Abstract Background: Inflammatory bowel diseases’ (IBD) increasing incidence and prevalence place a heavy health and economic burden on society. Objectives: This study assesses the burden and cost of IBD in Portugal to support the definition of health policies, resource allocation, and patient care. Methods: The burden of disease was expressed using disability-adjusted life years (DALY). Costs were estimated considering the societal perspective, using a prevalence-based model and prices established by law. An expert panel composed of 5 expert Portuguese gastroenterologists and a patient-reported study were conducted to support the cost analysis and fill in information gaps. Results: In Portugal, with a prevalence of 24,069 IBD patients and an incidence of 15/100,000, the burden of disease was estimated at 6,067 DALYs: 507 resulting from premature deaths and 5,560 from disability. Total cost was estimated at EUR 146 million per year, with direct costs representing 59%. Average yearly cost per IBD patient is EUR 6,075, where 60% is related to Crohn’s disease and 40% to ulcerative colitis (UC). Conclusion: This study estimates the annual health burden and cost of IBD in Portugal, thus generating information with the intent to raise awareness of the need to advance health policies as well as better clinical and economic decisions in this pathology.

Resumo Contexto: A crescente incidência e prevalência das Doenças Inflamatórias Intestinais (DII) representam um pesado fardo para a saúde e economia na sociedade. Objetivos: Este estudo avalia o custo e a carga da DII em Portugal, com o objetivo de suportar a definição de políticas de saúde, alocação de recursos e cuidados com o doente. Métodos: A carga da doença foi calculada utilizando anos de vida ajustados à incapacidade (DALY). Os custos foram estimados tendo em conta a perspetiva da sociedade, utilizando um modelo baseado na prevalência e preços estabelecidos por lei. Foi realizado um painel de peritos, composto por 5 gastroenterologistas portugueses, assim como um estudo de mercado a doentes, de forma a suportar a análise de custos e colmatar lacunas de informação. Resultados: Em Portugal, com uma prevalência de 24,069 doentes e uma incidência de 15/100,000, o peso das DII foi estimado em 6.067 DALYs: 507 dos quais resultantes de mortes prematuras e 5.560 de incapacidade. O custo total foi estimado em 146 milhões de euros por ano, com os custos diretos a representarem 59% do total. O custo médio anual por doente de DII é de 6.075 EUR, onde 60% está relacionado com Doença de Crohn (DC) e 40% com Colite Ulcerosa. Conclusão: Este estudo estima os encargos anuais para a saúde e o custo da DII em Portugal, gerando informação relevante, com o intuito de alertar para a necessidade de uma evolução nas políticas de saúde, assim como como suportar melhores decisões clínicas e económicas nesta patologia.

Abstract Background and Aim: Aeromonas are Gram-negative rods known to cause a spectrum of diseases. Inflammatory bowel disease (IBD) is an idiopathic complex condition resulting from interaction of multiple factors. Aeromonas infection in association with IBD is still largely unknown. We aim to look for the significance of Aeromonas infection and for significant differences between IBD and non-IBD patients. Methods: A retrospective observational analysis was performed of all patients positive for Aeromonas in stool cultures, during a 10-year period, from a tertiary and university hospital. Results: Fifty patients were included, 56% male with a mean age of 42.1 years. Thirty-eight (76%) were non-IBD and 12 (24%) IBD patients. IBD patients were more frequently under immunosuppressors. Two patients were asymptomatic and 44% developed mild, 44% moderate, and 16.7% severe infection. The main strains isolated were Aeromonas hydrophila/caviae. Bacterial co-isolation was found in 4 non-IBD and histological findings of cytomegalovirus in 2 IBD patients. Non-IBD patients presented more frequently with fever and IBD patients with bloody diarrhea and abdominal pain. There was higher tendency for severe infection rate in IBD patients with higher antimicrobial therapy use. Steroids were exclusively used in the IBD group. From IBD, 4 patients had the diagnosis of ulcerative colitis and 9 of Crohn’s disease with colonic involvement. Of these patients, 5 received IBD diagnosis after the acute episode of Aeromonas infection. Conclusions: Clinical presentation of Aeromonas infection differs between IBD and non-IBD patients. Non-IBD patients had milder severity of infection with less use of antibiotics. Aeromonas infection seems to greatly contribute to IBD manifestation.

