Abstract: Andrew Feenberg is an essential author in the field of philosophy of technology. His ideas are particularly relevant in revealing the political dimension of technology, be it shaping society or being shaped by society. However, current Feenberg’s reflection fails to consider the internal domain of technical disciplines more rigorously. Indeed, he usually stops his analysis in the border between lifeworld (where the democratizing mobilizations occur and new/different requirements or values arise) and the technical disciplines. To identify and overcome this failure, I articulate some elements from Boaventura de Sousa Santos and Yuk Hui. I also present the example of three decolonial (or empowering) technical interventions, which illustrate: how what Santos and Hui theorize is already being developed by some technical interventions in the Global South, and the impacts of such interventions on the technical teams (and, through that, also on the technical disciplines).

Resumo: Andrew Feenberg é um importante autor da filosofia da tecnologia, cujas ideias são particularmente relevantes para identificar a dimensão política da tecnologia, seja no seu papel de conformar a sociedade, seja em ser conformada por esta. A falha do estágio atual da sua reflexão está em não se voltar, de forma mais rigorosa, para o âmbito interno das disciplinas técnicas, usualmente interrompendo sua análise na fronteira entre o mundo da vida (no qual ocorrem as mobilizações democratizantes e de onde emergem suas pautas ou demandas) e tais disciplinas. Para identificar e superar essa falha, são articulados alguns elementos da reflexão de Boaventura de Sousa Santos e de Yuk Hui. Também são trazidos exemplos de intervenções técnicas decoloniais (ou emancipadoras), os quais ilustram: como aquilo teorizado por Santos e Hui já acontece em práticas técnicas desenvolvidas no Sul global; e os impactos disso nas equipes técnicas que as praticam (e, a partir disso, potencialmente também nas disciplinas a que seus membros estão vinculados).

Abstract Disarticulated shells of three bivalve mollusk species (Anomalocardia brasiliana, Codakia orbicularis, and Divaricella quadrisulcata) were experimentally tested in laboratory flumes to determine the threshold of motion and final orientation of the valves. A total of 150 current flow experiments were conducted on single shells resting on a fixed sand bed. This study demonstrated that shells in the convex-up position are more resistant to flow when the umbo is pointing downstream rather than upstream. Moreover, species with higher frontal areas were more likely to be entrained at lower flow velocities. Results of dimensionless shear stress exhibited values far below the threshold of grains movement for beds of uniform roughness (Shields curve). It was observed that circular shells in convex-up positions were mostly orientated with the umbo pointing downstream. Conversely, elliptical shells in convex-up position tended to align their longer axis parallel to the flow with the posterior side of the valve pointing downstream. These results are not only directly applicable in interpretations of incipient shell motions and in paleocurrent analyses from field and sample data, but also support construction of accurate geological models.

Abstract The Brazilian Practice Guidelines for Stroke Rehabilitation – Part II, developed by the Scientific Department of Neurological Rehabilitation of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, in Portuguese), focuses on specific rehabilitation techniques to aid recovery from impairment and disability after stroke. As in Part I, Part II is also based on recently available evidence from randomized controlled trials, systematic reviews, meta-analyses, and other guidelines. Part II covers disorders of communication, dysphagia, postural control and balance, ataxias, spasticity, upper limb rehabilitation, gait, cognition, unilateral spatial neglect, sensory impairments, home rehabilitation, medication adherence, palliative care, cerebrovascular events related to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, the future of stroke rehabilitation, and stroke websites to support patients and caregivers. Our goal is to provide health professionals with more recent knowledge and recommendations for better rehabilitation care after stroke.

Resumo As Diretrizes Brasileiras de Reabilitação do Acidente Vascular Cerebral (AVC) - Parte II, desenvolvida pelo Departamento Científico de Reabilitação Neurológica da Academia Brasileira de Neurologia é voltada para intervenções específicas de técnicas de reabilitação de déficits neurológicos e incapacidades. Seguindo o mesmo modelo da Parte I, a Parte II também se baseia em estudos randomizados, revisões sistemáticas, metanálises e outras diretrizes sobre o mesmo tema. A segunda parte aborda os distúrbios da comunicação, disfagia, controle postural e equilíbrio, ataxias, espasticidade, reabilitação do membro superior, marcha, cognição, negligência espacial unilateral, déficits sensoriais, reabilitação domiciliar, aderênciaao usode medicamentos, cuidados paliativos,ofuturodareabilitação no AVC, e websites de orientação sobre AVC para pacientes e cuidadores. Nosso objetivo é fornecer aos profissionais envolvidos na reabilitação conhecimento atualizado e recomendações para um melhor cuidado no pós-AVC.

