de RUC é usado como fonte de volumoso na dieta, e este nível pode ser recomendado para cordeiros terminados em confinamento.

Abstract This observational study aimed to describe and analyze data from two external evaluations of the National Program for Improving Access to and Quality of Dental Specialty Centers (PMAQ CEO), held in 2014 and 2018 in Brazil, which evaluated Dental Specialty Centers (CEO) using a national and census approach. We selected questions through a search in the microdata of the first and second evaluations. The groups were analyzed independently. To compare the groups, nonparametric tests were performed (Mann Whitney U). The formulated hypotheses were: there would be no differences between the data of these groups (h0) and there would be differences between the data of these groups (h1). For qualitative nominal variables, frequency distribution was verified and association tests were performed (chi-square test). The significance level for this study was set at 5%. We observed that orthodontic treatments were found in about 13% of the CEO. Regarding human resources, most professionals were specialists or had MSc or PhD degrees; were civil servants; had been hired by direct administration; or had been hired via public tender. Regarding the work process and inclusion of the CEO in the health care network, we observed a greater number of services that use single and electronic medical records, greater presence of services monitoring and analyzing goals, greater knowledge about monthly average of absenteeism (for 2018); and larger number of services with referrals from primary health care centers (for 2014). Expanding the view on orthodontics and including preventive, interceptive, and corrective treatments at different points in health care networks are essential strategies for achieving comprehensive care in universal health systems.

ABSTRACT Objective: To identify the scientific evidence on the impacts caused by the use of screens during the COVID-19 pandemic in children and adolescents, raising reflections for future interventions with this public. Data source: This is an integrative literature review, conducted in the databases Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Cumulative Index to Nursing and Allied Health Literature (CINAHL), United States National Library of Medicine (PubMed), Scopus, Web of Science, and Embase, published from March 2020 to January 2022, in Portuguese, English and Spanish. Data synthesis: The search strategies allowed retrieving 418 articles, of which 218 were duplicates. The analysis of titles and abstracts resulted in the maintenance of 62 studies. Of these, 31 were excluded from the reading of the full text, since they did not clearly present the phenomenon investigated. Thirty-one were eligible, resulting in five categories: eye consequences; increased sedentary behavior and weight; change in eating habits; implications for sleep quality and impacts on mental health. Conclusions: The excessive use of screens during the pandemic led to numerous consequences for children and adolescents, with a higher incidence of visual damage, sedentary lifestyle, inadequate eating habit and increased weight gain, in addition to impaired sleep quality and mental health. This study provides subsidy for health professionals to carry out continuing education focused on this theme, and elaborate effective interventions for this public in this transition to the post-pandemic period.

RESUMO Objetivo: Identificar as evidências científicas sobre os impactos causados pelo uso de telas durante a pandemia da COVID-19 em crianças e adolescentes, almejando reflexões para futuras intervenções com esse público. Fontes de dados: Trata-se de uma revisão integrativa da literatura, realizada nas bases de dados Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Cumulative Index to Nursing and Allied Health Literature (CINAHL), United States National Library of Medicine (PubMed), Scopus, Web of Science e Embase, publicados de março de 2020 a janeiro de 2022, em português, inglês e espanhol. Síntese dos dados: As estratégias de busca possibilitaram recuperar 418 artigos, sendo 218 duplicados. A análise de títulos e resumos resultou na manutenção de 62 estudos. Destes, após a leitura do texto integral foram excluídos 31, uma vez que não apresentavam com clareza o fenômeno investigado. Foram elegíveis 31 estudos, emergindo cinco categorias: consequências oculares; aumento do comportamento sedentário e do peso; alteração dos hábitos alimentares; implicações na qualidade do sono e impactos na saúde mental. Conclusões: Percebe-se que o uso telas em excesso durante a pandemia trouxe inúmeras consequências para o público infantojuvenil, com maior incidência de acometimentos visuais, sedentarismo, alimentação inadequada e, por consequência, maior ganho de peso, além de prejuízos à qualidade do sono e à saúde mental. Este estudo fornece subsídios para que os profissionais da saúde realizem educação continuada voltada para essa temática e elaborem intervenções efetivas para esse público nesta transição para o período pós-pandêmico.

