Abstract Objective To evaluate a fixed UV-C light emitting device for its antimicrobial effectiveness in the disinfection of distinct surfaces and its antifungal effectiveness on air quality in the hospital environment. Methods This quasi-experimental study was conducted in a hospital inpatient unit, in which a six-stage air Biosampler (Andersen®) was used for air analysis. In the evaluation of surfaces, three suspensions of microorganisms (Acinetobacter sp. multidrug-resistant, Escherichia coli, and KPC-producing Klebsiella pneumoniae) were used to contaminate the environment. In both evaluations, pre- (control) and post-activation of UV-C light (test) collections were made. Results In the air evaluation, an important reduction was observed in the colony count after irradiation with UV-C light, and pathogenic or toxigenic fungi were not found in either of the two moments. Regarding the disinfection of surfaces, no bacterial growth was observed after the application of UV-C light, showing 100% bacterial inactivation under the tested conditions. Conclusion The use of fixed UV-C light emission technology was effective and can be considered a promising intervention for hospital surface disinfection protocols.

Resumen Objetivo Evaluar la eficacia antimicrobiana de un dispositivo fijo emisor de luz UV-C para la desinfección de diferentes superficies del ambiente hospitalario y su eficacia antifúngica en la calidad del aire. Métodos Estudio cuasi experimental realizado en una unidad de internación hospitalaria, en que se utilizó el biomuestreador de aire Andersen® de seis etapas para el análisis del aire. En el análisis de las superficies, se utilizaron tres suspensiones de microorganismos (Acinetobacter sp. MDR, Escherichia coli y Klebsiella pneumoniae productora de KPC) para contaminar el ambiente. En ambos se tomó una muestra antes (control) y después de accionar la luz UV-C (prueba). Resultados En el análisis del aire hubo una reducción importante del recuento de colonias después de la luz UV-C y no se encontraron hongos patógenos ni toxigénicos en ninguno de los dos momentos. Con relación a la desinfección de las superficies, no se observó ningún crecimiento bacteriano después de la intervención de la luz, lo que demuestra un 100 % de inactivación bacteriana en las condiciones analizadas. Conclusión El uso de la tecnología con emisión de luz UV-C fija fue eficaz y puede ser considerada una intervención prometedora para protocolos de desinfección de superficies hospitalarias.

Resumo Objetivo Avaliar a eficácia antimicrobiana de um dispositivo fixo emissor de luz UV-C na desinfecção de diferentes superfícies do ambiente hospitalar e sua eficácia antifúngica na qualidade do ar. Métodos Estudo quase-experimental realizado em uma unidade de internação hospitalar, que utilizou o Bioamostrador de ar Andersen® de seis estágios para análise do ar; e na avaliação das superfícies, utilizaram-se três suspensões de microrganismos (Acinetobacter sp. MDR, Escherichia coli e Klebsiella pneumoniae produtora de KPC) para contaminar o ambiente. Para ambos foram feitas coletas pré (controle) e pós-acionamento da luz UV-C (teste). Resultados Na avaliação do ar houve uma redução importante da contagem de colônias após a luz UV-C e não foram encontrados fungos patogênicos ou toxigênicos em nenhum dos dois momentos. Em relação à desinfecção das superfícies, nenhum crescimento bacteriano foi observado após a intervenção da luz, demonstrando 100% de inativação bacteriana nas condições testadas. Conclusão A utilização da tecnologia com emissão de luz UV-C fixa foi eficaz e pode ser considerada uma intervenção promissora para protocolos de desinfecção de superfícies hospitalares.

ABSTRACT The tropical forest is extremely exploited and fragmented, making it essential to collect native seeds to meet the growing demand for its restoration and maintenance of biodiversity. Thus, the objective is to select potential forest fragments with a higher degree of conservation for seed harvesting, through the use and association of landscape ecology with fuzzy logic. The study was carried out in the watershed of the Itapemirim river. The stages of selection and photointerpretation of the images were carried out; error determination; application of landscape ecology metric indices; application of fuzzy logic in a computational application and validation of the methodology in loco. 7,515 forest fragments were determined, corresponding to 19.21% of the study area, with 89.53% of hits. Fragments smaller than 5 ha are the most fragile and are at risk of extinction, while those larger than 300 ha have a lower risk of extinction, even with the increase in the edge. With the application of fuzzy logic, the mean was between 0.15, standard deviation of 0.24 and the coefficient of variation at 161.73%. Scenario 1 (FLONA of Pacotuba), has 10.25% of families, 25.92% of genera and 33.62% of species more than Scenario 2 (PEAMA Ifes Campus of Alegre), among the identified individuals. The association of landscape ecology techniques and fuzzy logic made it possible to identify fragments with a higher degree of conservation, with potential for harvesting forest seeds.

