Resumo O viés atencional corresponde à alocação de recursos de atenção a materiais irrelevantes à tarefa. Supõe-se que pacientes com transtorno obsessivo-compulsivo (TOC) apresentem viés atencional voltado à ameaça. Com o objetivo de descrever os achados neurobiológicos do viés atencional voltado à ameaça no TOC, foi realizada uma busca sistemática por estudos experimentais com investigação neurobiológica nas bases de dados: MEDLINE, Web of Science, Scopus e LILACS. Quatro estudos com grupo controle são descritos nos resultados, todos indicam diferenças estatisticamente significativas na atividade encefálica associada a atenção em pacientes. Os achados neurobiológicos dos estudos incluídos na revisão sugerem a alocação de recursos da atenção a estímulos irrelevantes, independente da valência emocional no TOC.

Abstract The attentional bias is the allocation of attentional resources to irrelevant stimuli during a task. It is assumed that patients with obsessive-compulsive disorder (OCD) presents attentional bias toward threatening stimuli. In order to describe OCD’s neurobiological findings of attentional bias towards a threat, it was performed a systematic review of experimental studies with neurobiological assessment in the following databases: MEDLINE, Web of Science, Scopus and LILACS. Four studies with control group are described in this review, all presenting statistically significant differences in brain activity associated with attention in patients. The neurobiological findings of the studies included in the review suggest allocation of attentional resources to irrelevant stimuli, regardless of the emotional valence in OCD.

Resumo Introdução Pensamentos intrusivos podem causar um nível significativo de desconforto, uma vez que alguns indivíduos avaliam o conteúdo desses pensamentos como aversivos. A supressão dos pensamentos intrusivos é uma das estratégias adotadas por esses indivíduos, resultando no aumento do nível de desconforto. Supressão de pensamentos pode estar presente como um dos fatores de manutenção de diferentes transtornos psicológicos. Objetivos Descrever a tradução e adaptação para o português brasileiro do White Bear Suppression Inventory (WBSI), que avalia a supressão de pensamentos. Métodos Três traduções e retrotraduções foram conduzidas por avaliadores independentes. A equivalência semântica foi analisada e as três versões foram avaliadas para produzir uma versão preliminar. Os comentários dos participantes sobre a versão preliminar do questionário foram examinados. Resultados Foi desenvolvida uma versão final em português brasileiro do WBSI. Verificou-se que a maioria dos participantes entendeu as descrições das cognições apresentadas no questionário traduzido. Conclusão O uso de três versões diferentes de traduções e retrotraduções, discussão da versão preliminar e interação com a população-alvo conferiu viabilidade ao processo de equivalência semântica da versão final brasileira da WBSI.

Abstract Introduction Intrusive thoughts may cause a significant level of distress, since some individuals evaluate the content of those thoughts as aversive. Suppression of intrusive thoughts is one of the strategies adopted by these individuals, which increases the level of distress. Suppression of thoughts may be present as one of the factors that maintain different psychological disorders. Objective To describe the translation and adaptation into Brazilian Portuguese of the White Bear Suppression Inventory (WBSI), which measures thought suppression. Methods Three translations and back-translations were conducted by independent evaluators. Semantic equivalence was assessed and the three versions were evaluated to produce a synthesized version. Comments from participants on the preliminary version of the questionnaire were examined. Results We developed a preliminary Brazilian Portuguese version of the WBSI. It was found that most participants understood the descriptions of cognitions given in the translated questionnaire. Conclusion The use of three different versions of translations and back-translations, discussion of the synthetic version and interaction with the target population have conferred viability to the process of semantic equivalence of the Brazilian Portuguese final version of the WBSI.

Objective: Major depressive disorder (MDD) is a prevalent psychiatric condition characterized by multiple symptoms that cause great distress. Uncovering the brain areas involved in MDD is essential for improving therapeutic strategies and predicting response to interventions. This systematic review discusses recent findings regarding cortical alterations in depressed patients during emotional or cognitive tasks, as measured by electroencephalography (EEG). Methods: A search of the MEDLINE/PubMed and Cochrane databases was carried out using the keywords EEG and depression, confined to article title. Results: The studies identified reveal the frontal cortex as an important brain structure involved in the complex neural processes associated with MDD. Findings point to disorganization of right-hemisphere activity and deficient cognitive processing in MDD. Depressed individuals tend to ruminate on negative information and respond with a pattern of relatively higher right frontal activity to emotional stimuli associated with withdrawal and isolation. Conclusion: Patients with MDD may have altered dynamic patterns of activity in several neuroanatomical structures, especially in prefrontal and limbic areas involved in affective regulation. Identification of these alterations might help predict the response of patients to different interventions more effectively and thus maximize the effects both of pharmacotherapeutic and of psychotherapeutic strategies.