Resumo Introdução: A etiologia da Doença Inflamatória Intestinal (DII) é complexa e resultante da interação de diversos fatores, nomeadamente microbiológicos. A infeção por Aeromonas caracteriza-se por um espectro alargado de manifestações clínicas. O papel da infeção por Aeromonas na DII não está caracterizado. Objetivos: Avaliar o significado da infeção por Aeromonas na DII e as diferenças com a infeção em doentes não-DII. Métodos: avaliação retrospetiva e observacional de todos os doentes com isolamento microbiológico de Aeromonas em amostras fecais num período de 10 anos, num hospital terciário. Resultados: foram avaliados 50 doentes, 56% do sexo masculino, com idade média de 42.1 anos. Doze (24%) com diagnóstico de DII e trinta e oito (76%) não-DII. Os doentes com DII encontravam-se mais frequentemente sob imunossupressão. Dois doentes foram assintomáticos, 44% desenvolveram doença ligeira, 44% moderada e 16.7%severa, havendo maior tendência para infeção severa nos DII. Os doentes não-DII apresentaram mais frequentemente febre e os DII diarreia sanguinolenta e dor abdominal. O uso de antimicrobianos foi superior no grupo DII e a utilização de corticoesteroides foi exclusiva nestes doentes. Isolamento concomitante de outros agentes microbiológicos ocorreu em 4 doentes não-DII e 2 com DII tinham histologia compatível com infeção por Citomega-lovírus. Da população DII, 4 eram Colite Ulcerosa e 9 Doença de Crohn com envolvimento cólico. Destes, 5 receberam o diagnóstico após a infeção por Aeromonas. Conclusão: A apresentação clínica da infeção por Aeromonas foi distinta entre as populações DII e não-DII, sendo que os doentes DII apresentaram doença mais severa e maior utilização de antimicrobianos. A infeção na DII ocorreu essencialmente em doentes com envolvimento cólico.

In this work, iron ore tailing (IOT) was used for the production of two different materials: sodium silicate and geopolymers. Initially, reactions of IOT with NaOH were carried out by hydrothermal reaction in autoclave at 200 °C (1:1.5 and 1:2.5 SiO2:NaOH molar ratio) and reaction times of 4 and 8 h. X-ray fluorescence by dispersive energy (XRF), X-ray diffraction (XRD), Mössbauer spectroscopy, scanning electron microscopy (SEM-EDS) and titrations showed that IOT:NaOH ratios of 1:2.5 and reaction time 8 h completely dissolved the quartz from the IOT, obtaining a solid fraction consisting mainly of hematite and an aqueous phase of sodium silicate, which showed contents of ca. 23% SiO2 and 19% Na2O. This sodium silicate obtained was then combined with IOT (25 and 50 wt.%) to produce geopolimeric material with excellent physico-chemical properties, fast curing time and very good compressive strength results, which ranged from 41 to 58 MPa and many potential applications.

Resumo Este artigo apresenta os resultados de quatro ações de extensão realizadas no âmbito de um projeto de pesquisa-ação participativa da Universidade Nacional da Patagônia San Juan Bosco que envolveu estudantes de diferentes níveis educacionais em Comodoro Rivadavia durante 2020-2021. Propomos documentar e refletir sobre suas experiências educativas e participativas, os efeitos da pandemia e a projeção para o pós-pandemia. O objetivo central é apresentar uma análise reflexiva das formas como integramos pesquisa e extensão a partir de uma abordagem participativa com alunos de diferentes níveis educacionais. Buscamos destacar o potencial das metodologias participativas em projetos de extensão para favorecer instâncias de reflexão e aprendizagem sobre as diferentes experiências educativas neste contexto. Destacamos também a relevância de compreender a dimensão comunicacional e integrar as práticas de pesquisa e extensão com os alunos.

Resumen Este artículo expone resultados de cuatro acciones de extensión realizadas en el marco de un proyecto de Investigación–Acción Participativa de la Universidad Nacional de la Patagonia San Juan Bosco que involucró a estudiantes de diferentes niveles educativos de Comodoro Rivadavia durante 2020– 2021. Nos propusimos documentar y reflexionar sobre sus experiencias educativas y participativas, los efectos de la pandemia y la proyección hacia la pospandemia. El objetivo central es presentar un análisis reflexivo sobre las formas en que integramos investigación y extensión desde un enfoque participativo con estudiantes de diferentes niveles educativos. Buscamos resaltar el potencial de las metodologías participativas en los proyectos extensionistas para favorecer instancias de reflexión y aprendizaje respecto de las distintas experiencias educativas en este contexto. Destacamos además la relevancia de comprender la dimensión comunicacional e integrar las prácticas de investigación y extensión con estudiantes.

