RESUMO OBJETIVO Estimar a soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares de quatro a 14 anos de idade residentes no município de São Paulo, segundo variáveis clínicas, demográficas, epidemiológicas e sociais, durante o período de fechamento das escolas como medida de controle da covid-19. MÉTODOS Realizou-se um inquérito sorológico em setembro de 2020 com amostra aleatória estratificada por tipo de rede de ensino (pública municipal, pública estadual e privada). Foi coletada amostra de sangue venoso utilizando-se o teste de imunoensaio de fluxo lateral da fabricante Wondfo para detecção de anticorpos totais contra o vírus SARS-CoV-2. Aplicaram-se questionários semiestruturados para o levantamento de dados clínicos, demográficos, sociais e epidemiológicos. RESULTADOS A soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares foi de 16,6% (IC95% 15,4–17,8). O estudo encontrou soroprevalências mais elevadas na rede pública municipal (18,5%; IC95% 16,6–20,6) e estadual (16,2%; IC95% 14,4–18,2) em relação à rede privada (11,7; IC95% 10,0–13,7) e entre escolares da raça/cor preta e parda (18,4%; IC95% 16,8–20,2) e no estrato social mais vulnerável (18,5%; IC95% 16,9–20,2). A pesquisa identificou menores soroprevalências nos escolares que relataram seguir as medidas recomendadas de proteção contra a covid-19. CONCLUSÃO A soroprevalência de anticorpos contra o vírus SARS-CoV-2 atinge principalmente os escolares socialmente mais vulneráveis. Este estudo pode contribuir para embasar políticas públicas que reforcem a importância da suspensão das aulas presenciais e da necessidade de estratégias de medidas de proteção e acompanhamento do status sorológico daqueles que ainda não foram contemplados no calendário vacinal.

Combined liver-kidney transplant in polycystic diseases: a case report

ABSTRACT Polycystic liver disease, a hereditary pathology, usually manifests as autosomal dominant polycystic kidney disease. The many cysts in the liver cause massive hepatomegaly, majorly affecting the patient’s quality of life. In cases of refractory symptoms, liver transplantation is the only treatment choice. A 43-year-old woman was followed up as a hepatology outpatient in August 2020, with a progressive increase in abdominal volume, lower limb edema, and cachexia. The patient was diagnosed with polycystic renal and liver disease with massive hepatomegaly in March 2021, a combined kidney-liver transplant. Liver size represented 13% of the patient’s corporal composition, weighing 8.6kg. The patient was discharged on the 7th postoperative day with no complications. Only 10-20% of patients with polycystic liver disease have clinical manifestations, most of which result from hepatomegaly. An increase in liver volume deteriorates liver function until the condition becomes end-stage liver disease, as kidney function is already compromised; liver-kidney transplantation remains the only treatment choice. The case described drew significant attention to the massive hepatomegaly presented in the patient, with the liver representing over 10% of the patient’s body weight, approximately five to six times larger than a normal-sized liver.

ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

Do alternative scaffolds used in regenerative endodontics promote better root development than that achieved with blood clots?

