RESUMO OBJETIVO Identificar as normas jurídicas publicadas nos anos de 2020 e 2021 para regular direta ou indiretamente o teletrabalho em saúde no Brasil, analisando-as sob o enfoque dos seguintes temas, no contexto do teletrabalho: tempo da jornada de trabalho; ergonomia; ambiente laboral; segurança e saúde do trabalhador. MÉTODOS Pesquisa legislativa e documental, com abordagem qualitativa descritiva. Foram coletadas e selecionadas todas as normas jurídicas que tratam direta ou indiretamente da regulação do teletrabalho em saúde no Brasil, publicadas nas bases da Imprensa Nacional e dos Conselhos Profissionais da área da saúde até junho de 2021. RESULTADOS Destacam-se os seguintes: até junho de 2021 havia 113 normas jurídicas vigentes sobre a regulação do teletrabalho em saúde, sendo mais da metade (64) publicadas no ano de 2020. Dentre os achados, identificou-se um baixo número de normas voltadas a regular ou orientar aspectos relacionados ao tempo da jornada de trabalho; à ergonomia; ao ambiente laboral; à segurança e à saúde do trabalhador. Do total das 113 normas, apenas uma trata da jornada de trabalho e somente 13 destacaram a importância de que os ambientes de trabalho tenham condições para o oferecimento de um bom serviço. DISCUSSÃO Identificamos ausências, nas normas jurídicas selecionadas, de dispositivos que regulam o teletrabalho em saúde, a fim de prover a defesa dos direitos do trabalhador e dos pacientes, ou ainda para garantir condições favoráveis à realização do teletrabalho, seja realizado nos domicílios, seja em outro espaço.

RESUMO Objetivo: Inquérito nacional de cobertura vacinal aos 12 e 24 meses de idade foi realizado para investigar as quedas nas coberturas a partir de 2016. Métodos: Amostra de 37.836 nascidos vivos das coortes de 2017 e 2018 residentes nas capitais, Distrito Federal (DF) e 12 cidades com mais de 100 mil habitantes, acompanhados nos primeiros 24 meses por registros nas cadernetas de vacinação. Setores censitários foram estratificados segundo condições socioeconômicas, e o mesmo número de crianças foi incluído para cada estrato. Calcularem-se coberturas vacinais de cada vacina e coberturas completas aos 12 e 24 meses, doses aplicadas, válidas e oportunas. Fatores familiares, maternos e da criança associados à cobertura foram pesquisados. Os motivos para não vacinar analisados foram: contraindicações médicas, dificuldades de acesso, problemas no funcionamento do programa e hesitação vacinal. Resultados: Os resultados preliminares mostram que menos de 1% das crianças não foram vacinadas, as coberturas pelo esquema completo são menores que 75% em todas as capitais e no DF, as vacinas com mais de uma dose perdem cobertura progressivamente, há diferenças entre os estratos socioeconômicos, favoráveis aos estratos mais altos em algumas cidades e aos estratos mais baixos em outras. Conclusão: Houve realmente redução da cobertura vacinal em todas as capitais e no DF para as crianças nascidas em 2017 e 2018, denotando piora na execução do Programa Nacional de Imunizações durante os anos de 2017 a 2019. O inquérito realizado não mensurou os impactos da pandemia de COVID-19 que podem ter reduzido ainda mais as coberturas vacinais.

Conclusion: There was an actual reduction in full vaccination in all capitals and the Federal District for children born in 2017 and 2018, showing a deteriorating implementation of the National Immunization Program from 2017 to 2019. The survey did not measure the impacts of the COVID-19 pandemic, which may have further reduced vaccination coverage.

ABSTRACT. Weathering deterioration affects seed quality, especially in areas with excessive rainfall. This study aimed to evaluate the oxidative stress, physiological quality, and protein metabolism of seeds of different soybean cultivars under weathering deterioration at the pre-harvest phase. Six soybean cultivars (BMX Apolo, DM 6563, NS 5959, NA 5909, BMX Potência, and TMG 1175) were subjected to simulated rainfall at the R8 stage. Each level was divided into two applications at 72-h intervals: 60 mm (30 + 30), 120 mm (60 + 60), and 180 mm (90 + 90). Then, the seeds were harvested and evaluated for physiological potential, antioxidative enzymes, hydrogen peroxide, malondialdehyde, proteins, and protease activity. The simulated rainfall allowed the variation in seed moisture, promoting a significant reduction in germination and seed vigor, especially at 120 and 180 mm levels. There were also reductions in antioxidative enzyme activity with weathering deterioration (mainly for catalase, ascorbate peroxidase, and peroxidase), accumulation of hydrogen peroxide and malondialdehyde, and reductions in protein content and protease activity. The proposed rainfall system is efficient in inducing weathering deterioration during the pre-harvest phase and its deleterious effects. Weathering deterioration in soybean seeds in the pre-harvest stage is directly influenced by genotype.