Resumo Introdução: Reconhecendo a importância dos exames de imagem na prática clínica e a influência impacto da informação clínica fornecida na qualidade do relatório radiológico produzido, propusemo-nos a estudar as práticas relacionadas com a completude dos campos de preenchimento obrigatório nas requisições dos exames de imagem pelos médicos de um hospital português. Materiais e métodos: Realizámos uma análise retrospetiva de 2930 exames de imagem requisitados durante o mês de maio de 2022 pelos médicos do nosso hospital, tendo sido incluídas ecografias, tomografias computorizadas e ressonâncias magnéticas. Os dados foram categorizados com base no preenchimento da informação fornecida (completa/incompleta/ausente), local de origem da requisição (consulta externa; hospital de dia; internamento; urgência), exame solicitado e região anatómica avaliada. A associação entre variáveis foi avaliada utilizando a análise qui-quadrado de Pearson. Resultados: Das 2930 requisições de exames de imagem, 71,4% foram acompanhadas de informação clínica adequadamente preenchida, 27,3% das requisições preenchidas de forma incompleta e 1,3% com informação ausente. O exame mais frequentemente solicitado foi a tomografia computorizada correspondendo a 68,1%. Foram identificadas associações significativas (p<0,05) entre a adequação do preenchimento da informação clínica e método de imagem requisitado, assim como entre o local de origem da requisição do exame e a completude da informação clínica e qual a informação em falta. A informação clínica foi a mais frequentemente preenchida de forma adequada nas requisições originadas na consulta externa 73,4%. Discussão: O facto de no nosso hospital nem todas as requisições serem acompanhadas de informação clínica adequadamente preenchida, mostra que existe espaço para otimização das práticas de preenchimento vigentes. Também a compreensão dos fatores com impacto no seu adequado preenchimento permite-nos desenvolver formas de atuação com vista à otimização dos recursos disponíveis. Algumas das medidas implementadas passam por programas de sensibilização e capacitação assim como adoção de sistemas de informação eletrónicos facilitadores desse processo. Além disso, é crucial promover uma comunicação aberta entre radiologistas e clínicos. Conclusão: Os nossos resultados sublinham a necessidade de melhorar a comunicação e formação no processo de solicitação de imagens para aprimorar a qualidade dos cuidados de saúde prestados no nosso hospital.

Abstract Introduction: Acknowledging the importance of imaging examinations in clinical practice and the impactful influence of provided clinical information on the quality of the produced radiological report, we aimed to study the practices related to the completeness of the mandatory fields in imaging requests by doctors working in a Portuguese hospital. Materials and methods: We retrospectively analysed 2930 imaging requests ordered during May 2022 by physicians at our hospital, including ultrasound, computed tomography and magnetic resonance. Data were categorized based on the completeness of the provided information (adequately filled out/incomplete/absent), the origin of the request (outpatient consultation; outpatient clinic; inpatient; emergency), requested examination, and evaluated anatomical region. The association between variables was assessed using Pearson's Chi-square analysis. Results: Of the 2930 imaging examination requests, 71.4% were accompanied by adequately filled clinical information, 27.3% of the requests were incompletely filled, and 1.3% were lacking information. The most frequently requested examination was computed tomography comprising 68.1% of total requests. Significant associations (p<0.05) were identified between the adequacy of clinical information completion and the requested imaging method, as well as between the origin of the examination request and the completeness of the clinical information and which information was missing. Clinical information was most frequently adequately filled in requests originating from outpatient consultation in 73.4%. Discussion: The fact that at our hospital not all requests are accompanied by adequately filled clinical information shows that there is room to optimize current practices. Understanding the factors impacting its adequate completion allows us to develop approaches aimed at optimizing available resources. Some of the implemented measures include awareness and training programs as well as the adoption of facilitating electronic information systems. Furthermore, fostering open communication between radiologists and clinicians is crucial. Conclusion: Our findings underscore the need for enhanced communication and training in the imaging request process to improve the quality of health care provided at our hospital.