Resumo O objetivo desta revisão integrativa foi identificar se os scaffolds alternativos utilizados em endodontia regenerativa contribuem para um melhor desenvolvimento radicular, em relação ao aumento do comprimento e espessura das paredes da dentina, em comparação com os scaffolds de coágulo sanguíneo (BC). A pesquisa bibliográfica foi realizada nas bases de dados PubMed, SciELO e Lilacs, utilizando descritores relacionados ao tema. Após a aplicação dos critérios de elegibilidade, 11 artigos foram selecionados e analisados de acordo com o objetivo proposto. Cinco estudos clínicos e seis in vivo, realizados em animais, compararam diferentes tipos de scaffolds alternativos com BCs, com ênfase no plasma rico em plaquetas (PRP) e fibrina rica em plaquetas (PRF). Todos os scaffolds, alternativos ou BC, promoveram um aumento no comprimento da raiz e na espessura da parede dentinária, com percentuais variáveis de aumento entre os estudos. Em geral, houve um aumento significativo do comprimento da raiz e da espessura da dentina promovido pelos scaffolds PRF e PRP, em comparação com a BC. Concluiu-se que a maioria dos scaffolds testados contribuiu para o aumento do comprimento das raízes e da espessura das paredes dentinárias, com ênfase em PRF e PRP.
Abstract The aim of this integrative review was to identify whether alternative scaffolds used in regenerative endodontics contribute to better root development, in relation to the increase in root length and thickness of dentin walls, compared with blood clot (BC) scaffolds. The literature search was conducted in PubMed, SciELO and Lilacs databases, using descriptors related to the topic. After applying the eligibility criteria, 11 articles were selected and analyzed according to the proposed aim. Five clinical and six in vivo studies, conducted in animals, compared different types of alternative scaffolds with BCs, with emphasis on platelet-rich plasma (PRP) and platelet-rich fibrin (PRF). All scaffolds, alternative or BC, promoted an increase in root length and dentin wall thickness, with varying percentages of increase between studies. In general, there was a significant increase in root length and dentin thickness promoted by PRF and PRP scaffolds, compared with BC. It was concluded that the majority of the scaffolds tested contributed to the increase in root length and thickness of dentin walls, with emphasis on PRF and PRP.

ABSTRACT OBJECTIVE To estimate the evolution of the prevalence of SARS-CoV-2 virus infection among residents aged 18 years or over in the municipality of São Paulo. METHODS This is a population-based household survey conducted every 15 days, between June and September 2020, and January and February 2021. In total, the study comprised 11 phases. The presence of antibodies against SARS-CoV-2 was identified in venous blood using a lateral flow test, Wondfo Biotech. In the last phase, the researchers combined it with an immunoenzymatic test, Euroimmun. The participants also answered a semi-structured questionnaire on sociodemographic and economic factors, and on social distancing measures. Prevalence estimates and the 95% confidence interval were estimated according to regions, Human Development Index, sex, age group, ethnicity, education, income, and variables associated with risk or prevention of infection. To compare the frequencies among the categories of each variable, the chi-square test with Rao-Scott correction was used, considering a significance level of 5%. RESULTS In total, 23,397 individuals were interviewed and had their samples collected. The estimated prevalence of antibodies against SARS-CoV-2 ranged from 9.7% (95%CI: 7.9–11.8%) to 25.0% (95%CI: 21.7–28.7). The prevalence of individuals with antibodies against the virus was higher among black and brown people, people with lower schooling and income, and among residents of regions with lower Human Development Index. The lowest prevalences were associated with recommended measures of disease protection. The proportion of asymptomatic infection was 45.1%. CONCLUSION The estimated prevalence of the infection was lower than the cumulative incidence variation, except for the last phase of the study. The differences in prevalence estimates observed among subpopulations showed social inequality as a risk of infection. The lower prevalence observed among those who could follow prevention measures reinforce the need to maintain social distancing measures as a way to prevent SARS-CoV-2 infection.