Evaluation of nitrogen and phosphorus in surface reservoirs of the semi-arid region of Brazil using mass balance

ABSTRACT Water reservoirs were built in the semi-arid region to mitigate the effects of drought and water scarcity. However, these reservoirs are subject to variables that impact the volume and quality of the stored water, with the contribution of such nutrients as Total Nitrogen (TN) and Total Phosphorus (TP) being one of the main factors in intensifying the eutrophication process. The aim of this study was to quantify the principal inputs and outputs of nutrients (N and P) and calculate the nutrient balance, in order to verify the influence of seasonality on the import and export of these nutrients in surface reservoirs of the semi-arid region of Brazil. Seven campaigns were carried out between 2015 and 2016, with water samples collected at five points in the Pereira de Miranda reservoir and four in the General Sampaio reservoir. The inflow of nutrients was greater when the inflow rate of the water was higher. The balance showed the accumulation of 22.8 tons of TN and 2.8 tons of TP in the Pereira de Miranda reservoir, and 13 tons of TN and 1.5 tons of TP in the General Sampaio. These reservoirs are therefore retaining nutrients, acting as a storage site for nutrients transported by surface runoff in the basin of the Curu River. This increase in storage occurred mainly during the rainy season, when nutrient retention by the reservoirs impaired water quality, resulting in eutrophication, and making it difficult to meet the uses for which the reservoirs were intended.

Abstract This study examined how basic psychological needs satisfaction can predict youth participants’ life skills development according to gender and sport type. A sample of 461 Brazilian youth sport participants (325 boys and 136 girls) ranging between 10 and 17 years was included. Data collection was conducted through the Life Skills Scale for Sport and Basic Needs Satisfaction in Sport Scale. Overall, the three subscales of basic psychological needs satisfaction explained a significant amount of the variance in eight life skills (R2 range = .02 to .08 p < .05) and total life skills (R2 = .07; p < .001) in male athletes. Concerning female athletes, multiple regression analyses revealed significant variance only for teamwork (R2=.09; p<.01) and goal setting (R2= .06; p<.05). Regarding the type of sport, the model presented a significant amount of the variance in eight life skills (R2 range = .02 to .05 p < .05) and total life skills (R2 = .08; p < .001) in team sports. Lastly, in individual sports, multiple regression analyses revealed significant variance only on goal setting (R2= .04; p<.05). Findings showed that youth sport coaches could focus on youth participants’ basic psychological needs when trying to foster life skills development across sport contexts. Based on these findings, several suggestions for improving future research and practice in this area are presented.
Resumo Este estudo examinou como a satisfação das necessidades psicológicas básicas pode prever o desenvolvimento de competências para a vida dos jovens participantes de acordo com o género e o tipo de desporto. Foi incluída uma amostra de 461 atletas juvenis brasileiros (325 meninos e 136 meninas) com idades entre 10 e 17 anos. A coleta de dados foi realizada por meio da Escala de Habilidades para a Vida no Esporte e da Escala de Satisfação de Necessidades Psicológicas Básicas no Esporte. No geral, as três subescalas de satisfação de necessidades psicológicas básicas explicaram uma quantidade significativa da variância em oito habilidades para a vida (faixa R2 = 0,02 a 0,08 p < 0,05) e habilidades para a vida totais (R2 = 0,07; p < 0,001) em atletas masculinos. Em relação às atletas femininas, as análises de regressão múltipla revelaram variância significativa apenas para trabalho em equipe (R2=0,09; p<0,01) e estabelecimento de metas (R2= 0,06; p<0,05). Em relação ao tipo de esporte, o modelo apresentou uma quantidade significativa de variância em oito habilidades para a vida (faixa R2 = 0,02 a 0,05 p < 0,05) e habilidades para a vida totais (R2 = 0,08; p < 0,001) em equipes. Esportes. Por último, nos desportos individuais, as análises de regressão múltipla revelaram variância significativa apenas no estabelecimento de metas (R2= 0,04; p<0,05). Os resultados mostraram que os treinadores desportivos juvenis podem concentrar-se nas necessidades psicológicas básicas dos jovens participantes ao tentarem promover o desenvolvimento de competências para a vida em contextos desportivos. Com base nestas conclusões, são apresentadas uma série de sugestões para melhorar futuras pesquisas e práticas nesta área.