ABSTRACT Objective: To assess the impact of different vertical positions on lung aeration in patients receiving invasive mechanical ventilation. Methods: An open-label randomized crossover clinical trial was conducted between January and July 2020. Adults receiving invasive mechanical ventilation for > 24 hours and < 7 days with hemodynamic, respiratory and neurological stability were randomly assigned at a 1:1 ratio to the sitting position followed by passive orthostasis condition or the passive orthostasis followed by the sitting position condition. The primary outcome was lung aeration assessed using the lung ultrasound score (score ranges from 0 [better] to 36 [worse]). Results: A total of 186 subjects were screened; of these subjects, 19 were enrolled (57.8% male; mean age, 73.2 years). All participants were assigned to receive at least one verticalization protocol. Passive orthostasis resulted in mean lung ultrasound scores that did not differ significantly from the sitting position (11.0 versus 13.7; mean difference, -2.7; [95%CI -6.1 to 0.71; p = 0.11). Adverse events occurred in three subjects in the passive orthostasis group and in one in the sitting position group (p = 0.99). Conclusion: This analysis did not find significant differences in lung aeration between the sitting and passive orthostasis groups. A randomized crossover clinical trial assessing the impact of vertical positioning on lung aeration in patients receiving invasive mechanical ventilation is feasible. RESUMO Objetivo: Avaliar o impacto de diferentes posicionamentos verticais na aeração pulmonar em pacientes em ventilação mecânica invasiva. Métodos: Trata-se de ensaio clínico aberto, randomizado e transversal, realizado entre janeiro e julho de 2020. Adultos em ventilação mecânica invasiva por mais de 24 horas e menos de 7 dias com estabilidade hemodinâmica, respiratória e neurológica foram distribuídos aleatoriamente em uma proporção de 1:1 à postura sentada seguida da condição de ortostatismo passivo ou o ortostatismo passivo seguido de postura sentada. O desfecho primário foi a aeração pulmonar avaliada pelo lung ultrasound score. O escore varia de zero (melhor) a 36 (pior). Resultados: Foram selecionados 186 indivíduos; destes, 19 foram incluídos (57,8% do sexo masculino; média idade de 73,2 anos). Todos os participantes foram selecionados para receber pelo menos um protocolo de verticalização. O ortostatismo passivo resultou em escores médios de aeração pulmonar por ultrassonografia que não diferiram significativamente da postura sentada (11,0 versus 13,7; diferença média, -2,7; IC95% -6,1 a 0,71; p = 0,11). Ocorreram eventos adversos em três indivíduos no grupo ortostatismo passivo e em um no grupo postura sentada (p = 0,99). Conclusão: Esta análise não encontrou diferenças significativas na aeração pulmonar entre os grupos ortostatismo passivo e postura sentada. É factível conduzir um estudo clínico transversal randomizado para avaliar o impacto do posicionamento vertical na aeração pulmonar em pacientes em ventilação mecânica invasiva. Infelizmente, o estudo foi interrompido devido à necessidade de tratar pacientes com COVID-19. Registro ClinicalTrials.gov: NCT04176445

ABSTRACT BACKGROUND AND OBJECTIVES: The type of questionnaire that aims to capture a patient’s perception/view of an aspect to be measured (e.g. pain intensity) is called Patient Reported Outcome Measure (PROM). One of the biggest challenges that clinicians and researchers often face is making a decision about which PROM to use for the assessment of their patient with pain, especially due to the lack of scientific literacy needed to understand the criteria and terms used in the field of measurement properties. Thus, the objectives of this study (part II) were: (I) to introduce basic concepts about PROMs with a focus on the terminology and criteria defined by the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) and (2) to describe the measurement properties of the validity, responsiveness and interpretability domains and propose a checklist for assessing the quality of PROMs’ measurement properties. METHODS: This study was produced using a search for articles from the COSMIN initiative. For didactic purposes, the text was divided into two parts. RESULTS: This article included a description of the measurement properties of the validity (content, structural, construct), responsiveness (must be assessed through accuracy analyses, AUC≥0.70) and interpretability (which provides the minimum clinically important change) domains. In addition, a checklist was proposed for determining the quality of the measurement properties of assessment instruments. CONCLUSION: This study described the measurement properties within the validity and responsiveness domains, and the importance of interpretability for obtaining the minimum clinically important difference. The proposed checklist for evaluating these properties can help clinicians and researchers to determine the quality of an instrument and make a decision about the best option available.