OBJECTIVES: To compare demographic/clinical/laboratory/treatments and outcomes among children and adolescents with laboratory-confirmed coronavirus disease 2019 (COVID-19). METHODS: This was a cross-sectional study that included patients diagnosed with pediatric COVID-19 (aged <18 years) between April 11, 2020 and April 22, 2021. During this period, 102/5,951 (1.7%) of all admissions occurred in neonates, children, and adolescents. Furthermore, 3,962 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection samples were processed in patients aged <18 years, and laboratory-confirmed COVID-19 occurred in 155 (4%) inpatients and outpatients. Six/155 pediatric patients were excluded from the study. Therefore, the final group included 149 children and adolescents (n=97 inpatients and 52 outpatients) with positive SARS-CoV-2 results. RESULTS: The frequencies of sore throat, anosmia, dysgeusia, headache, myalgia, nausea, lymphopenia, pre-existing chronic conditions, immunosuppressive conditions, and autoimmune diseases were significantly reduced in children and adolescents (p<0.05). Likewise, the frequencies of enoxaparin use (p=0.037), current immunosuppressant use (p=0.008), vasoactive agents (p=0.045), arterial hypotension (p<0.001), and shock (p=0.024) were significantly lower in children than in adolescents. Logistic regression analysis showed that adolescents with laboratory-confirmed COVID-19 had increased odds ratios (ORs) for sore throat (OR 13.054; 95% confidence interval [CI] 2.750-61.977; p=0.001), nausea (OR 8.875; 95% CI 1.660-47.446; p=0.011), and lymphopenia (OR 3.575; 95% CI 1.355-9.430; p=0.010), but also had less hospitalizations (OR 0.355; 95% CI 0.138-0.916; p=0.032). The additional logistic regression analysis on patients with preexisting chronic conditions (n=108) showed that death as an outcome was significantly associated with pediatric severe acute respiratory syndrome (SARS) (OR 22.300; 95% CI 2.341-212.421; p=0.007) and multisystem inflammatory syndrome in children (MIS-C) (OR 11.261; 95% CI 1.189-106. 581; p=0.035). CONCLUSIONS: Half of the laboratory-confirmed COVID-19 cases occurred in adolescents. Individuals belonging to this age group had an acute systemic involvement of SARS-CoV-2 infection. Pediatric SARS and MIS-C were the most important factors associated with the mortality rate in pediatric chronic conditions with COVID-19.

Reading fluency performance of elementary-school fifth-grade students

RESUMO Objetivo: caracterizar e comparar o desempenho em fluência de leitura de escolares do 5º ano do ensino público e privado. Métodos: participaram deste estudo 44 escolares do 5° ano do ensino fundamental, de ambos os gêneros, com idade entre 10 e 11 anos, divididos em Grupo I (GI) 25 do ensino público e Grupo II (GII) 19 do ensino privado. Os escolares foram submetidos à Avaliação do Desempenho em Fluência de Leitura - ADFLU. A análise do desempenho foi realizada pelo número de palavras corretas por minuto, palavras incorretas por minuto, tempo total de leitura e velocidade de leitura. Para a análise dos dados foi utilizado testes estatísticos com valor de p ≤0,05, para a comparação inter e intragrupo. Resultados: na análise por grupo, houve diferença significativa para o tempo total de leitura entre os textos em GI e para a velocidade de leitura entre os textos no GII. Na comparação entre os grupos, todas as variáveis apresentaram diferença significativa, com melhor desempenho em GII. Conclusão: foi possível caracterizar o desempenho em fluência de leitura dos escolares do 5º ano do ensino público e privado, sendo o desempenho dos escolares da rede privada superior aos escolares da rede pública quando comparados.
ABSTRACT Purpose: to characterize and compare the reading fluency performance of public- and private-school fifth-grade students. Methods: a total of 44 elementary-school fifth-grade students of both sexes, aged 10 to 11 years, participated in the study and were divided into Group I (GI, with 25 public-school students) and Group II (GII, with 19 private school students). They were submitted to the Assessment of Reading Fluency Performance (ADFLU). The performance analysis was based on the number of correct words per minute, incorrect words per minute, total reading time, and reading speed. The data analysis was made with statistical tests, with significance set at p-value ≤0.05, for the inter- and intragroup comparisons. Results: in the analysis per group, there was a significant difference in the total reading time between the texts in GI, and in the reading speed between the texts, in GII. In the comparison between the groups, all variables had a significant difference, with a better performance in GII. Conclusion: the reading fluency performance of public- and private-school fifth-graders was characterized. The performance of the private-school students was superior to that of the public-school students.