RESUMO A floresta tropical é extremamente explorada e fragmentada, sendo imprescindível a coleta de sementes nativas, a fim de atender a crescente demanda para sua restauração e manutenção da biodiversidade. Dessa forma, objetiva-se selecionar fragmentos florestais potenciais com maior grau de conservação para a colheita de sementes, via utilização e associação da ecologia da paisagem à lógica Fuzzy. O estudo foi realizado na bacia hidrográfica do rio Itapemirim. Foram realizadas as etapas de seleção e fotointerpretação das imagens; determinação do erro; aplicação dos índices métricos de ecologia da paisagem; aplicação da lógica Fuzzy em aplicativo computacional e validação da metodologia in loco. Foram determinados 7.515 fragmentos florestais, correspondendo a 19,21% da área de estudo, com 89,53% de acertos. Os fragmentos menores que 5 ha são os mais frágeis e possuem risco de extinção, enquanto os maiores que 300 ha possuem menor risco de extinção, mesmo com o aumento da borda. Com a aplicação da lógica Fuzzy, a média ficou entre 0,15, desvio padrão de 0,24 e o coeficiente de variação em 161,73 %. O Cenário 1 (FLONA de Pacotuba) possui 10,25% de famílias, 25,92 % de gêneros e 33,62% de espécies a mais que o Cenário 2 (PEAMA Ifes Campus de Alegre), entre os indivíduos identificados. A associação das técnicas de ecologia da paisagem e lógica Fuzzy possibilitou identificar os fragmentos em maior grau de conservação, com potencial para colheita de sementes florestais.

RESUMO Objetivo: Inquérito nacional de cobertura vacinal aos 12 e 24 meses de idade foi realizado para investigar as quedas nas coberturas a partir de 2016. Métodos: Amostra de 37.836 nascidos vivos das coortes de 2017 e 2018 residentes nas capitais, Distrito Federal (DF) e 12 cidades com mais de 100 mil habitantes, acompanhados nos primeiros 24 meses por registros nas cadernetas de vacinação. Setores censitários foram estratificados segundo condições socioeconômicas, e o mesmo número de crianças foi incluído para cada estrato. Calcularem-se coberturas vacinais de cada vacina e coberturas completas aos 12 e 24 meses, doses aplicadas, válidas e oportunas. Fatores familiares, maternos e da criança associados à cobertura foram pesquisados. Os motivos para não vacinar analisados foram: contraindicações médicas, dificuldades de acesso, problemas no funcionamento do programa e hesitação vacinal. Resultados: Os resultados preliminares mostram que menos de 1% das crianças não foram vacinadas, as coberturas pelo esquema completo são menores que 75% em todas as capitais e no DF, as vacinas com mais de uma dose perdem cobertura progressivamente, há diferenças entre os estratos socioeconômicos, favoráveis aos estratos mais altos em algumas cidades e aos estratos mais baixos em outras. Conclusão: Houve realmente redução da cobertura vacinal em todas as capitais e no DF para as crianças nascidas em 2017 e 2018, denotando piora na execução do Programa Nacional de Imunizações durante os anos de 2017 a 2019. O inquérito realizado não mensurou os impactos da pandemia de COVID-19 que podem ter reduzido ainda mais as coberturas vacinais.