RESUMO JUSTIFICATIVA E OBJETIVOS: O tipo de questionário que pretende captar a percepção/visão de um paciente sobre um aspecto a ser medido (ex: intensidade da dor) é chamado de Instrumento de Medida Baseado no Relato do Paciente (Patient Reported Outcome Measure - PROM). Um dos maiores desafios que clínicos e pesquisadores costumam enfrentar é quanto a tomada de decisão sobre qual PROM utilizar para a avaliação de seu paciente com dor, especialmente devido à falta do letramento científico necessário para entender os critérios e termos empregados na área de propriedades de medida. Assim, os objetivos deste estudo (parte II) foram: (I) introduzir conceitos básicos sobre PROMs com enfoque na terminologia e critérios definidos através do COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN), e (2) descrever as propriedades de medida dos domínios validade, responsividade e interpretabilidade e propor um checklist para avaliação da qualidade das propriedades de medida de PROMs. MÉTODOS: Utilizando uma busca voltada para os artigos da iniciativa COSMIN, foi elaborado o presente estudo de revisão, que foi dividido em duas partes para fins didáticos. RESULTADOS: O presente artigo compreendeu a descrição das propriedades de medida dos domínios de validade (conteúdo, estrutural, construto), responsividade (deve ser avaliada através de análises de acurácia, AUC≥0,70) e interpretabilidade (que fornece a mínima mudança clinicamente importante). Além disso, foi proposto um checklist para determinação da qualidade das propriedades de medida de instrumentos de avaliação. CONCLUSÃO: Este estudo descreveu as propriedades de medida dentro dos domínios validade e responsividade, e a importância da interpretabilidade para a obtenção da mínima diferença clinicamente importante. O checklist proposto para avaliação dessas propriedades pode auxiliar clínicos e pesquisadores a determinarem a qualidade de um instrumento e tomar a decisão sobre a melhor opção disponível.

ABSTRACT Background: Alpha 1-antitrypsin deficiency (AATD) is a hereditary codominant autosomal disease. This liver disease ranges from asymptomatic cases to terminal illness, which makes early recognition and diagnosis challenging. It is the main cause of pediatric liver transplantation after biliary atresia. Objective: To describe the clinical characteristics, as well as those of histologic and laboratory tests, phenotypic and/or genetic evaluation and evolution of a cohort of pediatric patients with AATD. Methods: This is a retrospective observational study of 39 patients with confirmed or probable AATD (without phenotyping or genotyping, but with suggestive clinical features, low serum alpha 1-antitrypsin (AAT) level and liver biopsy with PAS granules, resistant diastasis). Clinical, laboratory and histological variables, presence of portal hypertension (PH) and survival with native liver have been analyzed. Results: A total of 66.7% of 39 patients were male (26/39). The initial manifestation was cholestatic jaundice in 79.5% (31/39). Liver transplantation was performed in 28.2% (11/39) of patients. Diagnosis occurred at an average of 3.1 years old and liver transplantation at 4.1 years of age. 89.2% (25/28) of the patients with confirmed AATD were PI*ZZ or ZZ. The average AAT value on admission for PI*ZZ or ZZ patients was 41.6 mg/dL. All transplanted patients with phenotyping or genotyping were PI*ZZ (or ZZ). Those who were jaundiced on admission were earlier referred to the specialized service and had higher levels of GGT and platelets on admission. There was no significant difference in the survival curve when comparing cholestatic jaundiced to non-cholestatic jaundiced patients on admission. Comparing patients who did or did not progress to PH, higher levels of AST and APRI score at diagnosis (P=0.011 and P=0.026, respectively) were observed and in the survival curves patients with PH showed impairment, with 20.2% survival with native liver in 15 years. Conclusion: Jaundice is an important clinical sign that motivates referral to a specialist, but it does not seem to compromise survival with native liver. Patients progressing to PH had higher AST, APRi score on admission and significantly impaired survival with native liver. It is important to pay attention to these signs in the follow-up of patients with AATD.