ABSTRACT Overactive bladder is a symptom complex consisting of bothersome storage urinary symptoms that is highly prevalent among both sexes and has a significant impact on quality of life. Various antimuscarinic agents and the beta-3 agonists mirabegron and vibegron are currently available for the treatment of OAB. Each drug has specific pharmacologic properties, dosing schedule and tolerability profile, making it essential to individualize the medical treatment for the patient's characteristics and expectations. In this manuscript, we review the most important factors involved in the contemporary pharmacological treatment of OAB.

RESUMO Objetivo: Fornecer recomendações para nortear o manejo clínico do potencial doador em morte encefálica. Métodos: O presente documento foi formulado em dois painéis compostos por uma força tarefa integrada por 27 especialistas de diferentes áreas que responderam a questões dirigidas aos seguintes temas: ventilação mecânica, hemodinâmica, suporte endócrino-metabólico, infecção, temperatura corporal, transfusão sanguínea, e uso de checklists. Os desfechos considerados foram: parada cardíaca, número de órgãos retirados ou transplantados e função/sobrevida dos órgãos transplantados. A qualidade das evidências das recomendações foi avaliada pelo sistema Grading of Recommendations Assessment, Development, and Evaluation. Resultados: Foram geradas 19 recomendações a partir do painel de especialistas. Dessas, 7 foram classificadas como fortes, 11 fracas e uma foi considerada boa prática clínica. Conclusão: Apesar da concordância entre os membros do painel em relação à maior parte das recomendações, o grau de recomendação é fraco em sua maioria.

Abstract Objective: To contribute to updating the recommendations for brain-dead potential organ donor management. Methods: A group of 27 experts, including intensivists, transplant coordinators, transplant surgeons, and epidemiologists, answered questions related to the following topics were divided into mechanical ventilation, hemodynamics, endocrine-metabolic management, infection, body temperature, blood transfusion, and checklists use. The outcomes considered were cardiac arrests, number of organs removed or transplanted as well as function / survival of transplanted organs. The quality of evidence of the recommendations was assessed using the Grading of Recommendations Assessment, Development, and Evaluation system to classify the recommendations. Results: A total of 19 recommendations were drawn from the expert panel. Of these, 7 were classified as strong, 11 as weak and 1 was considered a good clinical practice. Conclusion: Despite the agreement among panel members on most recommendations, the grade of recommendation was mostly weak.

Abstract Spondyloarthritis (SpA) is a group of chronic inflammatory systemic diseases characterized by axial and/or peripheral joints inflammation, as well as extra-articular manifestations. Over some decades, nonsteroidal anti-inflammatory drugs (NSAIDs) have been the basis for the pharmacological treatment of patients with axial spondyloarthritis (axSpA). However, the emergence of the immunobiologic agents brought up the discussion about the role of NSAIDs in the management of these patients. The objective of this guideline is to provide recommendations for the use of NSAIDs for the treatment of axSpA. A panel of experts from the Brazilian Society of Rheumatology conducted a systematic review and meta-analysis of randomized clinical trials for 15 predefined questions. The Grading of Recommendations, Assessment, Development and Evaluation methodology to assess the quality of evidence and formulate recommendations were used, and at least 70% agreement of the voting panel was needed. Fourteen recommendations for the use of NSAIDs in the treatment of patients with axSpA were elaborated. The purpose of these recommendations is to support clinicians’ decision making, without taking out his/her autonomy when prescribing for an individual patient.