ABSTRACT Objective: The national vaccination coverage survey on full vaccination at 12 and 24 months of age was carried out to investigate drops in coverage as of 2016. Methods: A sample of 37,836 live births from the 2017 or 2018 cohorts living in capital cities, the Federal District, and 12 inner cities with 100 thousand inhabitants were followed for the first 24 months through vaccine record cards. Census tracts stratified according to socioeconomic levels had the same number of children included in each stratum. Coverage for each vaccine, full vaccination at 12 and 24 months and number of doses administered, valid and timely, were calculated. Family, maternal and child factors associated with coverage were surveyed. The reasons for not vaccinating analyzed were: medical contraindications, access difficulties, problems with the program, and vaccine hesitancy. Results: Preliminary results showed that less than 1% of children were not vaccinated, full coverage was less than 75% at all capitals and the Federal District, vaccines requiring more than one dose progressively lost coverage, and there were inequalities among socioeconomic strata, favorable to the highest level in some cities and to the lowest in others. Conclusion: There was an actual reduction in full vaccination in all capitals and the Federal District for children born in 2017 and 2018, showing a deteriorating implementation of the National Immunization Program from 2017 to 2019. The survey did not measure the impacts of the COVID-19 pandemic, which may have further reduced vaccination coverage.

RESUMEN Paraguay se propuso alcanzar cobertura y acceso universal mediante redes basadas en Atención Primaria de la Salud con Unidades de Salud Familiar (USF). Un desafío es abordar las enfermedades no transmisibles, causantes de mortalidad prematura en el país. El objetivo del estudio fue evaluar la capacidad de las USF para la atención de personas con hipertensión arterial y diabetes. El diseño fue observacional, descriptivo, transversal con componente analítico. Incluyó a 172 USF de las 4 Regiones Sanitarias que conforman el Eje Sur. Se adaptó el método de evaluación SARA de la OMS con 75 indicadores trazadores, aplicando un cuestionario dirigido a profesionales de salud entre noviembre y diciembre de 2022. Se calculó índices de síntesis de disponibilidad y preparación que se midieron de 0 a 1. La mediana del índice de disponibilidad fue 0,71, el de preparación 0,76 y el índice SARA USF-DM/HTA 0,74. La disponibilidad de medicamentos fue la principal brecha seguida de equipamiento básico para neuro exploración, equipos de comunicación e internet. En la preparación resultaron con menor desempeño actividades de supervisión y evaluación, así como la gestión de casos en la red de servicios. Solo el 44,2 % de las USF tuvo un desempeño de disponibilidad y preparación suficiente. Las USF rurales tuvieron menor desempeño que las urbanas. Hubo diferencia significativa entre las regiones sanitarias p<0,001. En conclusión, las USF presentaron limitaciones para la atención de personas con diabetes e hipertensión en estas regiones del país.

ABSTRACT Paraguay set out to achieve universal coverage and access through networks based on Primary Health Care with Family Health Units (FHU). One challenge is to address non-communicable diseases, which cause premature mortality in the country. The objective of the study was to evaluate the capacity of the FHU to provide care to people with arterial hypertension and diabetes. The design was observational, descriptive, cross-sectional with an analytical component. It included 172 FHU from the four Sanitary Regions that make up the Southern Axis. The WHO SARA evaluation method was adapted with 75 tracer indicators, applying a questionnaire addressed to health professionals between November and December 2022. Availability and preparation synthesis rates were calculated, which were measured from 0 to 1. The median of the availability index was 0.71, the readiness index 0.76, and the SARA USF-DM/HTA index 0.74. The availability of medicines was the main gap followed by basic equipment for neuro examination, communication equipment and the Internet. In readiness, activities like supervision, evaluation, as well as case management in the service network resulted in lower performance. Only 44.2% of the FHUs had sufficient readiness and availability performance. Rural FHUs performed worse than urban ones. There was a significant difference between the health regions p<0,001. In conclusion, the FHUs presented limitations for diabetes and arterial hypertension patient care in these regions of the country.

Abstract The objectives of the study were to (1) describe the kinematic parameters of spermatozoa (2) compare methods of evaluating sperm viability (3) validate assays of functionality and integrity of the sperm membrane and (4) evaluate possible changes between spermatozoa from the epididymis and the vas deferens of the greater rhea. Semen samples were recovered from 7 adult individuals. Sperm motility was characterized by adjusting the set-up for Computer-assisted semen analysis (CASA) to that new species. For sperm viability evaluation, smears of bromophenol blue and eosin-nigrosine dyes were used. Five solutions of different osmolarities were then tested for the hypoosmotic swelling test (HOST). The combination of fluorescent probes (propidium iodide - IP and Hoechst 33342) was also used to assess plasma membrane integrity. Data were presented as mean ± SEM. Rhea spermatozoa from the vas deferens had an overall motility of 14.6 ± 2.5%. The bromophenol blue staining technique revealed that 64.6 ± 5.2% sperm were viable, while that proportion was 72.1 ± 2.5% using eosin-nigrosine. An average of 77.6 ± 4.8% of spermatozoa reacted to the HOST with distilled water at 0 mOsm/l. Fluorescent probes indicated that 65.3 ± 2.6% of spermatozoa had intact membranes. Interestingly, no statistical differences were observed between the parameters analyzed in the epididymal spermatozoa and the vas deferens. These new assays set reference values that can now be used to further exploration of sperm handling conditions and freezing protocols in rheas.