RESUMO Contexto: Deficiência de alfa 1-antitripsina (DAAT) é uma doença hereditária, de caráter autossômico codominante. A apresentação da doença hepática varia desde casos assintomáticos até doença terminal, o que dificulta reconhecimento e diagnóstico precoces. É a principal causa de transplante hepático pediátrico após atresia de vias biliares. Objetivo: Descrever as características clínicas, de exames laboratoriais, histológicos, avaliação fenotípica e/ou genética e sobrevida de uma coorte de pacientes pediátricos com DAAT. Métodos: Estudo observacional retrospectivo de 39 pacientes com diagnóstico de DAAT confirmada ou provável (sem fenotipagem ou genotipagem, mas com clínica sugestiva, baixo nível sérico de alfa 1-antitripsina (A1AT) e biópsia hepática com grânulos PAS, diástase resistentes). Variáveis clínicas, laboratoriais, histológicas, presença de hipertensão portal (HP) e sobrevida com fígado nativo foram analisadas. Resultados: Dos 39 pacientes, 66,7% eram do sexo masculino (26/39). A manifestação inicial foi icterícia colestática em 79,5% (31/39). Em 28,2% (11/39) houve necessidade de transplante hepático. O diagnóstico ocorreu com uma idade média de 3,1 anos e, o transplante hepático, 4,1 anos. Dos pacientes com DAAT confirmada, 89,2% (25/28) eram PI*ZZ ou ZZ. O valor médio de A1AT na admissão de pacientes PI*ZZ ou ZZ foi 41,6 mg/dL. Todos os transplantados com fenotipagem ou genotipagem eram PI*ZZ (ou ZZ). Os ictéricos à admissão foram referenciados mais cedo ao serviço especializado e apresentaram níveis mais elevados de GGT e plaquetas à admissão. Não houve diferença significativa na curva de sobrevida ao compararmos icterícia colestática ou não à admissão. Ao comparar os pacientes que progrediram ou não para HP, observou-se níveis mais elevados de AST e APRI escore ao diagnóstico (P=0,011 e P=0,026, respectivamente) e, nas curvas de sobrevida, pacientes com HP apresentaram comprometimento, com 20,2% de sobrevida com fígado nativo em 15 anos. Conclusão: Icterícia é um sinal clínico importante que motiva o encaminhamento ao especialista, mas parece não comprometer a sobrevida com fígado nativo. Pacientes com evolução para HP tiveram AST e escore APRi mais elevados à admissão e comprometimento significativo da sobrevida com fígado nativo. Importante atentar a esses sinais no seguimento de pacientes com DAAT.

Abstract The aim is to present validity evidence of the Brazilian-Portuguese Recovery Experience Questionnaire (REQ-PB) by applying a procedure to decentering cross-cultural scales translation and adaptation. First, we had a phase with bilingual experts, which assessed different criteria of translation quality. In sequence, we conducted the replication of the original research to achieve validity indicators in the Brazilian context. We carried out both Confirmatory Factor Analysis (to find structural validity indicators) and correlations with various external variables (to find convergent validity indicators). Step 1 showed promising results of decentering translation. In step 2 participated 164 workers and the CFA confirmed the four-factor model: psychological detachment from work, relaxation, mastery experience, and control over leisure time. The convergent validity showed a significant correlation with external variables. The REQ-PB showed adequate psychometric properties and may explain and compare empirical evidence of the recovery topic. We concluded that we have a good quality scale to be used in future research and integrated with other constructs to support interventions.

Resumo O objetivo é apresentar evidências da validade do Questionário de Experiência de Restauro em português brasileiro (REQ-PB) por meio da aplicação de um procedimento de descentralização para tradução e adaptação de escalas transculturais. Inicialmente, tivemos uma fase com especialistas bilíngues que avaliaram a qualidade da tradução utilizando diferentes critérios. Na sequência, realizamos a replicação da pesquisa original para alcançar os indicadores de validade no contexto brasileiro. Efetuamos tanto a análise fatorial confirmatória (para encontrar indicadores de validade estrutural) quanto correlações com variáveis externas (para encontrar indicadores de validade convergentes). O passo 1 mostrou resultados promissores na tradução descentralizada. No passo 2, participaram 164 trabalhadores e a análise fatorial confirmatória confirmou o modelo de quatro fatores: distanciamento psicológico do trabalho, relaxamento, experiência de domínio e controle do tempo livre. A validade convergente mostrou uma correlação significativa com variáveis externas. O REQ-PB mostrou propriedades psicométricas adequadas capazes de explicar e comparar evidências empíricas do tópico de restauro. Em resumo, a escala pode ser utilizada em pesquisas futuras e integrada aoutros construtos para orientar intervenções.