RESUMO Hipóteses neurobiológicas sobre o Transtorno Obsessivo-Compulsivo (TOC) sugerem alterações funcionais e anatômicas em determinadas áreas cerebrais relacionadas aos sintomas. Evidências indicam que a Terapia Cognitivo-Comportamental (TCC) é eficaz para o tratamento do TOC e capaz de modular padrões neurais disfuncionais. O presente estudo objetivou descrever as alterações neurobiológicas promovidas pela TCC para o TOC. Realizou-se uma revisão sistemática, cuja amostra final correspondeu a cinco estudos. Em todos, houve melhora significativa do TOC e alterações neurobiológicas após a TCC, tais como redução da atividade e volume no córtex orbitofrontal, aumento da atividade no córtex cingulado anterior, tálamo e núcleo caudado. A TCC mostrou-se capaz de modular neurocircuitos envolvidos no TOC.

ABSTRACT Neurobiological theories about Obsessive-Compulsive Disorder (OCD) signalize functional and anatomical changes in specific areas of the brain related to its symptoms. Evidence indicates that Cognitive-Behavioral Therapy (CBT) is effective in the treatment of OCD as well as capable of modulating dysfunctional neural patterns. This study aimed at describing neurobiological changes derived from the treatment of OCD with the CBT. A systematic review was carried out and its final sample consisted of five studies. In all of them, there was significant improvement of OCD as well as neurobiological changes, after CBT. The main findings were reduced activity and volume in the orbitofrontal cortex and increased activity in the anterior cingulate cortex, thalamus and caudate nucleus. CBT has shown to be able to modulate the neural circuits involved in OCD.

Alguns estudos relataram a importância da eletroencefalografia (EEG) como método de investigação de parâmetros anormais em transtornos psiquiátricos. Achados diferentes na análise do domínio do tempo e da frequência em relação à excitabilidade do sistema nervoso central durante estados agudos de pânico já foram obtidos. O objetivo deste estudo foi revisar sistematicamente os achados de EEG no transtorno do pânico (TP), discuti-los com base em uma hipótese neuroanatômica atualmente aceita para essa patologia e identificar limitações nos estudos selecionados. A pesquisa bibliográfica foi realizada nas bases de dados PubMed e ISI Web of Knowledge, utilizando as palavras-chave eletroencefalografia e transtorno do pânico; 16 artigos foram selecionados. Apesar da inconsistência dos achados de EEG no TP, as principais conclusões sobre a potência absoluta das bandas alfa e beta apontam para uma diminuição da potência em alfa, enquanto em beta a potência tende a aumentar. Diferentes padrões de assimetria foram encontrados entre estudos. Os estudos de coerência apontaram para um menor grau de conectividade funcional inter-hemisférica na região frontal e intra-hemisférica na região temporal bilateral. Estudos de potenciais eventos relacionados demonstraram mudanças no processamento da memória em pacientes com TP quando expostos a estímulos aversivos. Notou-se que a maioria dos resultados reflete a atual hipótese neurobiológica do TP, nos quais déficits inibitórios do córtex pré-frontal em relação à modulação da atividade da amígdala, e a subsequente ativação de regiões subcorticais, podem ser responsáveis por desencadear respostas de ansiedade. Foram abordadas algumas questões importantes que precisam ser consideradas em futuras pesquisas, especialmente o uso de diferentes métodos de análise de sinais de EEG. Some studies have reported the importance of electroencephalography (EEG) as a method for investigating abnormal parameters in psychiatric disorders. Different findings in time and frequency domain analysis with regard to central nervous system arousal during acute panic states have already been obtained. This study aimed to systematically review the EEG findings in panic disorder (PD), discuss them having a currently accepted neuroanatomical hypothesis for this pathology as a basis, and identify limitations in the selected studies. Literature search was conducted in the databases PubMed and ISI Web of Knowledge, using the keywords electroencephalography and panic disorder; 16 articles were selected. Despite the inconsistency of EEG findings in PD, the major conclusions about the absolute power of alpha and beta bands point to a decreased alpha power, while beta power tends to increase. Different asymmetry patterns were found between studies. Coherence studies pointed to a lower degree of inter-hemispheric functional connectivity at the frontal region and intra-hemispheric at the bilateral temporal region. Studies on possible related events showed changes in memory processing in PD patients when exposed to aversive stimuli. It was noticed that most findings reflect the current neurobiological hypothesis of PD, where inhibitory deficits of the prefrontal cortex related to the modulation of amygdala activity, and the subsequent activation of subcortical regions, may be responsible to trigger anxiety responses. We approached some important issues that need to be considered in further researches, especially the use of different methods for analyzing EEG signals. Panic disorder (PD) is a pluridimensional condition that leads to psychological suffering. Due to advances in neuroimaging techniques, important contributions have been made in the understanding of the neurobiological basis of PD. However, because of diverging research designs and protocols, more conclusive data concerning the neurocircuitry of PD remain difficult to achieve. To address this issue, a bibliographical search was performed using the Institute for Scientific Information Web of Science and Medline/PubMed databases. Fifteen articles were found, and their research methodology including sample, comorbidity, gender, and pharmacological criteria were explored. Although current functional magnetic resonance imaging studies of PD constitute fundamental tools for health sciences, more uniform research protocols must be implemented to provide more consistent and conclusive data concerning the neural substrates of PD.