Abstract This article presents the results of four extension actions carried out within the framework of a participatory action–research project of the National University of Patagonia San Juan Bosco that involved students of different educational levels in Comodoro Rivadavia during 2020–2021.We set out to document and reflect on their educational and participatory experiences, the effects of the pandemic and its aftermath. The main objective is to present a reflective analysis of the ways in which we integrate research and extension from a participatory approach with students of different educational levels. We seek to highlight the potential of participatory methodologies in extension projects to favor instances of reflection and learning regarding the different educational experiences in this context. We also highlight the importance of understanding the communicational dimension and integrating research and extension practices with students.

RESUMO: O estudo quantificou os parâmetros cinéticos de captação de K nas gramíneas Paspalum notatum, Paspalum urvillei e Axonopus affinis associados com o crescimento. Dez mililitros (10 ml) de solução nutritiva foram coletados em vasos de dois litros - com cinco repetições em 24 horas - para determinação das concentrações de K nas amostras. A matéria seca da parte aérea e raízes; comprimento, volume, diâmetro e área superficial radicular, bem como parâmetros cinéticos associados à absorção de nutrientes (Vmax, Km, Cmin e I) foram avaliados. A espécie P. notatum apresentou a maior produção de raiz, parte aérea e MS total, bem como o maior volume de raiz. P. urvillei apresentou o maior teor de K, que pode ser associado a raízes mais finas e resultando em maior área superficial. A. affinis registrou o maior valor de Vmax, o que levou a uma maior eficiência de absorção de K.

ABSTRACT: This study quantified K uptake kinetic parameters in grass species Paspalum notatum, Paspalum urvillei and Axonopus affinis to associate them with growth. Ten milliliters (10 ml) of nutrient solution were collected in two-liter pots - with five repetitions in 24 hours - in order to determine K concentrations in the samples. Shoot and root dry matter, root length, volume, diameter and surface area, as well as kinetic parameters associated with nutrient uptake (Vmax, Km, Cmin and I) were determined. Species P. notatum was the one presenting the highest root, shoot and total dry matter production, as well as the highest root volume. P. urvillei recorded the highest K content, which may be associated with thinner roots and greater root surface area. A. affinis recorded the highest Vmax value, which led to greater K uptake efficiency.

Resumo Objetivo: identificar as ocorrências relacionadas à punção venosa periférica e à hipodermóclise entre pacientes internados em um hospital geral e em um hospital exclusivo de assistência a pacientes em cuidados paliativos oncológicos. Método: estudo observacional, descritivo e multicêntrico. A amostra do tipo consecutiva e não probabilística foi constituída por 160 pacientes oncológicos internados sob cuidados paliativos. A variável desfecho correspondeu às ocorrências e complicações relacionadas a cada tipo de punção. Utilizou-se um questionário contendo as variáveis sociodemográficas e clínicas e um roteiro estruturado para acompanhamento e avaliação diária da punção. Foram utilizadas estatísticas descritivas para a análise dos dados. Resultados: as ocorrências relacionadas à punção venosa no hospital geral foram sujidade de sangue na inserção do cateter (17,4 %) e prazo de uso expirado (15,8%), enquanto no serviço específico para atendimento a pacientes sob cuidados paliativos foram prazo de uso expirado (32%) seguido de infiltração (18,9%). Quanto à hipodermóclise, foram duas punções subcutâneas com sinais flogísticos (1,0%) no hospital geral e um hematoma no local de inserção do cateter (0,5%). No serviço específico para atendimento a pacientes sob cuidados paliativos foram três punções subcutâneas com sinais flogísticos (5,7%). Conclusão: as ocorrências relacionadas à punção venosa periférica foram superiores às relacionadas à hipodermóclise.