Abstract This study aimed to develop a scale to evaluate coping strategies in cancer patients. Initially, 112 items were created and subjected to theoretical analysis by expert judges, followed by a trial with the target population. After these steps, 42 items remained and were answered by 392 cancer patients. An exploratory factor analysis was conducted to assess the scale’s Internal structure validity. The resulting Coping Strategies Scale for Cancer Patients (E3PO) consists of 20 items grouped into two factors, explaining 36.05% of the total variance. The first factor, Treatment Engagement, includes 14 items (with a composite reliability of .83), which measure commitment to addressing the stressor. The second factor, Emotional Reactions, includes six items (with a composite reliability of .75) that evaluate coping based on the expression of negative emotions coming from the stressor. In the future, E3PO may help in the development of therapeutic plans and the evaluation of behavioral interventions aimed at enhancing coping strategies among cancer patients.

Resumen El objetivo de este estudio fue desarrollar una herramienta para evaluar el afrontamiento de pacientes oncológicos. Se construyeron 112 ítems que fueron sometidos a análisis teórico por parte de jueces especialistas y prueba piloto con la población objetivo. Los 42 ítems restantes tras esta etapa fueron contestados por 392 pacientes oncológicos. El análisis de la validez de la estructura interna se hizo por análisis factorial exploratorio. Denominad Escala de Estrategias de Afrontamiento para Pacientes Oncológicos (E3PO), los resultados señalaron una estructura de 20 ítems divididos en dos factores que explican 36.05% de la varianza total. Compromiso con el Tratamiento con 14 ítems (fiabilidad compuesta .83) evalúa el compromiso orientado hacia el cambio del estresor. Reacciones Emocionales con seis ítems (fiabilidad compuesta .75) evalúa el afrontamiento basado en la expresión de sentimientos negativos derivados del estresor. En el futuro, E3PO puede ser útil en la elaboración de planes terapéuticos y en la evaluación de intervenciones conductuales que se centren en el desarrollo o la mejora del afrontamiento del cáncer.

Resumo O objetivo desse estudo foi desenvolver um instrumento capaz de avaliar o enfrentamento de pacientes oncológicos. Foram construídos 112 itens que passaram por análise teórica de juízes especialistas e pré-teste com o público-alvo. Os 42 que restaram após essas etapas foram respondidos por 392 pacientes oncológicos. A análise de validade de estrutura interna foi realizada por análise fatorial exploratória. Denominada como Escala de Estratégias de Enfrentamento para Pacientes Oncológicos (E3PO), os resultados indicaram uma estrutura de 20 itens divididos em dois fatores que explicam 36,05% da variância total. Engajamento no Tratamento, 14 itens (confiabilidade composta de 0,83), avalia compromisso orientado para mudança do estressor. Reações Emocionais, seis itens (confiabilidade composta de 0,75), avalia enfrentamento baseado na expressão de sentimentos negativos advindos do estressor. A E3PO pode futuramente auxiliar na elaboração de planos terapêuticos e avaliação de intervenções comportamentais que tenham como foco o desenvolvimento ou melhoramento do enfrentamento ao câncer.

Abstract Objectives: To review and provide evidence-based recommendations for the diagnosis and treatment of otosclerosis. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on otosclerosis were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: 1) Diagnosis – audiologic and radiologic; 2) Treatment – hearing AIDS, pharmacological therapy, stapes surgery, and implantable devices - bone-anchored devices, active middle ear implants, and Cochlear Implants (CI). Conclusions: The pathophysiology of otosclerosis has not yet been fully elucidated, but environmental factors and unidentified genes are likely to play a significant role in it. Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness.