RESUMO: Padrões empíricos de desequilíbrio de ligação (LD) podem ser utilizados para aumentar o poder estatístico do mapeamento genético. Este trabalho foi realizado com o objetivo de verificar a eficácia da análise de fatores (AF) aplicada a conjuntos de dados de marcadores moleculares do tipo SNP, visando identificar grupos de ligação e blocos de haplótipos. O conjunto de dados SNPs utilizado foi oriundo de um processo de simulação de uma população F2, contendo 2000 marcas com informações de 500 indivíduos. A estimação das cargas fatoriais (loadings) da AF foi feita de duas formas, considerando a matriz de distâncias entre os marcadores (A) e considerando a matriz de correlação (R). O número de fatores (k) a ser utilizado foi estabelecido com base no gráfico scree-plot e com base na proporção da variância total explicada. Os resultados indicam que as matrizes A e R conduzem a resultados similares. Com base no scree-plot considerou-se k igual a 10 e os fatores interpretados como sendo representativos dos grupos de ligação. O segundo critério conduziu a um número de fatores igual a 50, e os fatores interpretados como sendo representativos dos blocos de haplótipos. Isto mostra o potencial da técnica que permite obter resultados aplicáveis a qualquer tipo de população, corroborando a interpretação de estudos genômicos. O trabalho demonstrou que a AF foi capaz de identificar padrões de associação entre marcadores, identificando subgrupos de marcadores que refletem grupos de ligação fatorial e também grupos de desequilíbrio de ligação.

ABSTRACT: Empirical patterns of linkage disequilibrium (LD) can be used to increase the statistical power of genetic mapping. This study was carried out with the objective of verifying the efficacy of factor analysis (AF) applied to data sets of molecular markers of the SNP type, in order to identify linkage groups and haplotypes blocks. The SNPs data set used was derived from a simulation process of an F2 population, containing 2000 marks with information of 500 individuals. The estimation of the factorial loadings of FA was made in two ways, considering the matrix of distances between the markers (A) and considering the correlation matrix (R). The number of factors (k) to be used was established based on the graph scree-plot and based on the proportion of the total variance explained. Results indicated that matrices A and R lead to similar results. Based on the scree-plot we considered k equal to 10 and the factors interpreted as being representative of the bonding groups. The second criterion led to a number of factors equal to 50, and the factors interpreted as being representative of the haplotypes blocks. This showed the potential of the technique, making it possible to obtain results applicable to any type of population, helping or corroborating the interpretation of genomic studies. The study demonstrated that AF was able to identify patterns of association between markers, identifying subgroups of markers that reflect factor binding groups and also linkage disequilibrium groups.

ABSTRACT Introduction Acute liver failure (ALF) due to diffuse infiltrating solid malignancy without any focal lesions on radiographic imaging is rare. Case report A 70-year-old man was admitted due to mental confusion, abdominal pain, and ALF. Three years before, he had undergone a left nephrectomy for urothelial carcinoma followed by adjuvant chemotherapy. The abdominal computed tomography (CT) showed hepatomegaly and ascites. Ascitic fluid had transudate characteristics, with no malignant cells. Percutaneous liver biopsy (LB) showed diffuse liver infiltration of metastatic urothelial carcinoma. The patient rapidly deteriorated and died in a week due to ALF. Discussion History of solid cancer and hepatomegaly and/or liver failure without other obvious explanation should encourage to perform LB. Conclusion LB is warranted to avoid misdiagnosis, prolonged hospital stays, and delay in palliative care.

Abstract Psoriatic arthritis (PsA) is a chronic and systemic immune disease characterized by inflammation of peripheral and/ or axial joints and entheses in patients with psoriasis (PsO). Extra-articular and extracutaneous manifestations and numerous comorbidities can also be present. These recommendations replace the previous version published in May 2013. A systematic review of the literature retrieved 191 articles that were used to formulate 12 recommendations in response to 12 clinical questions, divided into 4 sections: diagnosis, non-pharmacological treatment, conventional drug therapy and biologic therapy. These guidelines provide evidence-based information on the clinical management for PsA patients. For each recommendation, the level of evidence (highest available), degree of strength (Oxford) and degree of expert agreement (interrater reliability) are reported.

