RESUMO Objetivo: Analisar o excesso de óbitos no município do Rio de Janeiro (MRJ), RJ, durante a pandemia de COVID-19 (março de 2020 a janeiro de 2022). Métodos: Foi realizado um estudo ecológico com dados secundários do Sistema de Informação sobre Mortalidade cuja unidade de análise foi o MRJ. O excesso de mortalidade foi calculado pela diferença entre a média de óbitos esperados e a média dos óbitos observados levando-se em conta o período de 2015 a 2019. Foi ajustado um método de regressão quantílica. Calculou-se o valor total dos casos acima do esperado pela série histórica. Foram selecionados os óbitos por causa básica COVID-19 e Influenza. Também foi calculada a razão entre o excesso de óbitos e os óbitos atribuídos à COVID-19. Resultados: Foi identificado excesso de 31.920 óbitos pela média (26,8% de incremento). Pela regressão quantílica, encontrou-se excesso de 31.363 óbitos. Ocorreram 33.401 óbitos por COVID-19 e 176 por Influenza. A razão entre o excesso de óbitos encontrado e os óbitos atribuídos à COVID-19 foi de 0,96 pela média e 0,95 pela regressão quantílica. Conclusão: O estudo apontou grande excesso de óbitos durante a pandemia de COVID-19 no MRJ, distribuído em ondas, incluindo-se o período do surto de Influenza.

The ratio between the verified excess mortality and deaths due to COVID-19 was 0.96 by the mean and 0.95 by the regression. Conclusion: The study pointed to large excess deaths during the COVID-19 pandemic in the city of Rio de Janeiro distributed in waves, including the period of the Influenza outbreak.

ABSTRACT Sialolithiasis is a common nonneoplastic disease of the major salivary glands that often affects the submandibular glands. Minor salivary gland involvement by sialolithiasis is uncommon, with only 273 cases reported. A long clinical history, acute symptoms, and mucopurulent discharge are unusual features of these cases. Herein, we report the case of a 63-year-old woman who complained of symptomatic nodular swelling of the buccal mucosa associated with purulent discharge for several days. The clinical history lasted 15 years, with episodes of asymptomatic non-suppurative swelling in the same area. The patient underwent surgical excision. The microscopic examination revealed chronic nonspecific sialadenitis associated with psammomatous calcifications, confirming minor salivary gland sialolithiasis. After 3 years of follow-up, the patient was free of symptoms. Patients with sialolithiasis are usually asymptomatic; however, swelling, pain, and fistula may be present in rare cases. The presence of purulent exudate should lead to the differential diagnosis of stomatitis glandularis, a rare inflammatory condition affecting the minor salivary glands. Sialolithiasis and stomatitis glandularis should be considered in the clinical differential diagnosis of symptomatic suppurative nodular swelling affecting the oral mucosa, and histopathological analysis is necessary for the diagnosis.

ABSTRACT Squamous odontogenic tumor (SOT) is a rare benign neoplasm of the jaw that likely arises from remnants of the dental lamina. It is a slow-growing lesion, with a radiolucent appearance in the central variant. Microscopically, SOT shows islands of squamous epithelium supported by fibrous stroma. In rare cases, squamous odontogenic tumor-like proliferation (SOT-LP) can be observed arising from odontogenic cysts (SOT-LPOC). Herein, we describe the case of a 42-year-old man who presented with discreet bleeding in the maxillary gingiva. Imaging revealed a well-defined, ovoid-shaped lesion with sclerotic margins involving tooth #18 in the intraosseous location. Fine needle aspiration supported the cystic nature of the lesion. After surgery, microscopy revealed a dentigerous cyst showing SOT-LP features. There was no recurrence after a 3-year follow-up. To the best of our knowledge, this is the first report of a dentigerous cyst showing SOT-LP features in the maxilla. Such cysts should be identified to avoid misdiagnosis, with the finding having therapeutic and prognostic implications.

OBJETIVO: avaliar a estabilidade de cirurgias bimaxilares com fixação interna rígida, na qual promoveu-se uma rotação anti-horária da mandíbula e conseqüente redução do ângulo do plano oclusal. METODOLOGIA: foram estudados 15 pacientes com padrão facial de Classe II e deficiência mandibular. Os movimentos cirúrgicos foram avaliados por meio de medidas lineares e angulares realizadas no programa CefX, obtidas de telerradiografias cefalométricas em norma lateral em três diferentes tempos: (T0) pré-operatório, (T1) pós-operatório imediato e (T2) pós-operatório de no mínimo 6 meses. RESULTADOS E CONCLUSÕES: os resultados demonstraram que a cirurgia bimaxilar promove mudanças faciais significativas e, principalmente, que a rotação anti-horária da mandíbula com redução do ângulo do plano oclusal mostrou-se estatisticamente estável, sendo uma opção confiável em cirurgias de pacientes com padrão facial de Classe II com deficiência mandibular.

AIM: to access the surgical stability combined with rigid internal fixation, where the counter-clockwise jaw rotation was carried out, and consequently the occlusal plane reduction. METHODS: fifteen Class II skeletal facial standard patients were studied. The surgical movements were assessed by linear measures and angles accomplished at the CefX program, collected from cephalometric teleradiography lateral norms in three different moments: (T0) presurgical, (T1) instantaneous postsurgical and (T2) postsurgical at least six months. RESULTS AND CONCLUSION: the results showed that the bimaxillary surgery causes meaningful facial changes and mainly, the counter-clockwise mandible rotation and consequently the occlusal plane angle reduction showed statistically stable, suggesting a reliable opinion in Class II standard facial surgeries with defective mandibular.

O objetivo deste trabalho foi analisar quantitativamente os microrganismos, Streptococcus do grupo mutans e Candida sp, da cavidade bucal de pacientes com carcinoma de orofaringe antes, durante e após o tratamento com radioterapia e correlacionar fatores salivares como pH, capacidade tampão (CT) e fluxo salivar (FS). Amostras de saliva foram coletadas, diluídas e inoculadas em ágar SB-20 e ágar Sabouraud, respectivamente para Streptococcus do grupo mutans e Candida sp. Previamente à diluição, a saliva concentrada foi analisada, determinando-se os fatores salivares. Após crescimento das colônias, o número de microrganismos foi determinado em UFC/ml. A análise dos resultados permitiu concluir que houve correlação positiva entre os fatores salivares e a presença de microrganismos ilustrada pelo aumento no número de UFC/ml dos microrganismos analisados concomitantemente com a diminuição do fluxo salivar. Os efeitos da radiação comprometeram a homeostasia salivar e favoreceram o aumento das infecções por leveduras e bactérias durante o tratamento radioterápico.

The aim of this study was to quantify the microorganisms Streptococcus mutans and Candida sp in the oral cavity of patients with oropharynx carcinoma, before, during and after radiotherapy, and to correlate the results with salivary factors such as pH, buffer capacity (CT) and flow rate (FS). Saliva samples were collected, diluted and inoculated in SB-20 agar and in Sabouraud agar, for Streptococcus mutans and Candida sp, respectively. Previously to dilution, the concentrated saliva was analyzed, and the salivary factors were determined. After the growth of colonies, the number of microorganisms was determined in CFU/ml. The analysis of the results allowed to conclude that the salivary factors are related to the presence of microorganisms, and that the number of CFU/ml increased as salivary flow rate decreased. The effects of radiation compromised salivary homeostasis and favored the increase of infection by yeasts and bacteria.