Abstract Epidermolysis Bullosa is a rare hereditary disease with genetic etiology in the vast majority of cases. It is characterized by the formation of blisters and ulcers on the skin or mucosa, caused by trauma or tissue manipulation. This disease has several subtypes, of which the recessive dystrophic is one of the most serious, with significant stomatological manifestations that deserve special attention from the dentist. The objective of this study is to report a clinical case of a 9-year-old patient with recessive dystrophic epidermolysis bullosa, addressing dental treatment with a focus on the particularities of the patient with epidermolysis bullosa

Resumo A epidermólise bolhosa é uma doença rara de caráter hereditário com etiologia genética na grande maioria dos casos. Ela é caracterizada pela formação de bolhas e úlceras na pele ou mucosa, causadas por traumas ou manipulação do tecido. Essa doença possui vários subtipos, sendo o distrófico recessivo um dos mais graves, com manifestações estomatológicas significativas e que merecem atenção especial pelo médico dentista. O objetivo do presente trabalho é relatar um caso clínico de um paciente de 9 anos de idade com epidermólise bolhosa distrófica recessiva, abordando o tratamento odontológico com foco direcionado para as particularidades do paciente com epidermólise bolhosa.

ABSTRACT The objective of this study is to present the protocol of a cluster randomized clinical trial to be conducted through the TeleICU project - Qualification of Intensive Care by Telemedicine. The study will consist of a cluster randomized clinical trial, open label, in pediatric intensive care units, with an allocation ratio of 1:1, to compare the intervention group (support of Telemedicine for patients admitted to the pediatric intensive care unit) with a control group (pediatric intensive care unit usual care). The study proposed to select 16 pediatric intensive care units, including 100 participants per site, with a total of 1,600 participants. The intervention group will receive telerounds from Monday to Friday and will have specialists and continuing education activities available. The primary outcome measure will be the length of stay in the pediatric intensive care unit, defined as the difference between the date of discharge of the participant and the date of admission to the intensive care unit. The secondary outcomes will be mortality rate, invasive mechanical ventilation-free days, days using antibiotics, days using vasoactive drugs and days using sedoanalgesia. This study will be conducted in accordance with Resolution 466/12 of the National Health Council, with approval by the Research Ethics Committee of the institutions involved. The present study has the potential to reproduce studies on Telemedicine in intensive care and may make important contributions to care in intensive care units in Brazil and other settings. If Telemedicine shows positive clinical care results compared to conventional treatment, more pediatric patients may benefit. ClinicalTrials.gov registry: NCT05260710

RESUMO O objetivo deste estudo será apresentar o protocolo de um ensaio clínico randomizado em cluster a ser realizado por meio do projeto TeleUTI - Qualificação da Assistência em Terapia Intensiva por Telemedicina. O estudo consistirá em um ensaio clínico randomizado por cluster, open label, em unidades de terapia intensiva pediátricas, com proporção de alocação de 1:1, com o intuito de comparar o grupo de intervenção (apoio da telemedicina para os pacientes internados na unidade de terapia intensiva pediátrica) com um grupo controle (cuidados habituais da unidade de terapia intensiva pediátrica). O estudo se propõe a selecionar 16 unidades de terapia intensiva pediátricas, incluindo 100 participantes por local, com o total de 1.600 participantes. O grupo intervenção receberá telerounds de segunda-feira a sexta-feira e terá à disposição especialistas e atividades de educação continuada. O desfecho primário a ser avaliado será o tempo de permanência nas unidades de terapia intensiva pediátricas, definido pela diferença entre a data de alta do participante com a data de admissão na unidade de terapia intensiva. Os desfechos secundários serão: taxa de mortalidade; dias livres de ventilação mecânica, dias de uso de antibióticos, dias de uso de drogas vasoativas e dias de uso de sedoanalgesia. Este estudo será conduzido em conformidade com a resolução 466/12 do Conselho Nacional de Saúde, com aprovação pelo Comitê de Ética em Pesquisa das instituições hospitalares envolvidas. O trabalho tem o potencial de reproduzir estudos sobre Telemedicina em cuidados intensivos, podendo trazer contribuições importantes ao atendimento em unidades de terapia intensiva no Brasil e em outras realidades. Se a Telemedicina mostrar resultados clínicos assistenciais positivos em relação ao tratamento convencional, mais pacientes pediátricos poderão ser beneficiados. ABSTRACT The emergence of HIV abruptly impacted health services in 1981, initially affecting restricted social groups. Currently, with the development of antiretroviral therapy, we are faced with a different disease from the one seen in the 80s, now with a chronic evolution. However, some barriers remain the same, such as the social, mental and physical impact of the diagnosis on the life of the person living with HIV. The objective of the present study was to verify the viral load, CD4+ lymphocyte count, and quality of life of people living with the human immunodeficiency virus. This was a cross-sectional observational research with a sample of 24 individuals of both sexes, with positive serology for HIV, in medical follow-up under antiretroviral treatment. A specific instrument was used to collect sociodemographic information. CD4+ count and HIV viral load results were collected from patients’ medical records. Quality of life (QoL) was assessed using the WHOQOL-HIV-BREF. Descriptive analysis was used to characterise the sample profile. Participants (46.8± 9.2 years) have completed high school (41.8%), with an income of one to three minimum wages (37.5%). They present a three-year diagnosis and CD4 lymphocyte count above 500 mm3 of blood (75%). Regarding QoL, the participants had a higher average for the physical domain (26.7± 5.3) and lower averages for the environment domains (10.9± 2.7) and social relationships (12.2± 2.8). The study concludes that the people living with HIV who participated in the study are young adults, with low income and level of education, with a low quality of life in relation to the environment and social relations domains.