RESUMO. Desde os anos 1980, reconhecem-se como partes constitutivas da tecnologia e do seu desenvolvimento o conhecimento técnico-científico e os valores instrumentais e cognitivos que balizam tal conhecimento, bem como valores ético-políticos. No entanto, segue grandemente negligenciada ou desconhecida uma quarta categoria de elementos que incidem no projeto: acervos imagéticos, valores estéticos e procedimentos estruturados. A desconsideração desses elementos impõe limites ao desenvolvimento técnico possível. Neste artigo, serão apresentados essa quarta categoria de elementos, o impacto dela sobre a prática projetiva e as soluções sociotécnicas construídas, e uma forma de pluralizar seus conteúdos. Será exposto também um esboço de duas das abordagens mais exitosas para a democratização do desenvolvimento técnico. A partir disso, serão discutidos: alguns limites e potencialidades da democratização da tecnologia; uma atualização de parte da compreensão simondoniana de desenvolvimento técnico; a necessidade teórica e prática de se descolonizar (ou seguir descolonizando) a filosofia da tecnologia.

ABSTRACT: Since the 1980s, technical-scientific knowledge, instrumental and cognitive values as well as ethical-political values are acknowledged as constitutive parts of technology and its development. However, a fourth category of elements that shapes design continues to be largely neglected or unknown: image collections, aesthetical values, and structured procedures. Disregarding such elements impose limits on the technical development. In this manuscript, I present this fourth category elements, its impact on the designing practice, and a way of pluralizing its contents. I also sketch two of the most successful approaches for democratizing the technical development. Then, I discuss: some limits and potentialities of democratizing technology; an actualization of part of Simondon’s understanding of technical development; the theoretical and practical necessity of decolonizing (or continuing the decolonization of) the philosophy of technology.

Abstract Spondyloarthritis is a group of chronic inflammatory systemic diseases characterized by axial and/or peripheral joints inflammation, as well as extra-articular manifestations. The classification axial spondyloarthritis is adopted when the spine and/or the sacroiliac joints are predominantly involved. This version of recommendations replaces the previous guidelines published in May 2013. A systematic literature review was performed, and two hundred thirty-seven studies were selected and used to formulate 29 recommendations answering 15 clinical questions, which were divided into four sections: diagnosis, non-pharmacological therapy, conventional drug therapy and biological therapy. For each recommendation the level of evidence supporting (highest available), the strength grade according to Oxford, and the degree of expert agreement (inter-rater reliability) is informed. These guidelines bring evidence-based information on clinical management of axial SpA patients, including, diagnosis, treatment, and prognosis.

RESUMO: Malformações congênitas são alterações funcionais e estruturais dos sistemas, tecidos ou órgãos que podem ocorrer na fase embrionária ou fetal. Malformações na medula espinhal, como hipoplasia segmentar da medula espinhal (HSME) e siringomielia, possuem raras descrições em bovinos. Uma bezerra Girolando, proveniente do Município de Valença, RJ, foi atendida no Hospital Veterinário da Universidade Federal Rural do Rio de Janeiro (UFRRJ), com quadro de incoordenação motora. À avaliação clínica, na coluna vertebral, à linha dorsal da região torácica havia uma área com depressão de 5 cm. Ao exame neurológico foi observado propriocepção reduzida, extensão de membros pélvicos com aumento da atividade nociceptiva e redução de reflexo anal. Ao exame radiográfico, o corpo vertebral T11 apresentou forma trapezoidal em cunha com hemivértebra ventral, provável agenesia ou hipoplasia dos processos espinhosos T11-T12 e fusão dos processos espinhosos T9-T10. À mielografia indicou compressão medular extradural provocada pelas malformações vertebrais. À necropsia não foram observados os processos espinhosos T11-T12, o canal medular apresentou estenose cranial (T11-T13) e, na medula espinhal torácica (T8-T11) foram observados poros de 1-3 mm na substância branca. À microscopia, os segmentos T9-T11 apresentaram dilatações císticas na substância branca e os segmentos T8-T9, formações de áreas císticas de tamanhos variados; nos segmentos T11-T13 denotou-se moderada redução da substância cinzenta ao redor do canal central da medula. O presente trabalho tem o objetivo de apresentar os achados clínicos e patológicos desta bezerra Girolando com HSME e siringomielia, que devem ser diferenciadas de outras síndromes da medula espinhal. As malformações congênitas possuem importância econômica e sua etiologia e diagnóstico são fundamentais para a condução de programas de controle de doenças e seleção de progenitores.