Abstract Tooth development depends on a series of reciprocal signaling interactions between the oral epithelium and ectomesenchyme. This study aimed to investigate the role of CK14, a protein involved in Wnt-1/β-catenin signaling, in odontogenesis and the development of odontomas. This cross-sectional, retrospective, immunohistochemical study analyzed 30 compound odontomas, 30 complex odontomas, and 17 tooth germs. Higher immunoexpression of CK14 was observed in odontogenic epithelial cells of tooth germs (p < 0.001) and odontogenic epithelial cells of odontomas (p < 0.001). There was higher immunoexpression of Wnt-1 and β-catenin proteins in epithelial cells of tooth germs (p = 0.002 and p < 0.001, respectively), as well as in the ectomesenchyme of odontomas (p = 0.003 and p < 0.001, respectively). β-Catenin was moderately and significantly correlated with CK14 in the membrane of reduced enamel epithelial cells in odontomas (p = 0.007). Higher immunoexpression of CK14 was observed in the odontogenic epithelium during the bud and cap stages and lower immunoexpression in the internal enamel epithelium during the bell stage. In odontomas, lower expression of Wnt-1/β-catenin and higher immunoexpression of CK14 were found in odontogenic epithelial cells, especially adjacent to the mineralized material resembling the tooth formed in these lesions.

Resumo O desenvolvimento dentário depende de uma série de interações de sinalização recíproca entre o epitélio oral e o ectomesênquima. O objetivo deste estudo foi investigar o papel da CK14 das vias WNT-1/β-catenina na odontogênese e no desenvolvimento de odontomas. Este estudo transversal, retrospectivo, imuno-histoquímico analisou 30 odontomas compostos, 30 odontomas complexos e 17 germes dentários. A CK14 apresentou maior imunoexpressão em células epiteliais odontogênicas de germes dentários (p<0,001) e em células epiteliais odontogênicas de odontomas (p<0,001). A Wnt-1 e a β-catenina apresentaram maior imunoexpressão de proteínas nas células epiteliais dos germes dentários (p = 0,002 e p<0,001, respectivamente), bem como no ectomesênquima dos odontomas (p = 0,003 e p < 0,001, respectivamente). A β-catenina correlacionou-se moderada e significativamente com a CK14 na membrana de células epiteliais reduzidas do esmalte em odontomas (p = 0,007). Maior imunoexpressão da CK14 foi observada no epitélio odontogênico nos estágios de botão e capuz com menor imunoexpressão no epitélio interno do órgão do esmalte no estágio de sino. Nos odontomas, foi observado menor expressão de Wnt-1/β-catenina e maior imunoexpressão da CK14 presente nas células epiteliais odontogênicas, especialmente, vizinhas ao material mineralizado semelhante ao dente formado nessas lesões.