RESUMO O surgimento do HIV impactou os serviços de saúde abruptamente em 1981, inicialmente afetando grupos sociais restritos. Atualmente, com o desenvolvimento da terapia antirretroviral, nos deparamos com uma doença diferente da encarada na década de 80, agora com uma evolução crônica. Todavia, algumas barreiras permanecem as mesmas, como o impacto social, mental e físico do diagnóstico na vida da pessoa que vive com HIV. O objetivo do presente estudo é verificar carga viral, contagem de linfócitos CD4+, e qualidade de vida de pessoas que vivem com o vírus da imunodeficiência humana. A presente pesquisa trata-se de um estudo observacional transversal com amostra de 24 indivíduos de ambos os sexos, com sorologia positiva para HIV, em acompanhamento médico sob tratamento antirretroviral. Foi utilizado um instrumento próprio para a coleta de informações sociodemográficas. Os resultados de contagem de CD4+ e a Carga Viral do HIV foram coletados dos prontuários médicos dos pacientes. A Qualidade de vida (QV) foi avaliada através do WHOQOL-HIV-BREF. Foi utilizada a análise descritiva para a caracterização do perfil da amostra. Os participantes (46,8± 9,2 anos) possuem ensino médio completo (41,8%), com renda de um a três salários mínimos (37,5%). Apresentam de um a três anos de diagnóstico e contagem de linfócitos CD4 acima de 500 mm3 de sangue (75%). Em relação à QV, os participantes apresentaram média mais alta para o domínio físico (26,7± 5,3) e mais baixa para os domínios de meio ambiente (10,9± 2,7) e relações sociais (12,2± 2,8). O estudo conclui que as pessoas que vivem com HIV participantes do estudo são adultos jovens, com renda e nível de escolaridade baixas, apresentam baixa qualidade de vida em relação aos domínios meio ambiente e relações sociais.

ABSTRACT This study aimed to characterize solar brightness, photoperiod, phenology and productivity of Chinook, Columbus, Cascade and Yakima Gold hop cultivars grown in the municipalities of Lages and São Joaquim in Santa Catarina state, southern Brazil. The phenological stages evaluated were: beginning of sprouting, emission of lateral branches, beginning of flowering, formation of cones and beginning of harvest in the 2018/2019 production cycle. Photoperiod and solar brightness were calculated with data obtained from the Environmental Resources and Hydrometeorology Information Center of Santa Catarina State, in Lages and São Joaquim. The observed results demonstrate hops develops at latitude 27º South, there was a difference in behavior in the phenological cycle of the cultivars, characterizing Yakima Gold as early, with 143 days and Columbus with 191 days, late. Since the first agricultural harvest, there is the production of cones. With 5.87 hours per day solar brightness average, in Lages, 3,300 kg of fresh hops were produced per hectare. In São Joaquim, with a 5.53 hour per day solar brightness average, 6,000 kg of fresh hops were produced per hectare.