ABSTRACT Several aspects make nitrocellulose based powders adequate as a solid propellant for rockets and missiles and the prefe rable propellant to be used in firearms and artillery: it produces less smoke and less fouling than other propellants; its burning rate shows a well-defined relation to pressure and the composition of the gaseous mixture resulting from its decomposition by burning can be predicted with relatively high levels of accuracy. This last feature, if coupled with adequate thermodynamic models, leads to a reliable framework for the simulation of internal ballistics and, ultimately, to an efficient design. Some simpler models use an ideal gas approach, which is obviously not adequate, and some others perform corrections based on the Virial E quation of S tate for sake of algebraic simplicity. This work employs the Peng-Robinson equation of state to model the pressure-volume-temperature (PVT) behavior of the gases produced during the burning reaction, once it is known to be accurate in the high pressures obtained inside combustion chambers. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

ABSTRACT This study was conducted to evaluate the effects of dietary supplementation with capsaicinoids on the performance and gene expression of broilers. At 18 days of age, 120 male broilers chickens (Cobb 500) were distributed in a completely randomized design with three treatments, eight replicates, and five birds per experimental unit. The treatments were a basal diet or basal diet with the addition of 1 or 2 mg of capsaicinoids/kg of diet. The birds had free access to water and feed throughout the experimental period (18 to 26 days of age). Broiler performance was evaluated at 26 days of age, and one bird per experimental unit was selected to collect serum and jejunum samples. Jejunum samples were used to analyze the mRNA content. Data were analyzed using one-way ANOVA, and the means were compared with Tukey’s test at a significance of 0.05. There was no effect of capsaicinoid supplementation on performance, serum metabolites, or the expression of glutathione peroxidase mRNA in the jejunum of broilers. However, broilers supplemented with capsaicinoids showed a higher mRNA expression of Cu, Zn-superoxide dismutase and a reduced mRNA expression of nuclear factor-κB in the jejunum. Supplementation with 1 and 2 mg/kg capsaicinoids did not improve the performance of broilers from 18 to 26 days of age but increased the mRNA expression of Cu, Zn-superoxide dismutase and reduced the mRNA expression of nuclear factor-κB in the jejunum of broilers.