Resumo As lutas por reconhecimento das identidades coletivas sempre produziram tensões no trabalho de historiadores, sinalizando as implicações éticas e políticas do pensamento histórico, o caráter situado das premissas epistemológicas das práticas acadêmicas e a condição social marcada nos sujeitos que fazem e escrevem história. Os artigos deste dossiê abordam, a partir de perspectivas diversas, as marcas de corporeidade implicadas na operação historiográfica, considerando a noção de corpo não apenas como condição da experiência e da escrita da história, mas como o traço mais tangível das diferenças e da situacionalidade temporal e espacial dos sujeitos epistêmicos e de suas possibilidades de negociação e pertencimento. Abstract Social fights for the acknowledgement of collective identities have always produced tensions in the work of historians, revealing the ethical and political dimensions of historical thought, the situated character of the epistemological principles and paradigms of the academic practices, and the marked social condition of the subjects who make and write history. The articles of this special issue address, from different standpoints, the marks of corporeality implicated in the historiographical operation, considering the body not only as a condition of the experience and writing of history, but as the most tangible trace of the positionality of the epistemic subjects and of their negotiation and belonging options. ABSTRACT Chagas disease is among the 21 neglected diseases according to the World Health Organization. This study aimed to investigate the morbidity and mortality distribution of Chagas disease for identifying areas with greater prevalences and deaths of the disease in Northeast Brazil. A population-based ecological study was performed from 2016 to 2018 using data on acute Chagas disease patients from the Disease Notification Information System, chronic cases from the Chagas Disease and the referral Heart Failure Outpatient Clinic in Pernambuco, and Chagas disease-related mortality from the Mortality Information System. The unit of analysis were Pernambuco State mesoregions. The indicators were spatialized into thematic maps on the occurrence and mortality of the disease per 100,000 inhabitants. No cases of acute disease were reported in the period analyzed. Data on 801 chronic Chagas disease patients were analyzed. The population showed an average age of 62 years, with female predominance. The most prevalent comorbidity was systemic arterial hypertension and cardiologic involvement without ventricular dysfunction. The average chronic disease occurrence rate was 3.2/ 100,000 people/ year. As for deaths in the mortality system; in total, 350 deaths were recorded, showing male predominance, age ≥ 60 years, and chronic disease with cardiac involvement as the main mortality cause. The annual average mortality proportion was 1.6/100,000 people. The chronic case distribution showed spatial heterogeneity, with the highest rates of chronic disease and deaths observed in two mesoregions, with the main cause of death being heart-related. This highlights the need for more specialized services in areas with higher burden of the disease to avoid delay in the patients’ care.

Abstract Verruciform xanthoma (VX) is a rare benign lesion of unknown etiology, with a rough or papillary aspect, painless, sessile, well-defined, most lesions do not exceed 2 cm in their largest diameter, the degree of keratinization of the surface influences color, varying white to red, affecting mainly the gingiva and alveolar mucosa, and can also be seen in skin and genital. Herein, we present a report a clinical case of oral verruciform xanthoma in the buccal mucosa associated with the lichen planus lesion, as well as the morphological and immunohistochemical characteristics of the lesion. The clinical diagnostic hypothesis of oral lichen planus of the white reticular lesions on the buccal mucosa and on the tongue was confirmed by histopathology before a subepithelial connective tissue exhibiting intense inflammatory infiltrate in a predominantly lymphocytic band. In contrast, the hypothesis of the verrucous lesion in the left buccal mucosa was leukoplakia, with histopathological evidence showing exophytic and digitiform proliferations with parakeratin plugs between the papillary projections. Subepithelial connective tissue was characterized by macrophages with foamy cytoplasm (xanthoma cells). An immunohistochemical examination was performed, showing positivity for CD68, a macrophage marker, in addition to testing by Schiff's periodic acid (PAS) with diastasis, which was detected the presence of lipids inside these macrophages. The patient is free of recurrences of verruciform xanthoma and is being monitored due to the presence of lesions of oral lichen planus.

It is well documented that Chagas disease (CD) can pose a public health problem to countries. As one of the World Health Organization Neglected Tropical Diseases undoubtedly calls for comprehensive healthcare, transcending a restricted biomedical approach. After more than a century since their discovery, in 1909, people affected by CD are still frequently marginalised and/or neglected. The aim of this article is to tell the story of their activism, highlighting key historical experiences and successful initiatives, from 1909 to 2019. The first association was created in 1987, in the city of Recife, Brazil. So far, thirty associations have been reported on five continents. They were created as independent non-profit civil society organisations and run democratically by affected people. Among the common associations’ objectives, we notably find: increase the visibility of the affected; make their voice heard; build bridges between patients, health system professionals, public health officials, policy makers and the academic and scientific communities. The International Federation of Associations of People Affected by CD - FINDECHAGAS, created in 2010 with the input of the Americas, Europe and the Western Pacific, counts as one of the main responses to the globalisation of CD. Despite all the obstacles and difficulties encountered, the Federation has thrived, grown, and matured. As a result of this mobilisation along with the support of many national and international partners, in May 2019 the 72nd World Health Assembly decided to establish World Chagas Disease Day, on 14 April. The associative movement has increased the understanding of the challenges related to the disease and breaks the silence around Chagas disease, improving surveillance, and sustaining engagement towards the United Nations 2030 agenda.