Abstract Objective: to identify the occurrences related to peripheral venipuncture and hypodermoclysis among patients hospitalized in a general hospital and in an exclusive hospital institution for the care of patients in palliative cancer care. Method: an observational, descriptive and multicenter study. The consecutive and non-probabilistic sample consisted of 160 cancer patients hospitalized in Palliative Care. The outcome variable corresponded to the occurrences and complications related to each type of puncture. A questionnaire containing the sociodemographic and clinical variables and a structured script for monitoring and daily evaluation of the puncture were used. Descriptive statistics were employed for data analysis. Results: the occurrences related to venipuncture at a general hospital were blood soiling at catheter insertion (17.4%) and expired use period (15.8%), while at a specific service for the care of patients under palliative care they were expired use period (32%) followed by infiltration (18.9%). As for hypodermoclysis, there were two subcutaneous punctures with phlogistic signs (1.0%) at the general hospital and a hematoma at the catheter insertion site (0.5%). At the specific service for the care of patients under palliative care there were three subcutaneous punctures with phlogistic signs (5.7%). Conclusion: the number of occurrences related to peripheral venipuncture was higher than those related to hypodermoclysis.

Resumen Objetivo: identificar los eventos relacionados con la venopunción periférica y la hipodermoclisis en pacientes hospitalizados en un hospital general y en un hospital que atiende exclusivamente a pacientes en cuidados paliativos oncológicos. Método: estudio observacional, descriptivo y multicéntrico. La muestra consecutiva y no probabilística estuvo compuesta por 160 pacientes oncológicos hospitalizados que reciben cuidados paliativos. La variable resultado correspondió a los eventos y complicaciones relacionados con cada tipo de punción. Se utilizó un cuestionario con variables sociodemográficas y clínicas y una guía estructurada para el seguimiento diario y la evaluación de la punción. Se utilizó estadística descriptiva para el análisis de datos. Resultados: los eventos relacionados con la venopunción en un hospital general fueron contaminación de sangre en la inserción del catéter (17,4%) y catéter vencido (15,8%), mientras que en un hospital específico que atiende exclusivamente a pacientes en cuidados paliativos oncológicos fueron catéter vencido (32%) seguido de infiltración (18,9%). En cuanto a la hipodermoclisis, hubo dos punciones subcutáneas con signos flogísticos (1,0%) en hospital general y un hematoma en el lugar de inserción del catéter (0,5%). En el hospital en un hospital que atiende exclusivamente a pacientes en cuidados paliativos oncológicos hubo tres punciones subcutáneas con signos flogísticos (5,7%). Conclusión: los eventos relacionados con la venopunción periférica fueron mayores que los relacionados con la hipodermoclisis.

In this work, we describe a sustainable alternative to recover iron from two iron ore tailings (IOT) using hydrogen reduction at relatively low temperatures followed by magnetic separation. X-ray powder diffraction (XRD), inductively coupled plasma (ICP), atomic absorption (AA), Mössbauer, scanning electron microscopy (SEM/EDS), Raman and thermogravimetry (TG) analyses indicated that the Fe oxide present in the IOTs (sandy tailing (ST) and mud tailings (MT)), can be reduced with H2 at 500 ºC to produce α-Fe. Upon magnetic separation the mud tailing produced a 77 wt.% magnetic fraction increasing the Fe content from 19.2 to ca. 56 wt.% of Fe. On the other hand, the sandy tailing resulted in a 15 wt.% magnetic fraction increasing the Fe content from 19.2 to 70 wt.%. These results indicate that up to 86% of iron can be recovered from the IOT wastes already in the metallic form which can be very interesting for the steel industry.

Considering the bicentenary of Brazil’s Independence in 2022 and that this year was proclaimed by UNESCO as the International Year of Basic Sciences for Sustainable Development, in mid-2021 the Brazilian Chemical Society (SBQ) launched the “Post 2022 Chemistry Movement - Sustainability and Sovereignty”, with the main goal of promoting different activities so that chemistry contributes for the sustainable development and sovereignty of Brazil in the coming years. One of these activities is the proposal of an action plan, with goals to be achieved by 2030 and 2050. This article describes in detail how this action plan was formulated by a working group, with the help of an advisory council and interviews of 18 professionals from universities, the government, industries, the third sector and other organizations. Mimicking the Agenda 2030 SDGs, the resulting action plan is based on two broad Chemistry Sustainable Development Goals: promoting sustainability through chemistry in basic education, and promoting sustainability in the industry and university through science, technology, innovation, and education (STI&E) in chemistry. After the definition of three driving technological challenges in STI&E, they shall be successively implemented through different actions starting in 2023 yet.