OBJETIVO: Estudos recentes sugerem que uma psicoterapia estruturada aplicada junto com a farmacoterapia pode alterar o curso do transtorno afetivo bipolar. Entretanto, poucos estudos investigam os resultados da terapia cognitivo-comportamental em grupo sobre este transtorno psiquiátrico. O objetivo desta pesquisa foi avaliar a eficácia de 14 sessões de terapia cognitivo-comportamental em grupo concomitante à farmacoterapia para bipolares e comparar com a farmacoterapia sozinha. MÉTODO: Quarenta e um pacientes com transtorno bipolar I e II participaram do estudo e foram alocados aleatoriamente para um dos dois grupos; trinta e sete preencheram todas as escalas. Os sintomas de humor e ansiedade de todos os participantes foram acessados. A análise estatística foi utilizada para investigar se os grupos diferiam com relação aos dados demográficos e entre os escores pré-, durante e pós-tratamento (intra/intergrupos). RESULTADOS: Os participantes dos dois grupos mostraram-se similares nas características demográficas. A adição da terapia cognitivo-comportamental em grupo ao tratamento farmacológico foi efetiva. O grupo da terapia cognitivo-comportamental em grupo apresentou menos sintomas de mania, depressão e ansiedade, bem como uma redução na frequência e duração dos episódios de humor. CONCLUSÃO: As sessões de terapia cognitivo-comportamental em grupo foram especialmente importantes na melhora dos sintomas depressivos.

OBJECTIVE: Recent studies suggest that, when combined with pharmacotherapy, structured psychotherapy may modify the course of bipolar disorder. However, there are few studies that have examined the effects of cognitive behavioral group therapy on the course of this disorder. The aim of the present study was to evaluate the effectiveness of 14 sessions of cognitive behavioral group therapy, combined with pharmacotherapy, on the treatment of patients with bipolar disorder, and to compare our results against those from the use of pharmacotherapy alone. METHOD: Forty-one patients with bipolar I and II disorder participated in the study and were randomly allocated to one of two treatment groups; thirty-seven patients remained in the study until its completion. Mood and anxiety symptoms were measured in all subjects. Statistical analysis was used to investigate if the groups differed with respect to demographic characteristics and the scores recorded in the pre- and post-treatment stages, as well as during treatment (intra/inter groups). RESULTS: Patients showed statistically similar population characteristics. The association of cognitive behavioral group therapy and pharmacological treatment proved to be effective. Patients who had undergone cognitive behavioral group therapy presented fewer symptoms of mania, depression and anxiety, as well as fewer and shorter mood change episodes. CONCLUSION: Cognitive behavioral group therapy sessions substantially contributed to the improvement of depression symptoms.