ABSTRACT: Congenital malformations are functional and structural changes in organ systems, tissues, or organs that may develop during the embryonic or fetal phase. Spinal cord malformations, such as segmental hypoplasia of the spinal cord (SHSC) and syringomyelia, are rare in bovines. A Girolando calf from Valença, Rio de Janeiro, was admitted to the Veterinary Hospital of Universidade Federal Rural do Rio de Janeiro because of motor incoordination. Clinical evaluation revealed a 5-cm depression area in the spine at the dorsal line of the thoracic region. Neurological examination revealed reduced proprioception, pelvic limb extension with increased nociceptive activity, and reduced anal reflex. In radiographic examination, the body of the T11 vertebra had a trapezoidal wedge shape with ventral hemivertebra, probable agenesis or hypoplasia of the T11-T12 spinous processes, and fusion of the T9-T10 spinous processes. Myelography revealed extradural spinal compression caused by vertebral malformations. Necropsy showed no spinous processes (T11-T12), cranial stenosis in the medullary canal (T11-T13), and 1-3-mm pores in the white matter of the thoracic spinal cord (T8-T11). Microscopy revealed cystic dilatations in the white matter (T9-T11), cystic areas of varying sizes (T8-T9), and moderate reduction in the gray matter around the central canal of the medulla (T11-T13). Here, we reported the clinical and pathological findings of SHSC and syringomyelia in a Girolando calf. The features should be differentiated from other spinal cord syndromes. Congenital malformations are of economic importance, and their etiology and diagnosis are fundamental to disease control and progenitor-selection programs.

Resume Dans diverses études à propos de la religion maya, un dieu particulier est mentionné : Hunab Ku. D’après des sources historiques, il s'agit d'une divinité créatrice qui n'avait pas de représentation et qui était incorporel. Bien que certains chercheurs ont mis en question son authenticité, il n’a toujours pas été déterminé s'il s’agissait d’une divinité préhispanique maya ou non. Il est possible que cette incertitude réside dans le fait que les informations sur Hunab Ku ont été utilisées sans considérer le contexte historique dans lequel elles ont été énoncées et documentées. Par conséquent, le but de cet article est d’identifier et d’analyser Hunab Ku dans l’Historia de Yucatán de Diego López Cogolludo, l'une des principales sources rendant compte de son existence et qui permet de vérifier si cette divinité est effectivement d'origine préhispanique ou s’il s’agit d’une «traduction» en maya yucatèque d’un titre du dieu chrétien. Pour connaître la raison pour laquelle López Cogolludo parler de Hunab Ku, il est nécessaire de faire appel à la méthode herméneutique, propre à l'analyse historique, et de comprendre son œuvre à l'intérieur de son propre contexte historiographique, de même que la construction de son discours historique. Ce n’est qu’après la considération de ces aspects, qu’une tentative de compréhension de l’identité de Hunab Ku peut être entreprise.

Abstract In several studies on the Maya religion, a particular god is mentioned: Hunab Ku. He is a creator deity that, according to historical sources, had no representation and was incorporeal. Although some researchers have questioned the authenticity, it is not yet determined whether this divinity was or not a Pre-Hispanic Maya deity. Possibly the problem is that the data on Hunab Ku are used without considering the historical context in which they were enunciated and recorded. Therefore, this article has as main objective to know, identify and analyze Hunab Ku in the History of Yucatan by Diego López Cogolludo, which is one of the main sources that gives an account of his existence, as well as checking if it is a deity of Prehispanic origin or a title of the Christian god “translated” into the Mayan language. To acknowledge why López Cogolludo spoke about Hunab Ku, it is necessary to use the hermeneutic method, advantageous for historical analysis, to understand his work within his own historiographical context, as well as the construction of the historical discourse. After considering these aspects, we can attempt to understand who Hunab ku was.