Resumo No Brasil, as pesquisas com produtos naturais tiveram um forte impulso quando a FAPESP apoiou a criação do Laboratório de Química de Produtos Naturais do Instituto de Química da USP (1966). Em 1999, a FAPESP lançou o Programa de Pesquisa em Caracterização, Conservação, Restauração e Uso Sustentável da Biodiversidade (BIOTA-FAPESP), que intensificou a exploração sustentável da biodiversidade, e que evoluiu para formar a Rede Biota de Bioprospecção e Bioensaios (BIOprospecTA), que integra grupos de todo o país, otimizando o aproveitamento das competências já instaladas para a bioprospecção de microrganismos, plantas, invertebrados, vertebrados e organismos marinhos. Dos 104 projetos relacionados às ciências vegetais, 35 realizaram a bioprospecção da flora brasileira, em diversas áreas como Química, Botânica, Fisiologia e Morfologia Vegetal, (Quimio)taxonomia Vegetal, Ecologia de Ecossistemas, Genética Vegetal, Recursos Florestais, Engenharia Florestal, dentre outros, levando a milhares de publicações, ao engajamento de centenas de estudantes e ao entendimento mais profundo dos produtos naturais em diferentes modelos biológicos por meio da análise de micromoléculas auxiliada por estratégias computacionais e espectrométricas, além de avaliações farmacológicas. O desenvolvimento de abordagens ômicas ampliou a visão sobre perfil químico dos organismos, possibilitou o estudo integrado e concomitante de várias amostras, e a anotação mais rápida de moléculas conhecidas, por meio do uso de técnicas hifenadas, quimiométricas e redes moleculares. Isso também contribuiu para superar a falta de informação sobre a segurança e eficácia dos fitopreparados, em projetos que tratam da padronização de produtos fitoterápicos, de acordo com normas internacionais. O programa BIOTA-FAPESP também tem focado em aspectos ambientais, de acordo com os princípios da Química Verde e teve reflexos positivos na colaboração internacional, no número e no impacto das publicações científicas e na parceria com empresas, etapa crucial para agregar valor e expandir a cadeia produtiva de bioprodutos. Ainda, a compilação, sistematização e compartilhamento de dados foram contemplados com a criação da base de dados NUBBEDB, de livre acesso, e que se integra com bases internacionais (ACD/labs, American Chemical Society – ACS), auxiliando pesquisadores e empresas no desenvolvimento de diferentes áreas da ciência, tecnologia, fortalecendo a bioeconomia e subsidiando políticas públicas.

Abstract In Brazil, research with natural products had a strong impulse when FAPESP supported the creation of the Laboratory of Chemistry of Natural Products of the Institute of Chemistry of USP (1966). In 1999, FAPESP launched the Research Program in the Characterization, Conservation, Restoration and Sustainable Use of Biodiversity (BIOTA-FAPESP), which intensified the sustainable exploitation of biodiversity, and which evolved to form the Biota Network for Bioprospection and Bioassays (BIOprospecTA), which integrates groups from all over the country, optimizing the use of the skills already installed for the bioprospecting of microorganisms, plants, invertebrates, vertebrates and marine organisms. Of the 104 projects related to plant sciences, 35 carried out bioprospection of Brazilian flora, belonging to the areas of Chemistry, Botany, Genetics, Plant Physiology, Plant Morphology, Plant (Chemo)taxonomy, Ecosystem Ecology, Plant Genetics. Physical Sciences, Forest Resources, Forestry Engineering, Agronomy, leading to thousands of publications, engagement of hundreds of students and a deeper understanding of natural products in different biological models through macromolecules analysis aided by computational and spectrometric strategies, in addition to pharmacological evaluations. The development of omics approaches led to a more comprehensive view of the chemical profile of an organism, and enabled integrated and concomitant studies of several samples, and faster annotation of known molecules, through the use of hyphenated and chemometric techniques, and molecular networking. This also helped to overcome the lack of information on the safety and efficacy of herbal preparations, in projects dealing with the standardization of herbal products, according to international standards. The BIOTA-FAPESP program has also focused on environmental aspects, in accordance with the principles of Green Chemistry and has had positive effects on international collaboration, on the number and impact of scientific publications and on partnership with companies, a crucial step to add value and expand the production chain of bioproducts. Also, the compilation, systematization and sharing of data were contemplated with the creation of the NUBBEDB database, of free access, and that integrates with international databases (ACD/labs, American Chemical Society – ACS), helping researchers and companies in the development from different areas of science, technology, strengthening the bioeconomy and subsidizing public policies.