Assim, este trabalho objetivou comparar o custo dos nutrientes de alimentos típicos da dieta brasileira, realizando coletas de preços em mercados virtuais e calculando o custo para se atingir 30% das necessidades diárias de um adulto saudável para oito nutrientes. A carne de frango foi a fonte mais barata de proteína. A fibra de cereal teve o menor custo para fibra e ferro. As fontes mais baratas de cálcio foram o Ovomaltine e os lácteos, e o fígado bovino foi o alimento melhor ranqueado para vitamina A. A vitamina C foi o nutriente mais barato na dieta brasileira, e o suco de acerola a fonte mais barata deste nutriente. Os lácteos ocuparam as seis primeiras posições no “ranking” da vitamina D, e nozes e sementes apresentaram o menor custo para vitamina E. Os resultados mostraram que os consumidores brasileiros podem alcançar a ingestão recomendada de nutrientes considerados críticos a um baixo custo por meio da aquisição dos alimentos melhor ranqueados. No entanto, esse ranqueamento deve ser ponderado em função da qualidade do alimento, conforme preconiza o Novo Guia Alimentar para a População Brasileira.

Vitamin C was found to be the cheapest nutrient in the Brazilian diet, with acerola juice being the cheapest source of this nutrient. Dairy products occupied the first six positions in the ranking of vitamin D, while nuts and seeds were the cheapest sources of vitamin E. The results showed that Brazilian consumers can meet the recommended intake of critical nutrients at a low cost through the purchase of the best ranked foods. However, this ranking should be weighted according to the food quality, as recommended in the New Dietary Guidelines for the Brazilian Population.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

Resumo: O zoólogo Dr. José Santiago Lima-Verde (1945-2019) fez uma série de importantes contribuições para o campo da biologia animal no nordeste do Brasil, em particular através de sua pesquisa com serpentes. Enquanto professor da Escola Superior de Agricultura de Mossoró, atualmente Universidade Federal Rural do Semi-Árido, o Prof. Lima-Verde estabeleceu uma coleção herpetológica que permaneceu esquecida por várias décadas. No presente estudo, descrevemos a revitalização dessa coleção, que incluiu a atualização da identificação das espécies e a compilação dos metadados da coleção, que agora é denominada Coleção Herpetológica Lima-Verde. Nós catalogamos 80 espécimes representando 30 espécies, incluindo 23 espécies de serpentes, cinco lagartos e duas anfisbenas. Um terço (30%) dos exemplares possuíam informações sobre datas e locais de coleta, sendo a maioria desses espécimes coletados no Estado do Rio Grande do Norte durante a década de 1970. Também catalogamos 315 ovos de sete espécies de serpentes, provenientes de 22 ninhadas obtidas em cativeiro. A coleção apresenta dados inéditos e constituirá uma referência para futuras pesquisas herpetológicas sobre as espécies encontradas na região oeste do Estado do Rio Grande do Norte. Nossos achados reforçam a importância das coleções zoológicas para a compreensão dos padrões de biodiversidade, e nós recomendamos a criação de mais coleções científicas regionais e o reconhecimento mais amplo de seu valor como fonte básica de dados biológicos.
Abstract: The zoologist Dr. José Santiago Lima-Verde (1945-2019) made a number of major contributions to the field of animal biology in the northeastern Brazil, in particular through his research with snakes. While employed as a professor at Escola Superior de Agricultura de Mossoró, currently the Universidade Federal Rural do Semi-Árido, Prof. Lima-Verde established a herpetological collection which remained forgotten for a number of decades. In the present study, we describe the revitalization of this collection, which included the updating of the identification of the species and the compilation of the metadata on the collection, which is now named the Coleção Herpetológica Lima-Verde. We catalogued 80 specimens representing 30 species, including 23 species of snakes, five lizards, and two amphisbaenians. One third (30%) of the specimens had information on dates and sites, with the majority of these specimens being collected in the Brazilian State of Rio Grande do Norte during the 1970's. We also catalogued 315 eggs of seven snake species, from 22 clutches laid in captivity. The collection presents unpublished data and will constitute a reference for future herpetological research on the species found in western region of State of Rio Grande do Norte. Our findings reinforce the importance of zoological collections for the understanding of patterns of biodiversity, and we would recommend the creation of more regional scientific collections and the broader recognition of their value as a basic source of biological data.