Resumen En diversos estudios sobre la religión maya se menciona a un dios particular: Hunab Ku. Se trata de una deidad creadora que, de acuerdo con las fuentes históricas, no tenía representación y era incorpóreo. Aunque algunos investigadores han puesto en duda su autenticidad, aún no se determina si fue o no una deidad maya prehispánica. Tal vez el problema radica en que los datos sobre Hunab Ku son utilizados sin considerar el contexto histórico en el que se enunciaron y se registraron. Por lo anterior, este artículo tiene como objetivos conocer, identificar y analizar a Hunab Ku en la Historia de Yucatán de Diego López Cogolludo, que es una de las fuentes principales que da cuenta de su existencia, además de comprobar si es una deidad de origen prehispánico o se trata un título del dios cristiano “traducido” a la lengua maya. Para conocer el porqué López Cogolludo habló sobre Hunab Ku es necesario recurrir al método hermenéutico propio del análisis histórico y comprender su obra dentro de su propio contexto historiográfico, así como la construcción de su discurso histórico, al considerar estos aspectos podremos entender quién fue Hunab ku.

BACKGROUND Field testing required to license the combined measles, mumps, and rubella (MMR) vaccine must take into account the current recommendation of the vaccine in Brazil: first dose at 12 months and second dose at 15 months of age in combination with a varicella vaccine. OBJECTIVES This study aimed to evaluate the clinical consistency, immunogenicity, and reactogenicity of three batches of MMR vaccine prepared with active pharmaceutical ingredients (API) from Bio-Manguinhos, Fiocruz (MMR-Bio), and compare it to a vaccine (MMR produced by GlaxoSmithKline) with different API. METHODS This was a phase III, randomised, double-blind, non-inferiority study of the MMR-Bio administered in infants immunised at health care units in Pará, Brazil, from February 2015 to January 2016. Antibody levels were titrated by immunoenzymatic assays. Adverse events were recorded in diaries. FINDINGS Seropositivity levels after MMR-Bio were 97.6% for measles, 84.7% for mumps, and 98.0% for rubella. After the MMRV vaccine, seroconversion rates and GMT increased substantially for mumps. In contrast, approximately 35% of the children had no detectable antibodies to varicella. Systemic adverse events were more frequent than local events. CONCLUSION The demonstration of batch consistency and non-inferiority of the Bio-MMR vaccine completed the technology transfer. This is a significant technological achievement with implications for immunisation programs.

Resumo Objetivo: Comparar a avaliação efetuada pelas equipes de enfermagem de unidades pediátricas e materno-infantis com as de unidades de pacientes adultos acerca da inclusão das famílias nas políticas e práticas institucionais. Método: Estudo transversal e descritivo, realizado em um hospital universitário com 148 profissionais da equipe de enfermagem das unidades pediátricas e materno-infantis e 43 das unidades de adulto. Os dados foram analisados pelo programa SPSS 22, sendo utilizado o teste Qui-quadrado e teste Exato de Fisher. Resultados: Os grupos apresentaram percepções discordantes acerca da inclusão das famílias nas políticas e práticas institucionais, sobretudo, acerca da diferenciação entre famílias e visitantes nas políticas e práticas; percepções semelhantes relacionaram-se às políticas e práticas não serem favoráveis à presença de irmãos e crianças, e à não inclusão das famílias em aspectos relacionados ao cotidiano assistencial. Conclusão e implicações para a prática: A percepção das equipes de enfermagem indica que as famílias não estão incluídas nas políticas e práticas institucionais. Os resultados de uma autoavaliação organizacional acerca da inclusão das famílias subsidiam a construção de um caminho para melhoria das políticas e práticas organizacionais, possibilitando traçar um plano de ação para implementar o Cuidado Centrado no Paciente e na Família em âmbito institucional.

Abstract Objective: To compare the evaluation carried out by the nursing teams of pediatric and maternal-infant with adult patient wards on the inclusion of families in institutional policies and practices. Method: A cross-sectional and descriptive study carried out in a university hospital with 148 professionals from the nursing team of the maternal and pediatric units and 43 from adult units. The data were analyzed by the program Statistical Package for the Social Sciences 22, using the chi-square test and Fisher's exact test. Results: The groups presented distinct perceptions about the differentiation between families and visitors in policies and practices; similar perceptions related to the policies and practices are not favorable to the presence of siblings and children, and to the non-inclusion of the families in aspects related to daily care. Conclusion and Implications for Practice: The perception of nursing teams indicates that families are not included in institutional policies and practices. The results of an organizational self-assessment about families' inclusion subsidize a way to improve organizational policies and practices, making it possible to draw up an action plan to implementing Patient and Family Centered Care at the institutional level.