Currently, approximately 1.4 billion tons per year of iron ore tailing wastes (IOT) are generated, mainly in Australia, Brazil, and China. This work describes the characterization and application of two typical IOT, i.e., fine and coarse wastes. The physicochemical characterization of these IOT by different techniques such as XRF (X-ray fluorescence), XRD (X-ray diffraction), Mössbauer spectroscopy, and granulometry, indicates for the fine tailing a composition of Fe2O3/FeOOH (10-55%), SiO2 (18-65%) and Al2O3 (up to 15%) with particles of 6-40 µm, whereas the coarse tailing presents 40-150 µm particles with the composition of 8-48% Fe2O3/FeOOH, 30-90% SiO2 and Al2O3 (up to 20%). The main IOT applications discussed in this review are related to civil construction (aggregates for concrete, mortar, Portland cement additives), ceramic industry, geopolymer, synthesis of new materials such as zeolites, mesoporous silica, carbon nanotubes, adsorbents, catalysts for different reactions, in batteries and in fuel cells. It was also carried out an analysis of patents related to IOT applications and the main technological and market barriers that hinder the industrial and commercial uses of these wastes.

RESUMO Objetivo: Avaliar a capacidade do Teste de Caminhada de 6 Minutos para predizer a melhora do estado funcional físico em longo prazo de pacientes sobreviventes à unidade de terapia intensiva. Métodos: Foram avaliados, de forma prospectiva, entre fevereiro de 2017 e agosto de 2018, em um ambulatório pós-unidade de terapia intensiva, 32 sobreviventes à unidade de terapia intensiva. Foram inscritos consecutivamente os pacientes com permanência na unidade de terapia intensiva acima de 72 horas (para admissões emergenciais) ou acima de 120 horas (para admissões eletivas) que compareceram ao ambulatório pós-unidade de terapia intensiva 4 meses após receberem alta da unidade de terapia intensiva. A associação entre a distância percorrida no Teste de Caminhada de 6 Minutos realizado na avaliação inicial e a evolução do estado funcional físico foi avaliada durante 8 meses, com utilização do Índice de Barthel. Resultados: A distância média percorrida no Teste de Caminhada de 6 Minutos foi significantemente mais baixa nos sobreviventes à unidade de terapia intensiva do que na população geral (405m versus 557m; p < 0,001). A idade (β = -4,0; p < 0,001) e a fraqueza muscular (β = -99,7; p = 0,02) se associaram com a distância percorrida no Teste de Caminhada de 6 Minutos. A distância percorrida no Teste de Caminhada de 6 Minutos se associou com melhora do estado funcional físico no período de 8 meses de acompanhamento desses pacientes (razão de chance para cada 10m: 1,07; IC95% 1,01 - 1,16; p = 0,03). A área sob a curva Característica de Operação do Receptor para predição da melhora funcional física pelo Teste de Caminhada de 6 Minutos foi de 0,72 (IC95% 0,53 - 0,88). Conclusão: O Teste de Caminhada de 6 Minutos, realizado 4 meses após a alta da unidade de terapia intensiva, predisse com precisão moderada a melhora do estado funcional físico de sobreviventes à unidade de terapia intensiva.