RESUMO Os canabinoides compreendem os endocanabinoides, fitocanabinoides e os canabinoides sintéticos e desempenham ações no sistema nervoso central e periférico. Uma quantidade enorme de publicações tem sido lançada nos últimos anos, embora a cannabis seja conhecida por milênios. Os Departamentos Científicos da Academia Brasileira de Neurologia descreveram as evidências do uso médico em suas áreas. A literatura está em constantes mudanças e possíveis novas evidências podem surgir nos próximos dias ou meses. A prescrição dessas substâncias deve ser discutida com os pacientes e suas famílias, com conhecimento sobre eventos adversos e sua eficácia.
ABSTRACT Cannabinoids comprehend endocannabinoids, phytocannabinoids, and synthetic cannabinoids, with actions both in the central and peripherical nervous systems. A considerable amount of publications have been made in recent years, although cannabis has been known for over a thousand years. Scientific Departments from the Brazilian Academy of Neurology described evidence for medical use in their areas. Literature is constantly changing, and possible new evidence can emerge in the next days or months. Prescription of these substances must be discussed with patients and their families, with knowledge about adverse events and their efficacy.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Objective: To build and validate the content of an instrument to support the teaching and learning of the nursing process in the Post-Anesthetic Recovery Room, intended for nursing students. Method: This is a methodological study with a quantitative approach. In the content validation, seven judges, nurse doctors and postdoctors participated in teaching and research in the area of perioperative nursing. In the semantic analysis, 30 nursing students participated. The construction of the instrument was based on a literature review, Horta's theoretical framework and a nursing classification system. In data analysis, the Content Validity Index and the Agreement Index were calculated. Results: In the content validation, the instrument obtained Content Validity Index high in the psychometric criteria: relevance (CVI = 0.99), clarity (CVI = 1) and objectivity (CVI = 1). In the semantic analysis, the concordance index of the items was also high (CI> 0.80). Conclusion: The instrument was considered valid and understandable, with the potential to be a pedagogical tool that facilitates the process of caring for patients in the Post-Anesthetic Recovery Room.
Objetivo: Construir e validar o conteúdo de um instrumento para subsidiar o ensino e aprendizagem do processo de enfermagem em Sala de Recuperação Pós-Anestésica, destinado aos acadêmicos de enfermagem. Método: Trata-se de um estudo metodológico com abordagem quantitativa. Na validação de conteúdo participaram sete juízes, enfermeiros doutores e pós-doutores atuantes no ensino e pesquisa na área da enfermagem perioperatória. Na análise semântica, participaram 30 acadêmicos de enfermagem. A construção do instrumento foi pautada em revisão de literatura, referencial teórico de Horta e sistema de classificação de enfermagem. Na análise dos dados, o Índice de Validade de Conteúdo e o Índice de Concordância foram calculados. Resultados: Na validação de conteúdo, o instrumento obteve Índice de Validade de Conteúdo elevados nos critérios psicométricos relevância (IVC =0,99), clareza (IVC=1) e objetividade (IVC=1). Na análise semântica, o Índice de Concordância dos itens também foi elevado (IC>0,80). Conclusão: O instrumento foi considerado válido e compreensível, com potencial para ser uma ferramenta pedagógica facilitadora do processo de cuidar dos pacientes em Sala de Recuperação Pós-Anestésica.

SUMMARY OBJECTIVE: Several prospective randomized trials have shown that hypofractionation has the same efficacy and safety as the conventional fractionation in the treatment of localized prostate cancer. There are many benefits of hypofractionation, including a more convenient schedule for the patients and better use of resources, which is especially important in low- and middle-income countries like Brasil. Based on these data, the Brazilian Society of Radiotherapy (Sociedade Brasileira de Radioterapia) organized this consensus to guide and support the use of hypofractionated radiotherapy for localized prostate cancer in Brasil. METHODS: The relevant literature regarding moderate hypofractionation (mHypo) and ultra-hypofractionation (uHypo) was reviewed and discussed by a group of experts from public and private centers of different parts of Brasil. Several key questions concerning clinical indications, outcomes and technological requirements for hypofractionation were discussed and voted. For each question, consensus was reached if there was an agreement of at least 75% of the panel members. RESULTS: The recommendations are described in this article. CONCLUSION: This initiative will assist Brazilian radiation oncologists and medical physicists to safely treat localized prostate cancer patients with hypofractionation.

BACKGROUND The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) B.1.1.33-derived lineage named N.9 was described recently in Brazil and it’s considered a potential variant of interest (VOI) due to the presence of E484K substitution at the receptor-binding domain (RBD) of the Spike (S) protein. OBJECTIVE To describe the first detection of variant N.9 in Rio de Janeiro State. METHODS SARS-CoV-2 N.9 was confirmed by quantitative reverse transcription polymerase chain reaction (qRT-PCR), whole-genome sequencing and phylogenetic analysis. FINDINGS Here, we report two SARS-CoV-2 N.9 lineage strains in Rio de Janeiro. One of them had only the E484K substitution of the six N.9 lineage-defining mutations. Other three strains pre-defined as N.9 have the same genomic profile. These four strains are grouped within the B.1.1.33 lineage and basal to the N.9 lineage in our phylogenetic analysis, and we call them “N.9-like/B.1.1.33 + E484K”. MAIN CONCLUSIONS The phylogenetic analysis shows four independent introductions of N.9 in the state of Rio de Janeiro in October and December 2020, January and March 2021. SARS-CoV-2 N.9 dissemination in the Rio de Janeiro could have been limited by the emergence and dominance of other variants, mainly by the lineage P.2 VOI Zeta that emerged in the same period and co-circulated with N.9, as observed in the neighboring State of São Paulo.