INTRODUÇÃO: O medo de dirigir pode restringir a vida do indivíduo e gerar sérios problemas pessoais, sociais e ocupacionais. Usar um instrumento adequado para a avaliação de cognições do paciente é importante, já que contribui para uma formulação dos problemas apresentados, podendo guiar um plano de tratamento coerente. O objetivo deste estudo foi apresentar as etapas de tradução e adaptação para a língua portuguesa do instrumento Driving Cognitions Questionnaire, que mede as cognições que podem estar presentes no medo de dirigir MÉTODOS: Três traduções e três retrotraduções foram realizadas por seis avaliadores independentes. Realizou-se a equivalência semântica e avaliação das versões, elaborando-se uma versão síntese. Comentários dos 10 participantes sobre a versão preliminar do questionário foram examinados. Foi construída a versão brasileira preliminar do questionário. RESULTADOS: A maioria dos participantes entendeu as descrições das cognições expostas no questionário em português. CONCLUSÃO: A utilização de três versões de tradução e retrotradução, a discussão sobre a versão síntese preliminar e a interlocução com a população-alvo proporcionaram viabilidade ao processo de equivalência semântica da versão final brasileira.

INTRODUCTION: The fear of driving may restrict the life of patients and cause serious personal, social, and occupational problems. It is important to use a suitable tool while assessing patients’ cognitions, once these findings may help formulate the problems observed and may guide the selection of a consistent treatment plan. The objective of this study was to describe the stages of translation and adaptation into Brazilian Portuguese of the Driving Cognitions Questionnaire, designed to measure cognitions that may be present in the fear of driving. METHODS: Three translations and three back translations were conducted by six independent evaluators. Semantic equivalence assessment and evaluation of the different versions were conducted and resulted in a synthesized version of the instrument. Comments made by 10 participants on the preliminary version of the questionnaire were examined. The preliminary Brazilian version of the questionnaire was developed. RESULTS: Most participants understood the cognitions as described in the Brazilian Portuguese version of the questionnaire. CONCLUSION: The use of three different translations and back translations, discussion of a preliminary synthetic version and interaction with the target population contributed to the viability of the process aimed to assess the semantic equivalence of the Brazilian final version.

Contexto: O estudo de cybersickness, sintomas desconfortáveis relacionados à interação em ambientes virtuais, é importante para a implementação de melhorias destes, para ajudar a preservar o bem-estar dos usuários e reduzir o abandono às exposições virtuais. Usar um instrumento adequado para identificar e mensurar os sintomas de cybersickness, de forma padronizada, pode contribuir para essa finalidade. Objetivo: Este estudo tem como objetivo apresentar as etapas de tradução e adaptação para a língua portuguesa do instrumento Simulator Sickness Questionnaire, que mede os sintomas de cybersickness. Background: The study of cybersickness, uncomfortable symptoms related to interaction in virtual environments, is important for these environments’ improvement that help to preserve the welfare of users and reduce the abandonment of virtual exposures. Using an appropriate instrument to identify and measure the symptoms of cybersickness in a standardized way can contribute to this purpose. Objective: The objective of the current study is to present the stages of translation and adaptation into Portuguese of the instrument "Simulator Sickness Questionnaire", which measures cybersickness’ symptoms. Methods: Three translations and back translations were conducted by independent evaluators, the semantic equivalence and versions’ evaluation were made, producing a synthesized version. Um crescente número de pesquisas têm surgido sobre a aplicação da terapia de exposição por realidade virtual (VRET) para transtornos ansiosos. O objetivo deste estudo foi revisar algumas evidências que apoiam a eficácia da VRET para tratar fobia de dirigir. Os estudos foram identificados por meio de buscas computadorizadas (PubMed/Medline, Web of Science e Scielo databases) no período de 1984 a 2007. Alguns achados são promissores. Índices de ansiedade/evitação caíram entre o início e o fim do tratamento. VRET poderia ser um primeiro passo no tratamento da fobia de dirigir, uma vez que pode facilitar a exposição ao vivo, evitando-se os riscos e elevados custos dessa exposição. Entretanto, mais estudos clínicos randomizados/controlados são necessários para comprovar sua eficácia.