ABSTRACT Objective: To evaluate the ability of the 6-Minute Walk Test to predict long-term physical functional status improvement among intensive care unit survivors. Methods: Thirty-two intensive care unit survivors were prospectively evaluated from February 2017 to August 2018 in a post-intensive care unit outpatient clinic in Brazil. Individuals with intensive care unit stays > 72 hours (emergency admissions) or > 120 hours (elective admissions) attending the post-intensive care unit clinic four months after intensive care unit discharge were consecutively enrolled. The association between the 6-Minute Walk Test distance at baseline and physical functional status was assessed over 8 months using the Barthel Index. Results: The mean 6-Minute Walk Test distance was significantly lower in intensive care unit survivors than in the general population (405m versus 557m; p < 0.001). Age (β = -4.0; p < 0.001) and muscle weakness (β = -99.7; p = 0.02) were associated with the 6-Minute Walk Test distance. A 6-Minute Walk Test distance was associated with improvement in physical functional status over the 8-month follow-up (odds ratio for each 10m of 1.07; 95%CI 1.01 - 1.16; p = 0.03). The area under the Receiver Operating Characteristic curve for the 6-Minute Walk Test prediction of physical functional status improvement was 0.72 (95%CI 0.53 - 0.88). Conclusion: The 6-Minute Walk Test performed 4 months after intensive care unit discharge predicted long-term physical functional status among intensive care unit survivors with moderate accuracy.

Resumo: Introdução: Os cuidados centrados no doente são uma intenção de quase todas as declarações de missão de um hospital. No entanto, nem sempre são colocados em prática. A doença por COVID-19 trouxe novos desafios, mesmo para os hospitais mais bem organizados e com sistemas de saúde desenvolvidos. Métodos: Na atual pandemia COVID-19, os doentes com doença intestinal inflamatória (DII), têm um risco potencialmente maior de complicações desta doença infeciosa, quer pelo uso de tratamentos imunossupressores e/ou biológicos, quer pela natureza crónica da doença, que evolui com agudizações, que frequentemente requerem cuidados urgentes e, às vezes, hospitalização. Os doentes sob biológicos intravenosos (IV) necessitam de recorrer ao hospital para infusão programada. Discussão: Em hospitais como o nosso, onde doentes com COVID-19 são recebidos e tratados, a organização de “circuitos limpos” é essencial para minimizar os riscos de infeção em doentes não-COVID-19, nomeadamente nas unidades de infusão de biológicos. No nosso Hospital, a unidade de infusão localiza-se dentro do Departamento de Gastrenterologia, o que em circunstâncias normais é muito vantajoso para os doentes, mas não o foi no contexto da atual pandemia. O nosso objetivo, num momento de profunda mudança da atividade da Gastrenterologia e num Departamento com aumento diário do número de doentes COVID-19, foi o de maximizar a adesão aos tratamentos biológicos IV e a sua segurança clínica. Conclusão: Para isso iniciamos um programa proactivo de teste a COVID-19, nos doentes com DII, sob biológicos IV, e mudamos a unidade de infusão para uma Instituição “COVID free,” mantendo a habitual orientação destes doentes pela equipe diferenciada de DII do nosso Serviço. O objetivo deste trabalho é mostrar que, com uma estratégia de cuidados centrados no doente, foi possível atingir níveis elevados de adesão à terapêutica, de conforto e satisfação.

Abstract: Introduction: Although patient-centered care can be found in the mission statement of nearly every hospital, it is not always put into practice, and COVID-19 brings new challenges even to the best-organized hospitals and well-developed health care systems. Methods: In the current COVID-19 pandemic, inflammatory bowel disease (IBD) patients have a potentially higher risk of complications from this infectious disease due to the use of immunosuppressant and/or biologic treatments and due to flares of this chronic illness, which often require urgent care and sometimes hospitalization. Moreover, patients undergoing biologic intravenous (IV) treatment visit the hospital for scheduled IV infusions. Discussion: In hospitals like ours, where COVID-19 patients are treated, the organization of “clean circuits” is essential to minimize the risks of infection for non-COVID-19 patients, such as patients in IBD infusion units. In our hospital, the IBD infusion unit is located within the gastroenterology department, which, under normal circumstances, is very advantageous for patients but in the current context is not. Our goal was to maximize adherence to biologic IV treatment and clinical safety at a time of profound changes in gastroenterology activity and in a department with daily increases in the number of COVID-19 patients. Conclusion: To this end, we initiated proactive COVID-19 testing in IBD patients undergoing biologic IV treatment and changed the location of the infusion unit to a “COVID-free” institution, maintaining the care of these patients by the dedicated IBD team of our department. The purpose of this report is to show that a patient-centered care strategy allowed us to reach very high levels of patient comfort, satisfaction, and compliance with therapeutics.

ABSTRACT INTRODUCTION : A participatory National Public Health Research Agenda (ANISP) with thematic prioritization is a strategic element to generóte evidence-based recommendations and public policies that have a positive impact on the health of populations and enable to achieve health objectives. The Directorate of Health Research (DIS) ofthe Argentine Ministry of Health (MSAL), through the Ministerial NetWork of Health Research (REMINSA), along with adors from the provincial and national government levels belonging to public, privóte, health, academic and research sectors participated in the update of the ANISP. They adapted the original tooI proposed by the Pan American Health Organizatlon and used in the process in 2019. The update included different stages. The selection ofthe topics had the legitimacy, recognition and participation ofthe actors involved, related to health, to government and privóte management and to scientific research; the work was conducted in a federal and transversal manner by consensus with the provincial networks and a Central Advisory Committee in the MSAL. Based on the preliminary guidelines obtained, a semi-structured online survey was developed and distributed to all federal actors, receiving 431 responses. The process resulted in 55 priorilized guidelines, divided into 6 thematic oreas and 33 sub-themes, selected by voting according to importance, impact and feasibility.

RESUMEN INTRODUCCIÓN : Una Agenda Nacional de Investigación en Salud Pública (ANISP) participativa y con priorización temática constituye un elemento estratégico para generar recomendaciones y políticas públicas basadas en evidencia, que imparten positivamente en la salud de las poblaciones y permitan lograr los objetivos sanitarios. En la actualización de la ANISP participaron la Dirección de Investigación en Salud (DIS) del Ministerio de Salud de la Nación (MSAL), a través de la Red Ministerial de Investigación en Salud (REMINSA), y actores de los niveles gubernamentales provinciales y nacionales pertenecientes a los sectores público, privado, de la salud, académico y de investigación. Se adaptó la herramienta original propuesta por la Organización Panamericana de la Salud, utilizada en el proceso en 2019. La actualización abarcó diferentes etapas. La selección de los temas contó con la legitimidad, reconocimiento y participación de los actores vinculados a la salud, a la gestión gubernamental y privada y a la investigación científica; se trabajó de manera federal y transversal, por consenso con las redes provinciales y un Comité Central Asesor en el MSAL. A partir de los lineamientos preliminares obtenidos, se elaboró una encuesta en línea semiestructurada, que fue distribuida a todos los actores federales y recibió 431 respuestas. El proceso resultó en 55 lineamientos priorizados, divididos en 6 áreas temáticas y 33 subtemas, seleccionados por votación según importancia, impacto y factibilidad.

ABSTRACT Appendiceal tumors comprise a variety of histologic types, including appendiceal mucinous neoplasms, which can be grouped as premalignant lesions, tumors of uncertain malignant potential, and malignant lesions. The appendiceal mucinous neoplasms are characterized by mucinous epithelial proliferation with extracellular mucin and pushing tumor margins, commonly an incidental finding during operative exploration. We report the case of a low-grade appendiceal mucinous neoplasm presenting as a subepithelial lesion in Crohn´s Disease patient. The diagnosis was not straightforward, and only surgical resection allowed an accurate diagnosis. Although Inflammatory Bowel Disease is a risk factor for the development of colorectal neoplasms, the absolute risk for appendiceal tumors is uncertain. The frequency of progression to malignancy remains to be determined.

RESUMO A urticária multiforme é uma reação de hipersensibilidade aguda, sendo um subtipo morfológico de urticária. É mais comum em crianças e, uma vez que sua apresentação clínica se assemelha a outras dermatoses, como o eritema multiforme, a vasculite urticariana e a doença do soro-like, tem sido subdiagnosticada. Relatamos assim, um caso de uma criança de 1 ano e 9 meses com lesões cutâneas exuberantes associado a manifestações sistêmicas, com recuperação completa do quadro após tratamento com anti-histamínicos e corticosteróides endovenoso em unidade de medicina intensiva.

ABSTRACT Urticaria multiforme is an acute hypersensitivity reaction which can be considered a morphological subtype of urticaria. It is more common in children and as its clinical presentation resembles other pathologies such as erythema multiforme, urticarial vasculitis and serum-sickness-like reaction, it has been often underdiagnosed. We report a case of a 1-year and 9-month-old child with exuberant skin lesions associated with systemic manifestations with complete and quick recovery after H1-antihistamines and iv steroids in an intensive care unit.