A growing number of researches has appeared on virtual reality exposure therapy (VRET) to treat anxiety disorders. The purpose of this article was to review some evidences that support the VRET efficacy to treat driving phobia. The studies were identified through computerized search (PubMed/Medline, Web of Science, and Scielo databases) from 1984 to 2007. Some findings are promising. Anxiety/avoidance ratings declined from pre to post-treatment. VRET may be used as a first step in the treatment of driving phobia, as long as it may facilitate the in vivo exposure, thus reducing risks and high costs of such exposure. Notwithstanding, more randomized/controlled clinical trials are required to prove its efficacy.

CONTEXTO: A terapia cognitivo-comportamental é a modalidade psicoterapêutica mais estudada no transtorno de pânico (TP) e demonstrou, nos mais diferentes estudos experimentais, ser bastante eficaz, inclusive quando comparada a grupos-controle, tratamentos psicofarmacológicos e outras formas de psicoterapias não estruturadas. A literatura sobre o tratamento cognitivo-comportamental do TP contempla comparações entre as técnicas mais utilizadas nesse tipo de psicoterapia. OBJETIVOS: Revisar a literatura que compara a eficácia dos enfoques cognitivo, comportamental e cognitivo-comportamental do TP e que destaca as limitações referentes a esses tratamentos psicológicos e às metodologias de pesquisa aplicadas. MÉTODOS: Revisão sistemática da literatura, principalmente por meio da base de dados PubMed. RESULTADOS: Verificou-se que, na maioria dos estudos, a eficácia das intervenções cognitiva, comportamental e de sua combinação equiparou-se e hipóteses a esse respeito foram levantadas. CONCLUSÕES: Deve-se atentar para o aprimoramento de importantes aspectos metodológicos, visando a resultados cada vez mais fidedignos que possam apontar direcionamentos que contribuam para o refinamento das técnicas utilizadas.

BACKGROUND: Cognitive-behavioral therapy is the most studied psychotherapeutic modality in panic disorder (PD) and has demonstrated, in the most different experimental studies, to be sufficiently efficient when compared with control groups, psychopharmacological treatments and other forms of unstructured psychotherapies. Literature on the cognitivebehavioral treatment of PD contemplates comparisons between the most used techniques. OBJECTIVES: Revise the literature that compares the effectiveness of the cognitive, behavioral and cognitive-behavioral approaches of PD, detaching the referring limitations to these psychological treatments and the applied research methodologies. METHODS: Literature systematic review, mainly revised in PubMed database. RESULTS: The effectiveness of the cognitive and behavioral interventions and its combination were equalized and hypotheses to this respect have been raised. DISCUSSION: We should attempt to improve important methodological aspects aiming at results even more trustworthy, pointing out directions to contribute to the refinement of the used techniques.

O uso de realidade virtual (RV) é útil no tratamento de exposição em pacientes com transtorno de pânico (TP). Em ambientes virtuais, os pacientes experimentam alterações fisiológicas, psíquicas e somáticas similares aos sintomas em situações da vida real, facilitando o processo de habituação. OBJETIVO: Divulgar o panorama atual do uso de RV no tratamento do TP. MÉTODO: A literatura sobre o tratamento do TP a partir da RV foi revisada com metodologia sistemática na base de dados PubMed, por meio das palavras-chave: virtual reality e panic disorder. RESULTADOS: O tratamento do TP com RV está em geral associado a bons resultados e à boa aceitabilidade por parte dos pacientes. São poucos os estudos que mensuram respostas fisiológicas durante a exposição, apesar da importância destes dados para avaliação da eficácia do tratamento. CONCLUSÕES: Apesar da utilidade do tratamento do TP por RV ser verificada nos estudos, constata-se necessidade de estudos controlados e protocolos de tratamento padronizados.

Virtual reality (VR) is a valuable tool in exposure treatment of panic disorder (PD). In virtual environments the patients experience similar physiological, psychic and somatic changes as in real life situations facilitating the habituation process. OBJECTIVE: We intend to divulge the current panorama of VR in treatment of PD. METHOD: The literature on VR treatment for PD was revised with a systematic methodology in the PubMed database, using the keywords: virtual reality and panic disorder. RESULTS: Virtual exposure treatment of PD usually shows good results and a great patient acceptability. Only a few studies measure physiological responses during the exposure despite the importance of these data for evaluation of the treatment effectiveness. CONCLUSIONS: Although the VR treatment for PD usefulness is clear in the trials, a lack of controlled studies and standardized treatment protocols are verified.
